[No] The treatment for stranguria (Lin syndrome) before and after childbirth differs. Before childbirth, the fetus should be anchored; after childbirth, blood stasis should be removed. Dianthus Herba (Qumai) and Polygoni Multiflori Caulis et Folium (Puhuang) are the most important herbs for postpartum conditions.
Maogen Decoction: Mainly treats various types of stranguria after childbirth.
Imperatae Rhizoma (Baimaogen) (8 liang), Dianthus Herba (Qumai), Abutilonis Fructus (Kuizi) (2 liang), Poria (Baifuling) (4 liang each), Ginseng Radix et Rhizoma (Renshen) (2 liang), Polygoni Multiflori Caulis et Folium (Puhuang), Pruni Mume Gum (Taojiao), Talcum (Huashi), Glycyrrhizae Radix et Rhizoma (Gancao) (1 liang each), Margaritifera Concha (Zibei) (10 pieces, calcined), Litorinae Concha (Shishouyu Zhen Shi) (appropriate amount).
The above are crushed into a powder. Each dose is 4 qian, decocted with 1.5 sheng of water, 3 slices of Zingiberis Rhizoma Recens (Jiang), and 20 stems of Junci Medulla (Dengxin), until reduced to 7 fen. Strain and take warm. It can also be made into powder and taken with Tongcao Decoction.
[Da] Treats postpartum urinary retention. Use Chenpi (Citri Reticulatae Pericarpium) peeled of white pith, ground into powder. Take on an empty stomach, mixed with wine, 2 qian per dose. Urination will resume after one dose.
To treat postpartum urinary retention with abdominal distension like a drum, and confusion/unconsciousness. Fill the umbilicus with salt. Peel the coarse skin off Scallion Bulb (Congbai), bundle about ten stalks together, cut into a thickness of one finger, place on the salt. Apply a large moxa cone on top of the scallion cake and moxibustion. When heat is felt entering the abdomen, urination will immediately resume; this is highly effective.
[Luo] Helehesun, Right Vice Director of the Secretariat, suffered from frequent but scanty urination, approximately twenty times day and night. His navel and intestines felt full, his waist and legs were heavy, and he could not sleep peacefully. In the late days of the third lunar month, one day he was ordered by imperial decree to be treated, so I went to examine him. The pulse was deep and slow, occasionally rapid. I often recall that there are three types of difficult urination, which cannot be generalized. If body fluids leak excessively into the intestines and stomach, causing diarrhea while urination is scanty and obstructed, this is the first type, suitable for diuresis alone. If heat binds with fluids in the Lower Jiao, causing damp-heat stagnation, this is the second type, requiring渗泄 (draining dampness and purging) to heal. If Spleen and Stomach Qi is stagnant, unable to regulate water passages and transport downward to the Bladder for transformation, this is the third type; regulating Qi to facilitate transformation and excretion is appropriate. Now, the Vice Director has accumulated damp-heat from rich food internally due to habit, preventing normal circulation, leading to orifice obstruction in the Bladder. Hence, frequent but scanty urination occurs. Without渗泄分利 (draining dampness and separating turbidity), smooth urination cannot be achieved. Thus, I prescribed a formula named Fuling Hupo Tang (Poria and Amber Decoction). The *Neijing* states: Sweet flavors soothe and bland flavors drain dampness. Heat binding with fluids causes internal accumulation and abdominal distension, requiring soothing. Draining heat must primarily use sweet and bland substances. Therefore, Poria (Fuling) is used as the Sovereign herb; its sweet, cold, and slippery nature unblocks orifices and relieves obstruction. Polyporus (Zhuling) and Amber (Hupo) have bland properties to drain and promote water passage, serving as Ministers. The Spleen dislikes dampness; internal dampness stagnation leads to Spleen Qi discomfort. To benefit the Spleen and overcome dampness, sweet flavors assist, so Licorice (Gancao) and Atractylodis Macrocephalae Rhizoma (Baizhu) serve as Assistants. Salty flavors enter the Kidney; salty flavors induce vomiting and purging and belong to Yin. Thus, Alisma Rhizoma (Zexie) is used for its salty flavor to drain retained water. The Kidney dislikes dryness; urgent consumption of pungent flavors moistens it. When fluids do not circulate, pungent flavors disperse them. Cinnamon Twig (Guizhi) is pungent, dispersing dampness and moistening dryness, serving as the Guide. Decoct in "Ganlan Water" (water stirred vigorously for a long time) to prevent aiding Kidney Qi excessively. Prepare as a strong decoction to direct the medicine rapidly to the lower part. After two doses, symptoms halved; within ten days, complete recovery.
Fuling Hupo Tang (Poria and Amber Decoction)
Poria (Fuling), Atractylodis Macrocephalae Rhizoma (Baizhu), Amber (Hupo) (each 0.5 liang), Licorice (Gancao) (roasted), Cinnamon Twig (Guizhi) (peeled) (each 3 qian), Alisma Rhizoma (Zexie) (1 liang), Talcum (Huashi) (7 qian), Polyporus (Zhuling) (0.5 liang).
