[Danxi] A male patient developed fever, headache, and a tight pulse after bathing in cold water. This indicates the presence of cold-dampness, and it is appropriate to treat with warm herbs to induce sweating.
Cangzhu (Atractylodis Rhizoma), Mahuang (Ephedrae Herba), Gange (Puerariae Radix) (each 1.5 qian); Gancao (Licorice Root, processed with honey, a little); Chenpi (Citri Reticulatae Pericarpium), Chuanxiong (Ligustici Chuanxiong Rhizoma) (each 2 qian).
Divide into two doses for administration. After sweating occurs and the disease recedes, administer the following tonic prescription.
Shaoyao (Paeoniae Radix Alba) (0.5 liang); Chenpi (Citri Reticulatae Pericarpium), Banxia (Pinelliae Tuber) (each 3 qian); Baizhu (Atractylodis Macrocephalae Rhizoma), Cangzhu (Atractylodis Rhizoma), Renshen (Ginseng Radix et Rhizoma), Mutong (Akebiae Caulis) (each 2 qian); Gancao (Licorice Root) (0.5 fen).
Divide into four doses, decoct with ginger water, and take warm.
Another female patient presented with headache, fever, and thirst.
Baizhu (Atractylodis Macrocephalae Rhizoma) (0.5 liang); Chenpi (Citri Reticulatae Pericarpium), Chuanxiong (Ligustici Chuanxiong Rhizoma) (each 3 qian); Gange (Puerariae Radix) (2 qian); Mutong (Akebiae Caulis) (1.5 qian); Gancao (Licorice Root, processed with honey, a little).
The above ingredients are divided into four doses, decocted with water, and taken warm.
[From *Huo Ren Shu*] If sweating has already been induced, or if it has not yet been induced, but the headache is severe as if the head were breaking apart, use Lianxu Congbai Tang (Scallion White Decoction with Roots).
Formula for Lianxu Congbai Tang (Scallion White Decoction with Roots):
Congbai (Allii Fistulosi Bulbus, with roots, chopped, 0.5 sheng); Shengjiang (Zingiberis Rhizoma Recens) (2 liang).
Boil the above ingredients in two sheng of water until one sheng remains. Divide into two doses and take warm. If the pain does not cease, it is appropriate to take the following formula.
Formula for Gegen Congbai Tang (Kudzu and Scallion White Decoction):
Gegen (Puerariae Radix), Shaoyao (Paeoniae Radix Alba), Zhimu (Anemarrhenae Rhizoma) (each 0.5 liang); Congbai (Allii Fistulosi Bulbus), Chuanxiong (Ligustici Chuanxiong Rhizoma), Shengjiang (Zingiberis Rhizoma Recens) (each 1 liang).
Boil the above ingredients in three sheng of water until one sheng remains. Take hot.
If there is non-paroxysmal headache accompanied by chest fullness, alternating chills and fever, and a tight but not large pulse, this indicates phlegm-fluid retention above the diaphragm. It is appropriate to use Guadi Mo (Cuscuta Seed Powder? Note: Guadi usually refers to Melon Seed/Trichosanthes Semen used for emesis, specifically Gua Di Zi or similar emetic agents like Chai Tou in some contexts, here translated as Guadi Powder) one qian, mixed with warm water and administered. Vomiting the phlegm-fluid will lead to immediate recovery.
[Yunqizi] After sweating or purgation in Shanghan (Cold Damage), if headache arises accompanied by vertigo/dizziness, Duhuo Tang (Angelica Pubescens Decoction) is appropriate.
Fangfeng (Saposhnikoviae Radix), Duhuo (Angelicae Pubescentis Radix), Xuanjinhua (Inulae Flos), Danggui (Angelicae Sinensis Radix) (each 7 qian).
Coarsely crush the above ingredients, decoct with ginger water, and take.
After the heat stage of Shanghan (Cold Damage) or febrile disease, if headache persists, use Shigao Chuanxiong Tang (Gypsum and Ligusticum Decoction).
Shigao (Gypsum Fibrosum), Chuanxiong (Ligustici Chuanxiong Rhizoma) (each 1 liang).
Coarsely crush the above ingredients. Each dose is five qian, decocted with water and taken.
[H aizang ] Taiyang (Greater Yang) channel headache: if there is sweating, use Guizhi Tang (Cinnamon Twig Decoction); if there is no sweating, use Mahuang Tang (Ephedra Decoction). Yangming (Yang Brightness) channel headache: use Baihu Tang (White Tiger Decoction). Shaoyang (Lesser Yang) channel headache: use Xiaochaihu Tang (Minor Bupleurum Decoction). Taiyin (Greater Yin) channel headache: if the pulse is floating, use Guizhi Tang (Cinnamon Twig Decoction); if the pulse is deep, use Lizhong Tang (Regulate the Middle Decoction). Shaoyin (Lesser Yin) channel headache: use Xiaochaihu Tang (Minor Bupleurum Decoction). Jueyin (Terminal Yin) channel headache due to external injury to the channel itself: use Guizhi Mahuang Gehan Tang (Cinnamon Twig and Ephedra Half-Size Decoction). For vomiting with slight expulsion of bitter fluid: use Wuzhuyu Tang (Evodia Decoction).
[Lidongyuan] Patients with Taiyin (Greater Yin) channel headache must have phlegm; patients with Shaoyin (Lesser Yin) channel headache have cold feet and rebellious Qi. Although the Taiyin and Shaoyin channels do not directly reach the head, if phlegm and rebellious Qi obstruct the middle jiao (diaphragm area), the clear Qi of the head cannot descend smoothly, resulting in headache.
[Yunqizi] If the pulse is floating and there is headache, the pathology lies in the Hand and Foot Taiyang channels; puncture Wanggu (GB12) and Jinggu (BL64). If the pulse is floating and long, the pathology lies in the Hand and Foot Yangming channels; puncture Hegu (LI4) and Chongyang (ST42). If the pulse is floating and wiry, the pathology lies in the Hand and Foot Shaoyang channels; puncture Yangchi (TE4), Qiuxu (GB40), Fengfu (GV16), and Fengchi (GB20). These are the methods for puncturing to treat headache.
[From *Zhengzhi Zhunsheng*] For headache caused by Shanghan (Cold Damage), puncture Hegu (LI4) and Cuanzhu (BL2).
