[From *Chanbao*] When breast milk flows spontaneously before delivery, it is called "Ru Zhu" (Breast Leakage), and the newborn often fails to survive. When breast milk flows spontaneously after delivery, it is due to body deficiency; it is appropriate to take tonifying medicines to stop the flow.
If the breasts are distended, urgent, painful, and leaking milk, apply hot cloth compresses (Wen Bo Yun) to the area; Loulu San (Descurainia Powder) may also be used.
Loulu (Radix Descurainiae, 2.5 qian) Shetui (Ophisthotropis Peripatus Skin, 1 strip) Gualou (Fructus Trichosanthis, 10 fruits)
Grind the above into powder, mix with wine to take 2 qian, at any time.
[Zhu Danxi] A woman in her thirties, two days after giving birth to a daughter, had an object protruding from the vaginal opening resembling a handkerchief hanging down, with a pointed tip, weighing approximately one jin. I reasoned that this was caused by Qi deficiency resulting from fatigue during pregnancy, leading to Gan Wei (Liver Flaccidity). Fortunately, blood deficiency was not severe. At that time, it was late in the year and cold; fearing cold invasion might damage the tissue, I urgently prescribed Zhi Huangqi (Prepared Radix Astragali, 0.5 qian), Renshen (Radix Ginseng, 1 qian), Baizhu (Rhizoma Atractylodis Macrocephalae, 0.5 qian), Danggui (Radix Angelicae Sinensis, 1.5 qian), and Shengma (Rhizoma Cimicifugae, 0.5 qian). After taking three doses consecutively, the prolapse retracted. The patient sweated all over and then felt secure. However, a piece of tissue adhered to the bedsheet where the lower body touched had dried and fallen off, weighing about five or six liang; this was adipose membrane. Subsequently, her appetite improved, she slept well, and her pulse was wiry-rough (Se) generally, slightly wiry on the left. Her physical constitution appeared solid. I prescribed Baizhu (Rhizoma Atractylodis Macrocephalae) and Shaoyao (Radix Paeoniae Alba), each 1.5 qian, Chenpi (Pericarpium Citri Reticulatae, 1 qian), and Shengjiang (Rhizoma Zingiberis Recens, 1 slice), decocted for two or three doses to nourish her.
Another woman, after childbirth, had an object protruding from the vagina resembling a lidded bowl, bifurcated into two parts. Her husband sought treatment. I reasoned that this was uterine prolapse (Zigong Xia Tuo), likely due to weak Qi and Blood causing sinking. Thus, I prescribed Shengma (Rhizoma Cimicifugae), Danggui (Radix Angelicae Sinensis), and Huangqi (Radix Astragali) in large doses for two packets. Half a day later, the husband returned saying: "After taking two doses, there was a popping sound, and upon inspection, it had retracted into the vaginal opening. However, because it had been exposed to air overnight against the mat, a piece the size of a palm broke off and remained on the mat. My wife is crying at home, fearing intestinal rupture which cannot be healed." I reasoned that this was not intestine but necrotic tissue (Cao Po); muscle tears can heal, and if Qi and Blood become abundant, the tissue will regenerate. I prescribed Siwu Tang (Four Substance Decoction) added with Renshen (Radix Ginseng) for one hundred doses. Three years later, she gave birth again.
For treating uterine prolapse, use Huangqi (Radix Astragali, 1.5 qian), Renshen (Radix Ginseng, 1 qian), Danggui (Radix Angelicae Sinensis, 0.7 qian), Shengma (Rhizoma Cimicifugae, 0.3 qian), and Gancao (Radix Glycyrrhizae, 0.2 qian) as one dose. Add one zhong of water, decoct until one-third remains, strain, and take before meals. Then wash the affected area with a decoction of Wubeizi (Galla Chinensis) powder and apply the powder topically. Repeat several times; continuous medication will prevent recurrence permanently.
Treating Yin Ting (Prolapse of the uterus or vaginal walls): This is often caused by excessive straining during labor, resulting in prolapse resembling rectal prolapse, with protrusion near the anus accompanied by swelling and pain. Heavy lifting and sexual exhaustion can trigger it. Clear fluid flows continuously, and urination is dribbling. Use Liuhuang (Sulfur) and Wuzhegu (Cutula Sepiae), each half liang, and Wuweizi (Fructus Schisandrae), one fen, ground into powder and applied to the lesion. Concurrently administer tonics such as Renshen (Radix Ginseng), Huangqi (Radix Astragali), Danggui (Radix Angelicae Sinensis), Gancao (Radix Glycyrrhizae), and Shengma (Rhizoma Cimicifugae).
Treating postpartum genital prolapse. Burn human feces to ash, take one cunbi (square inch measure) with warm wine, once daily. (*Qianjin Fang*, Wuxie Xiepi San added with Bie head, roasted, also treats this. See Rectal Prolapse section.)
[From *Chanbao*] Treating postpartum vulvar swelling and prolapse with inability to close the Yumen (vaginal introitus). Use Duanshi (Calcined Iron Oxide), one sheng, stir-fried until extremely hot, pour into two sheng of boiling water, let cool to lukewarm, clarify, and sit in the liquid to soak the Yumen. It will recover to normal shortly. Another method: Apply Tiejing (Iron Rust) powder internally by pushing it in, and moxa-burn seven times (two sets of seven burns = 14 burns) below the umbilical transverse crease. Another method: Mix Tiejing (Iron Rust) and Yangzhi (Mutton Fat), stir until thick, wrap in cloth, heat, and apply as a hot compress while pushing it inside. Tiejing refers to dust-like particles flying from the forge, purple in color, light and porous, used for polishing vessels. Another method: Stir-fry one sheng of Shechuangzi (Fructus Cnidii) alone, wrap in cloth while hot, and apply as a warm compress to the affected area; this also treats postpartum vulvar pain.
[From *Zimu Milu*] Treating postpartum genital prolapse. Burn rabbit head to ash and apply to the site.
[Zhang Yuansu] For women with prolapsed vaginal introitus that does not retract, often damp: Shenque (CV8), Yuquan (Moxa 50 burns). Shenjiao (Located in the gap below the umbilicus, Moxa 50 burns, repeated three times).
Another method: Three cun lateral to Yuquan (Moxa, age-based burns, repeated three times). For cold swelling of the female genital opening: Guilai (ST29, Moxa 30 burns).
[From *Jiayi Jing*] For women with vaginal prolapse, lethargy in the four limbs, and bodily heaviness/stuffiness, Shaohai (HT3) is indicated. (Alternatively stated as Zhaohai, KI6.)