Grind into fine powder. Each dose is 5 qian, taken with 1 sheng of prepared Ganlan Water, on an empty stomach before meals. Wait a moment and then eat a good meal to settle the medicine.
[Shi] Mainly treats Lower Jiao deficiency-cold with frequent, weak urination. Raw Dioscoreae Rhizoma (Shanyao) 0.5 jin, peeled and chopped. Boil in wine in a pan, add Shanyao without stirring. When cooked, add salt, Scallion Bulb (Congbai), and more wine. Drink 2-3 cups on an empty stomach; excellent.
[Luo] Shuizhi Wan (Water Lotus Pill): Treats Upper Jiao true Qi deficiency, frequent urination without control day and night.
Lotus Plumule (Lianrou) (peeled, quantity unspecified, soaked in good wine for one night). One Pig Stomach (Zhudu). Place the wine-soaked lotus seeds into the stomach until half-full. Boil in water until cooked, remove lotus seeds, slice, and dry-roast.
Grind into fine powder. Make pills using wine-boiled flour paste, size of Euphorbia Seed (Qianshi). Take 50 pills per dose with Rice Decoction (Miyin Tang) before meals.
[Shi] Treats nocturnal polyuria. Use Alpinia Oxyphylla Fruit (Yizhiren) 34 pieces, ground into powder. Add 5 fen of Salt. Decoct with 1 sheng of water until 0.8 sheng remains. Take warm before bed.
[Ben] Weizhen Tang: Treats men and women with depleted Yuan Qi, weak Ying and Wei, insecure True Yang, disharmony of the Three Jiao, excess above and deficiency below, nocturnal emissions/dreams of sexual intercourse, morning sweating, lackluster complexion, dry lips/mouth/tongue, tinnitus like cicadas, lower back pain and fatigue, Heart Qi deficiency, mental restlessness, palpitations, forgetfulness, loss of taste, gradual emaciation, external genital itching/dampness, nocturnal polyuria, swelling/heavy/cold/painful sensation radiating to the lower abdomen, weak knees/legs, difficulty walking; women with chronic cold in the Blood Sea, irregular menstruation, delayed menses, twice-monthly cycles, leukorrhea (red/white or five colors), uterine cold infertility. All are treated. This medicine greatly generates Qi and Blood. At midnight (Zi hour) when Kidney Water is at its peak, it tonifies the Kidney and solidifies the viscera. For men, it secures blood and transforms essence. Before diseases arise, it controls them, preventing recurrence.
Angelica Sinensis Radix (Danggui) (1 liang, soaked in wine overnight), Ginseng Radix et Rhizoma (Renshen) (1.5 liang), Dendrobii Nobile Caulis (Jinchai Shihu), Poria (Baifuling), Aucklandiae Radix (Muxiang), Myristica Semen (Roudoukou), Dioscoreae Rhizoma (Shanyao) (above each 3 liang), Rehmanniae Radix (Shengdi) (2 liang), Prepared Rehmanniae Radix (Shudi) (washed with warm water, 3 liang), Eugeniae Flos (Dingxiang) (1 liang), Citri Unshiu Pericarpium (Qingpi) (1 liang, peeled), Cyathulae Radix (Chuan Niuxi) (2 liang, soaked overnight in child's urine and wine).
Grind into fine powder. Each dose is 3 large sheng, taken with wine (salt is acceptable), on an empty stomach before meals. For women's diseases, mix with child's urine and wine, take on an empty stomach.
Another Prescription: Treats all deficiency-cold diseases in men and women, invigorates blood, preserves youth, reduces urination, stops night sweats. Treats women who have not given birth for a long time, resembling leukorrhea but not, with occasional dribbling. All are treated. Effects are too numerous to list.
Atractylodis Rhizoma (Cangzhu) (sliced and roasted), Toosendan Fructus (Chuanlianzi), Foeniculum Vulgare Fructus (Huixiang), Evodiae Fructus (Wuzhuyu) (washed with hot water), Psoraleae Fructus (Poguzhi), Phaseoli Semen (Huluba) (above each 1 liang, stir-fried), Ginger (Chuanjiang), Aconiti Lateralis Radix Praeparata (Chuanwu), Aconiti Radix (Caowu) (above each 0.5 liang, processed), Dioscoreae Rhizoma (Shanyao) (2 liang).
Process each according to method, grind into fine powder, make pills with vinegar paste, size of Sesame seed. Take 15 pills per dose on an empty stomach with warm wine/salt water. Women take with Mugwort Vinegar Decoction. Twice daily. Eyes and ears will never become dim/deaf, hair will not turn white.
