Any patient with hemiplegia (hemiplegia) will inevitably present with deviation of the mouth and eyes (deviation of the mouth and eyes). There are also cases presenting with deviation of the mouth and eyes without hemiplegia; therefore, it is listed separately here.
Regarding the theory of Six Qi (Six Qi): Deviation of the mouth and eyes is mostly related to the Stomach Earth (Stomach Earth). When Wind Wood (Wind Wood) qi is insufficient, Metal (Metal) overcomes it, causing Earth to lack restraint (as Wood fails to control Earth, or Metal overcomes weak Wood and attacks Earth). The Classics state: "Insufficiency of Wood is called Weihe (Weihe)." In the era of Weihe, its pathological manifestations include spasms, flaccidity, or stiffness. It also states: "When Jueyin (Jueyin) Wind arrives, it causes shortening." "Shortening" refers to the contraction of tendons. When Wood qi is insufficient, the "shortening" characteristic of Metal invades to overcome the free-flowing and spreading nature of Wood. "Li" (Li) refers to deviation of the mouth and eyes; "Ju" (Ju) refers to tendon spasm and stiffness; "Huan" (Huan) refers to tendon flaccidity. Because Wood is pulled by Metal's shortening nature, it leads to deviation of the mouth and eyes with tendon spasm and stiffness. "Huan" implies tendon relaxation. When Wood is overcome by Metal, Earth lacks restraint, so Earth assumes the transformation of relaxation/flaccidity in the gaps, resulting in a mix of spasm and flaccidity. Thus, deviation of the mouth and eyes often belongs to Phlegm-Dampness in the Stomach Earth (Stomach Earth). The treatment method should use pungent-warm herbs to drain the shortening pathogen of Metal and resolve the Damp-Phlegm of Earth.
[Li Dongyuan] Qingyang Tang (Clear Yang Decoction): Treats tendon shortening that pulls and causes deviation of the mouth and eyes. (Formula see below)
[Meng Shen] For sudden wind invasion causing deviation of the mouth and inability to speak: Take one sheng of Evodiae Fructus (Wuzhuyu, 吴茱萸) and one sheng of clear wine, boil together for four or five boils, take half a sheng at a time, three times a day. Slight sweating indicates improvement.
[Qianjin Fang] To treat deviation of the mouth and eyes: Take three sheng of soybeans, stir-fry until charred, pour three sheng of good wine over them to extract the juice, and drink it all at once, once daily. To treat stroke causing facial muscles pulling each other, mouth deviating towards the ear, tight jaw, and immobile tongue:
Duhuo (Radix Angelicae Pubescentis, 独活) (3 liang)
Zhuli (Bamboo Sap, 竹沥)
Shengdi Huang (Radix Rehmanniae Preparata, 生地汁) (each 1 sheng)
Decoct the above three ingredients together to obtain two sheng of liquid, drink it all at once for recovery.
To treat sudden stroke with deviation of the mouth and eyes: Take one bean-sized amount of Kongqing powder (Azurite Powder, 空青末), hold it in the mouth for immediate recovery. The pathology of deviation of the mouth and eyes generally lies in the Stomach meridian, involving tendons and vessels. The Classics state: "The Foot-Yangming and Hand-Taiyang meridians, when their tendons are tense, cause deviation of the mouth and eyes, with eyes unable to look suddenly due to tension; this is Wind-Evil invading the tendons of Stomach Earth." The Classics also state: "The Foot-Yangming Stomach meridian runs alongside the mouth and around the lips; diseases generated here include deviation of the mouth and lips, which is Wind-Evil invading the meridians of Stomach Earth."
[Li Dongyuan] Qingyang Tang (Clear Yang Decoction): Treats deviation of the mouth and eyes, tightness of cheeks and jaws, excess fire in the Stomach, continuous sweating, and frequent urination.

