[Tang] Bulu (Bulu Syndrome) is caused by the inability to digest milk and food, leading to spleen and stomach weakness. Gradually, appetite diminishes, qi and blood are depleted, muscles lose nourishment, resulting in emaciation with exposed bones. It is accompanied by a persistent sensation of bitter heat, hence called Bulu. The appropriate treatment is Muxiang Jinshi Wan (Musk Appetite-Enhancing Pill). (Formula see this section.)
[Tang] Pediatric Ji Syndrome (Accumulation Syndrome): The face is yellow and swollen, the abdomen is distended, the child lies prone with the face pressed to the ground, urine is turbid like oil, frequent vomiting occurs, eyes are yellow, there is empty rumbling in the abdomen, excessive sleepiness with yellowish whites of the eyes, dysentery with red and white discharge, jaundice, red eyes, and frequent diarrhea.
Whenever there is accumulation, one must differentiate between excess and deficiency. Moreover, children have weak and thin constitutions; their height, skin color, and regional or temporal contexts vary greatly, so they cannot be generalized. My current method is: for excess patterns, Shijin Wan (Appetite-Enhancing Pill) can be taken; for deficiency patterns with slightly pale complexion and emaciation (Gan emaciation), Fei'er Wan (Flesh-Nourishing Pill) is suitable. (Fei'er Wan formula see Gan Gate Heart Gan section, made into pills the size of millet seeds. For a one-year-old child, ten pills per dose, taken with rice drink.)
[Qian] When a child has Pi (masses) in the abdomen, manifested by refusal to eat but willingness to drink milk, it should gradually be treated with Baixing Zi (White Cake Pills).
Baixing Zi
Talcum (Huashi), Calomel (Qingfen), Pinellia Tuber (Banxia - washed in boiling water, dried by baking), Arisaema Rhizome (Nanxing) (each 1 qian, powdered), Croton Seed (Badou - 24 seeds, peeled and membrane removed, boiled in one sheng of water until water is exhausted).
Grind all ingredients uniformly. Add Badou last to the other powders. Form pills the size of small mung beans using glutinous rice paste. Shape into cakes. For children over three years old, 3-5 cakes per dose; for children under four years old, 1-2 cakes per dose. Take with scallion white decoction at bedtime.
[Jufang] Shijin Wan (Appetite-Enhancing Pill) treats indigestion of milk and food, chest and epigastric fullness/distension, Gan accumulation abdominal pain.
Costus Root (Muxiang - processed), Bitter Orange Pericarp (Zhiqiao - white pith removed, stir-fried), Angelica Sinensis Radix (Danggui), Hematite (Daizheshi), Cinnabar (Zhusha - separately ground, 3 qian), Croton Seed (Badou - oil and membrane removed, 1 qian), Musk (Muxiang - separately ground, 5 fen).
Grind into fine powder. Form pills the size of millet seeds using wheat paste. For a one-year-old child, two pills per dose, adjust according to the child's size. Take with rice drink.
[Huo'an] Shuangsheng Wan (Double Sage Pill) treats pediatric fever and headache, indigestion, epigastric and abdominal distension and pain, difficulty in urination and defecation, or tenesmus with frequent urges. If not cured, it may be taken again.

Grind Maidong, Gancao, Gansui, and Muli into fine powder. Add Badou, Zhusha, and Rui Ren. Mix and pound for two thousand strokes. Add a little honey and pound until very well incorporated, then form pills. For a half-year-old, dose size of perilla seed; for a one-year-old, dose size of sesame seed, divided into two pills. For a two-year-old, one pill size of sesame seed. For 3-4 year olds, two pills size of sesame seed. For 5-6 year olds, three pills size of sesame seed. For 7-8 year olds, two pills size of adzuki bean. For a ten-year-old, three pills slightly larger than adzuki bean. Take during the chicken-crowing period (3-5 AM). If stool does not pass by sunrise, drink several he of hot porridge; the medicine will act immediately, and the pills usually pass completely. If diarrhea is severe, give several he of thick, cold porridge decoction to stop it.
[Qian] Zhenzhu Wan (Pearl Pill) treats various accumulations in children, fright-phlegm, retained food, Ru Pi (milk masses), difficult urination and defecation, abdominal distension, and promotes qi movement.

Generally, if a child has warm-feverish heat and it is not Bianzheng (physiological growth fever), and the stool is white and sour-smelling, it indicates cold accumulation in the stomach; use this medicine to dissipate and purge, followed by warming stomach medicines. If the child has warm-feverish heat and the stool is red and sour-smelling, it indicates heat accumulation in the stomach; use this medicine to purge, followed by cooling stomach medicines. There will be no failure to heal. (Another prescription includes seven Semen Quisqualis [Shijunzi].)

[Tang] Jingpi (Fright Pi) must be treated with Mengshi (Lapis Lazuli) preparations.
The above seven formulas for treating Pi are all harsh/potent agents. If there is any involvement of deficiency, one must concurrently administer Sizijunzi Tang (Four Gentlemen Decoction) or Yihuang San (Yellow-Enriching Powder).
[Milu] Treating pediatric Qi Pi. Make a soup/porridge using a three-edged needle, prepared with rice and flour, and feed it to the lactating mother. Every day, give the child an amount the size of a jujube to eat. It can also be made into porridge or soup and eaten hot. This treats children under ten years old and newborns up to 100 days old, regardless of epilepsy, fever, Wugu Pi, etc. The effect is miraculous and indescribable.
[Tian] For pediatric Pi Qi that persists for a long time without healing: Moxibustion on Zhongwan (CV12) one point, Zhangmen (LR13) two points, each seven moxa cones. Zhangmen is located lateral to Dahan (SP15), directly opposite the umbilicus, at the end of the costal margin in the hypochondrium. Lie on the side, flex the upper leg, extend the lower leg, raise the arm to locate the point. Zhongwan is one cun below Shangwan, four cun above the umbilicus, midway between the xiphoid process and the umbilicus. Locate the point using the patient's four fingers below HT (Heart Meridian). Also moxa twenty-eight cones (two sets of seven) on the midline of the spine behind the umbilicus. No case fails.