[Yan] A neighbor's man had urinated dozens of times a day, urine like thick rice washings, mental confusion, emaciation, poor appetite. Prescribed Sangpiaoxiao San (Mantis Egg Case Powder). He recovered before finishing one dose. This medicine calms the Spirit, stabilizes the Will, treats forgetfulness, frequent urination, and tonifies Heart Qi. Use Mantis Egg Case (Sangpiaoxiao), Polygalae Radix (Yuanzhi), Acori Tatarinowii Rhizoma (Changpu), Dragon Bone (Longgu), Ginseng Radix et Rhizoma (Renshen), Poria (Fuling), Angelica Sinensis Radix (Danggui), Testudinis Carapax et Plastrum (Guijia) (vinegar-roasted). Above each 1 liang, ground into powder. Take 2 qian mixed with Ginseng Decoction.
[Hai] Jiangfu Shizhi Shensha Dan (Ginger, Aconite, Kaolin, Cinnabar Pill): Treats frequent, incontinent urination, palpitations, and forgetfulness. (Formula see under Dysentery.)
[Dan] Because I saw in *Lu Shi Yi Jing* that this prescription has the same ingredients and dosages, except for using calcined Cinnabar (Fuhuo Dansha), and the disease improved. The conditions treated were frequent, incontinent urination, palpitations, forgetfulness, and persistent nightmares, differing only slightly. Observing these differences and examining carefully, I found this treatment ineffective, so I discuss it generally. Classics state: Kidney governs large bowel movements; Liver governs stranguria and urination. *Nanjing* says: Small Intestine is the Red Intestine, manifesting as red face and red urine/feces, originating from Heart and Small Intestine, Southern Red color appearing externally. Classics say: Lower Jiao is like a ditch, referring to feces and urine. Feces are Yin, tangible Qi-transformation, located at the bottom of Lower Jiao. Saying Kidney governs feces, not Large Intestine, implies Son performing Father's function, indicating Kidney disease. Urine is Yang, intangible Qi-transformation, located at the top of Lower Jiao. Saying Liver governs stranguria/urination also implies Son performing Father's function, indicating Fu-organ disease. All intangible Qi-transformations are governed by Fu-organ diseases; all tangible Qi-transformations are governed by Zang-organ diseases. These two syndromes are both in Lower Jiao, but differ in tangibility and Zang/Fu location, both involving Dantian decline. Heart Fire is not mentioned because Ministerial Fire acts on behalf of Sovereign Fire. Specifically, Bladder Ren Water overcomes Bing Fire. Small Intestine does not transmit to Yin, hence bleeding. Bleeding and incontinence are tangible diseases. Not transmitting to Yin means Yin is not diseased. Why tangible disease? It is the substance of Yin, the function of Fu. Heaven and Earth Yin/Yang are mutually substance/function, visible here. Clearly, Five Zang are used by Six Fu, Six Fu are used by Five Zang. Tangible things all transmit to Yin. Incontinence of feces/urine: Bladder incontinence is enuresis, not transmitting to Yin. This is Dantian/Bao Luo (Uterus/Ligaments) receiving Cold, overcome by Ren. Generally, all Fu organs attack tangible things; tangible diseases are in Fu. Blaming the attacked factor lies in Zang. Using calcined Cinnabar removes cold nature. Treatments are similar because both are in Lower Jiao, tonifying intangibles, residing in Bao Luo. Since Bao faces Kidney and has channels connecting them. Kidney governs feces, Liver governs urine, how can their treatments be identical? Classics say: Kidney and Liver share one treatment. Also: Shaoyang governs bones, diseases produced include difficult urination. Bladder governs sinews, diseases produced...可知 (can be known). Incontinence of urine: take with Poria Decoction. Incontinence of feces: take with Rice Decoction. (This discussion originates from *Ju Fang* Jiangfu Shensha Dan.)
Influence of Yunqi (Six Qi), frequent urination belongs to Fire. Classics say: Return of Shaoyang, frequent urination and aversion to wind. Frequent urination involves only two Zang.
One belongs to Lung. Classics say: Hand-Taiyin Lung Meridian, Qi excess leads to shoulder/back pain, wind-cold sweating/stroke, frequent urination and yawning. Regarding acupuncture, draining Hand-Taiyin cures it.
One belongs to Liver. Classics say: Foot-Jueyin Malaria makes people feel like stranguria, with difficult urination. Also: Liver Bi, waking startled at night, excessive drinking, frequent urination. View deficiency/excess and apply tonification/draining to cure.
[Dong] Frequent urination with abdominal pain: Niaopao (Bladder acupoint, 1 cun below Yuquan, at the end of the curved bone.)
[Yu] Frequent urination: Mingmen (Moxibustion according to age years), Shenshu (1 fen, along skin outward 6 fen, tonify 6 breaths, drain 1 breath).
[Ji] Slippery/frequent urination: Zhongji (Moxibustion), Shen [Kidney?], Yinlingquan (If not cured, take lower acupoints), Qihai, Yinval, Sanyinjiao.