Guizhi (Ramulus Cinnamomi, 桂枝) (1 qian)
Sheng Gancao (Raw Glycyrrhizae Radix et Rhizoma, 甘草) (0.5 qian)
Huangbai (Cortex Phellodendri, 黄柏) (1 qian, soaked in wine)
Crush the herbs. Prepare as one dose, decoct in three cups of wine until one cup and three fen remain, strain, and drink slightly warm before meals. Then apply heat massage to the tight area for recovery. Tendon tension in deviation of the mouth and eyes is caused by severe cold in the tendons and blood collaterals. This medicine acts somewhat like burning needle therapy (burning needle therapy) to break bad blood and remove coagulation, while internally draining the excessive fire of the Chong Mai (Penetrating Vessel).
[Luo Tianyi] Case of Wind Invading Blood Vessels: Grand Marshal Zhong Wushi, aged 68, in early October of the first year of Zhiyuan, was attending the State Preceptor in the study room of Sheng'an Temple. A coal stove was on his left. He felt facial heat and slight sweating on the left cheek. After the preceptor and others left, the left cheek became flaccid and was invaded by Wind-Cold, while the right cheek became tense, causing deviation of the mouth and eyes to the right. The pulse was floating and tight, but deep pressure revealed it to be surging and slow. I recommended Hu Jifu, the Medical Supervisor, who specialized in acupuncture and moxibustion. He first moxa-ed the Dicang acupoint (Dicang, ST4) on the left cheek seven moxa cones, then moxa-ed Jiache (Jiache, ST6) fourteen cones. He then applied hot hand massage to the right cheek. He prescribed Shengma Tang (Cimicifuga Decoction) added with Fangfeng (Radix Saposhnikoviae, 防风), Qinjiao (Radix Gentianae Macrophyllae, 秦艽), Baizhi (Radix Angelicae Dahuricae, 白芷), and Guizhi (Ramulus Cinnamomi, 桂枝) to disperse Wind-Cold. After several doses, he recovered. Someone asked: Most physicians use Xuming Tang (Continuation Life Decoction) etc., why did you use Shengma Tang with four additional herbs? He replied: The Foot-Yangming meridian starts from the nose, crosses the glabella, follows the outside of the nose, and enters the upper teeth. The Hand-Yangming meridian also passes through the lower teeth. Moreover, both cheeks belong to the Yangming meridians. Shengma Tang is a formula for the Yangming meridian; Baizhi circulates through the Hand-Yangming meridian; Qinjiao treats lockjaw; Fangfeng disperses Wind-Evil; Guizhi consolidates the exterior and secures Ying and Wei (Nutritive and Defensive Qi), preventing further injury by pathogens. This is the rationale. Diseases have distinctions in root/branch and meridians; medicines have differences in flavor intensity. Investigating the source of the disease and choosing appropriate medication yields effects as swift as a drumstick hitting a drum. Ignoring meridian pathways and drug properties, sticking rigidly to one formula, is not only useless but harmful. Students should reflect carefully.
Qinjiao Shengma Tang (Gentian and Cimicifuga Decoction): Treats stroke affecting the Hand and Foot Yangming meridians, causing deviation of the mouth and eyes, limb spasms, and aversion to Wind-Cold.
Shengma (Rhizoma Cimicifugae, 升麻)
Gegen Gen (Puerariae Radix Dry Root, 干葛根)

Baizhi (Radix Angelicae Dahuricae, 白芷)
Fangfeng (Radix Saposhnikoviae, 防风)
Guizhi (Ramulus Cinnamomi, 桂枝) (each 3 qian)
Crush the herbs. Take 1 liang per dose, decoct with two cups of water and three stalks of green onion white (with roots) until one cup remains, strain, and drink slightly warm after meals. After taking the medicine, avoid Wind-Cold. Lie down until slight sweating occurs, then stop.
[Zhu Danxi] Lou Sheiren suffered from deviation of the mouth and eyes, starting with toothache on the right side.
