[From *Mai Jing* (Pulse Classic)] When a woman's menstruation occurs but is scanty, and the physician palpates the pulse, stating contrary to expectation that she is pregnant, which is later confirmed. What does this pulse resemble? How can it be distinguished? The Master replied: The yin and yang aspects of the cunkou (inch position) pulse are both balanced; Ying (Nutritive Qi) and Wei (Defensive Qi) are harmonious. Pressing deeply reveals a slippery quality, while light palpation feels floating. The Yangming and Shaoyin pulses conform to standard methods. The body experiences chills and shivering, there is a lack of appetite, headache, mental confusion, retching, and a desire to vomit. Exhalation is slightly rapid, while inhalation is not startled. Yang excess leads to qi overflow; Yin slipperiness indicates abundant qi and blood. A slippery pulse indicates excess or surplus blood; with the six meridians nourished, menstruation appears. Yin encountering Yang essence causes fluids to coagulate in the uterus; if uterine qi scatters, damage and abortion may result. If Yang becomes excessive again, twin pregnancy may occur. Now, due to insufficient Yang, this condition is termed "Ji Jing" (stimulated menstruation/bleeding during pregnancy). (A slippery pulse governs blood surplus; since menstruation is scanty, pregnancy is known.)
[Zhu Danxi] Fetal leakage (vaginal bleeding during pregnancy) is caused by Qi deficiency, Blood deficiency, or Blood heat.
[Zhang Zhongjing] The Master said: Women may suffer from continuous non-menstrual bleeding, persistent bleeding after spontaneous abortion, or bleeding during pregnancy. If abdominal pain accompanies pregnancy bleeding, it is called "Bao Zu" (uterine obstruction), treated with Xiong Gui Jiao Ai Tang (Cinnamon Twig, Angelica, Donkey-hide Gelatin, and Moutan Decoction). (Formula see section on Metrorrhagia.)
[Master Mei] To treat fetal restlessness with bleeding and heart/abdominal pain, where life or death is uncertain: taking this decoction will stabilize the fetus if alive, or expel it if dead. This is the Cui Sheng Fo Shou San (Hand-of-Buddha Powder for Inducing Labor), see section on Traumatic Injury to Pregnancy. (It consists of equal parts Dang Gui and Chuan Xiong powdered.) For sudden, incessant bleeding during pregnancy without cause: take three liang of Ejiao (Donkey-hide Gelatin), roasted and powdered, mixed with one and a half sheng of wine, boiled until dissolved. One dose cures.
[Luo Qianfu] Li Sheng San (Establishing Sage Powder) treats incessant bleeding during pregnancy.
Three chicken livers, cooked with one sheng of wine, eaten together. Highly effective.
[*Baoming Ji*] For fetal leakage, use Er Huang San (Two Yellow Powders).
Equal parts of Sheng Di Huang (Rehmanniae Radix Recens) and Shu Di Huang (Rehmanniae Radix Preparata), chopped. Boil in three zhen of water until reduced by half, strain, and drink.
[Master Mei] For fetal bleeding. Two liang of Ejiao (Donkey-hide Gelatin), powdered; half a jin of Sheng Di Huang (Rehmanniae Radix Recens), juiced. Mix with three sheng of clear wine, extract juice, and divide into three doses.
[*Da Yi*] For incessant fetal leakage bleeding, where dryness of the uterus leads to fetal death, urgent treatment is required.
Sheng Di Huang Zhi (Juice of Rehmanniae Radix Recens, one sheng); Chen Jiu (Aged Wine, five he).
Boil together for three to five boils, warm, and divide into three doses. Stop when bleeding ceases.
[*Baoming Ji*] For women with fetal leakage or bleeding due to external factors, use Zhi Ke Tang (Immature Bitter Orange Decoction).
Zhi Ke (Aurantii Fructus Immaturus), Huang Qin (Scutellariae Radix) (each half liang); Bai Zhu (Atractylodis Macrocephalae Rhizoma) (one liang).
Grind into coarse powder. Take seven qian each time, boil in one zhen of water until reduced to seven parts, take before meals.
[Zhu Danxi] A woman in her early twenties, three months pregnant, developed pale bloody discharge and abdominal fullness/thirst after malaria.
Bai Shao (Paeoniae Radix Alba), Bai Zhu (Atractylodis Macrocephalae Rhizoma), Fu Ling (Poria) (each one qian); Huang Qin (Scutellariae Radix), Gui Wei (Tail of Angelicae Sinensis Radix), Chuan Xiong (Ligustici Chuanxiong Rhizoma), Chen Pi (Citri Reticulatae Pericarpium) (each five fen).
A woman from Dongyang, aged thirty-five, eight to nine months pregnant, with incessant fetal leakage. The fetus felt smaller than before, blood was slightly purplish with clots, appetite was one-third of normal, mild abdominal pain, and low mood.
Ren Shen (Ginseng Radix et Rhizoma), Chao Bai Zhu (Stir-fried Atractylodis Macrocephalae Rhizoma), Bai Shao (Paeoniae Radix Alba) (each one qian); Chen Pi (Citri Reticulatae Pericarpium), Chuan Xiong (Ligustici Chuanxiong Rhizoma), Fu Ling (Poria), Suo Sha Ren (Amomi Fructus), Da Fu Pi (Arecae Pericarpium) (each three fen); Mu Tong Teng (Vitis Stem, seven leaves).
Decoct together, take fifty pills of San Sheng Wan (Three Sage Pills) before meals.
[Zhang Zhongjing] A woman with an existing Zheng disease (masses/stagnation in the abdomen) has missed her period for three months and suffers from continuous leakage. If fetal movement is felt above the umbilicus, it is considered harm caused by the Zheng mass. If fetal movement occurs at six months of pregnancy, and menstruation was regular for the first three months, the fetus is normal. If bleeding occurs after stopping menstruation for three months, it is due to Zheng. The reason bleeding continues is that the Zheng mass remains. It should be expelled using Gui Zhi Fu Ling Wan (Cinnamon Twig and Poria Pill). (Fetal movement usually occurs around the umbilicus; since it is above, it indicates Zheng disease.)
Gui Zhi (Cinnamomi Ramulus), Fu Ling (Poria), Mu Dan Pi (Moutan Cortex), Tao Ren (Persicae Semen), Bai Shao (Paeoniae Radix Alba) (equal parts).
Powder these five ingredients, mix with refined honey to make pills the size of rabbit droppings. Take one pill before meals daily. If ineffective, increase to three pills.
[*Bencao*] For incessant fetal bleeding: Take dried peaches that have not fallen from peach trees, burn them to ash, mix with water, and ingest to cure. (Compendium notes: Tao Nu [Dried Peach] invigorates blood and treats Fuliang accumulation/qi stagnation.)