Fangfeng (Radix Saposhnikoviae, 防风) (0.5 qian)
Nanxing (Rhizoma Arisaematis Cum Bile, 南星) (1 qian)

Decoct these, add 1 he of Ginger Juice (Jiangzhi, 姜汁) at the end, and take.
[Bencao Gangmu] Treating Wind Evil lodging in Yang meridians, damaging the interstices, causing back stiffness, deviation of the mouth and eyes, fever with aversion to cold, phlegm-jue headache, muscle twitching, Xin Yi Bi Yuan (Olfactory Sinusitis), vomiting saliva after excessive drinking, headache and dizziness as if on a boat. Regular consumption resolves Wind-Cold, dispels miasma, clears the spirit, prevents various winds. Dingfeng Bingzi (Wind-Calming Cake).

Chuanxiong (Rhizoma Chuanxiong, 川芎)
Baifuling (Poria with White Hosta, 白茯苓)
Sheng Gancao (Raw Glycyrrhizae Radix et Rhizoma, 甘草) (equal parts)
Grind into fine powder, make pills with Ginger Juice, size of a longan, form into cakes, coat with Cinnabar (Zhusha, 朱砂). Take one cake per dose, chew thoroughly, swallow with hot Ginger Tea, anytime. During the Xifeng era, Chancellor Wang took this regularly to prevent wind diseases with miraculous effect.
[Qianjin Fang] Zhiru Jiu (Arctic Fruit Wine): Mainly for severe deviation of the mouth and eyes, effective when other medicines fail.
Take powder from Zhisshi (Fructus Aurantii Immaturus, 枳实) scraped from the green skin, up to five sheng. Stir-fry lightly to remove moisture, soak in one dou of wine, warm slightly over low fire to extract the flavor, drink according to tolerance.
[He Zhizi] A man from Dongjing suffered from deviation of the mouth and eyes, with hands tense and twitching rapidly like a drawn bowstring, strong and solid. His constitution was robust, with Wind and Fire battling. Prescribed Tiaowei Chengqi Tang (Regulating the Stomach and Purging Down Decoction) six liang, decocted in four sheng of water to three sheng, divided into four doses, taken slightly warm. He defecated about 40-50 times. Reduced dose by one liang, then administered an emetic, detoxified several times, regulating Water and Fire, cured in less than ten days. Maijue says: Heat generates Wind. Neijing states that deviation of the mouth and eyes is mostly a disease of the Foot-Yangming tendons, as they knot above the cheeks. Cold makes them tense, heat makes them flaccid. Left cold/right heat causes the left tense tendon to pull the right flaccid side, leading to leftward deviation. Right cold/left heat causes the right tense tendon to pull the left flaccid side, leading to rightward deviation. Treatment involves moxibustion, applying ointment/massage to the tense side, and smearing Gui Zhi (Cinnamon Twig) mixed with white wine on the flaccid side. Use a mulberry hook to straighten the tongue/deviated corner, ensuring symmetry. Apply raw mulberry ash paste to the cheek to tighten the flaccid side. Drink Ginger Wine and eat rich meat to harmonize Qi in tendons/vessels, assisting external treatment. Non-drinkers should strengthen sinews/bones and pat the tense areas manually to relieve symptoms. (See Men Jin section).
[Qianjin Fang] Another Formula:
Muli (Concha Ostreae, 牡蛎)
Fuzi (Radix Aconiti Lateralis Preparata, 附子)
Fanshi (Alumen, 矾石)
Zaoxia Huangtu (Yellow Earth from Hearth, 灶下黄土)
Grind equal parts into powder. Mix with blood from a three-year-old rooster's comb, apply to the affected area. Watch in a mirror; wash off immediately when correction begins. If not washed off quickly, it may over-correct. (Qianjin Yiyi says: Apply to the opposite side).
Another Formula: Decoct Guizhi (Cinnamon Twig) in wine to get one sheng of juice. Soak old cloth in it and apply to the lesion to correct it. Apply to the right for left deviation, to the left for right deviation. This is a secret recipe, highly effective. (This is the method from Neijing using wine-soaked Cinnamon Twig).
Another Formula: Da Zaojiao (Gleditsiae Sinensis Fructus, 大皂荚) one liang, peel and remove seeds, grind/sift. Mix with powder from three-year-old Duanshi (Smelted Iron Ore, 锻石) and water. Apply to the opposite side for deviation. Reapply when dry.
[Yanyi] Take one he of hard Duanshi (Smelted Iron Ore), stir-fry with vinegar to a mud-like consistency. Apply to the healthy side of the lips in patients with hemiplegic deviation of the mouth and eyes. It immediately pulls the face straight.
[Sun Simiao] For sudden hemiplegia with deviation of the mouth and slurred speech: Take Baiyu (White Fish/Lice, 衣中白鱼) from clothes and rub under the ear. Rub towards the right if deviated left, towards the left if deviated right, to correct it.
[Shengji Zonglu] For stroke with deviation of the mouth and eyes: Take seven Baiduo (Seminar Crotonis Tiglii, 巴豆) beans, peel and grind. Apply to the palm of the right hand for left deviation, to the left palm for right deviation. Place a cup of warm water over the palm. It corrects quickly. Wash off the medicine and frequently pull/stretch the middle finger.
[He Zhizi] A magistrate suffered from deviation of the mouth and eyes. I treated him. For eye deviation, moxa Chengqi (Chengqi, ST1); for mouth deviation, moxa Dicang (Dicang, ST4). Both were effective. If ineffective, moxa Renying (Renying, ST9). Qi deficiency allows Wind entry causing deviation; if Qi cannot exit upwards or exit downwards, True Qi is trapped by Pathogenic Qi, so moxibustion is suitable for immediate cure. (Note: This works because the pulse indicates deviation. If it is tendon tension causing deviation, moxibustion alone won't work; oral medication, burning needle therapy (burning needle therapy) on the tense area, or Horse Fat application (Horse Fat application) is needed. Chengqi, Dicang, and Renying are all Foot-Yangming points, originating from the Stomach Meridian).
[Yuji Weiyi] Deviation of the mouth and eyes: Dicang (Needle insert 2 fen, oblique along skin towards Jiache, 1.5 cun, retain needle for 10 breaths, reduce method). Jiache (Insert 2 fen, oblique towards Dicang. For these three points, tonify left/reduce right for right deviation, tonify right/reduce left for left deviation).
[Jia Yi Jing] Koupi (Deviation of the mouth): Quanliao (Quanliao, SI18), Yingjiao (Yingjiao, DU26), Xiaguan (Xiaguan, ST7) govern it. Facial aversion to Wind-Cold, swelling, abscess pain, erratic gaze, convulsions, deviation of the mouth: Juliao (Juliao, ST3) governs it. Inability to drink water, deviation of the mouth: Shuigou (Shuigou, DU26) governs it. Jue-ni deviation of the mouth, yawning, lower toothache, cheek swelling, aversion to cold, inability to close mouth, speak, or chew: Daying (Daying, ST5) governs it. Deviation of the mouth: Pianli (Pianli, LI6) governs it. Deviation of the mouth: Needle Taiyuan (Taiyuan, LU9) to guide downward. Deviation of the mouth with lockjaw: Waiguan (Waiguan, TE5) governs it. Deviation, yawning, inability to open mouth: Yifeng (Yifeng, TE17) governs it.
[Qianjin Fang] Another Formula: Use a reed tube 5 cun long. Insert one end into the ear canal, seal edges with dough to prevent air leakage. Put one soybean and moxa at the other end, burn and moxa 7 cones. Cured immediately. Treat right lesion by moxa-ing left, left lesion by moxa-ing right. Secret Qianjin method, also applicable for ear diseases.
