[Hai Zang] Treating women with Blood Wind Syndrome (Blood Wind Disorder), caused by massive blood loss, metrorrhagia and metrostaxis, or bleeding from the anterior or posterior orifices, leading to dryness and depletion. The internal heat persists, manifesting as picking at clothes, grasping at empty air, and groping for the bed; eyes closed with unconsciousness; waving hands and looking up; restlessness and agitation; incoherent speech and loss of spirit. The pulse is wiry, floating, and deficient, indicating extreme internal dryness and heat. Breathing is coarse, the nose is dry without moisture, and dryness pervades both upper and lower parts; this is difficult to treat. Shengdi Huang Huanglian Tang (Rehmannia Root and Coptis Decoction) should be administered.
Chuanxiong Rhizoma (Chuanxiong), Shengdi Huang Radix Rehmanniae Preparata (Shengdihuang), Danggui Radix Angelicae Sinensis (Danggui) (each 7 qian); Chishao Radix Paeoniae Rubra (Chishao), Zhizi Fructus Gardeniae (Zhizi), Huangqin Cortex Scutellariae (Huangqin), Huanglian Rhizoma Coptidis (Huanglian) (each 3 qian); Fangfeng Radix Saposhnikoviae (Fangfeng) (1 liang).
Grind the above into a coarse powder. For each dose, take 3 qian, add two zhan of water, decoct until reduced to seven-tenths, strain and retain the clear liquid. Drink at any time, administering slowly. If the pulse is excess/full, add Dahuang Radix et Rhizoma Rhei (Dahuang) to purge.
Da Chengqi Tang (Major Order the Qi Decoction) is a formula regulating Qi, used when pathogenic factors move from exterior to interior. Shengdi Huang Huanglian Tang is a formula regulating Blood, used when pathogenic factors move from interior to exterior. When Qi and Blood are simultaneously affected, presenting with picking at clothes and grasping at empty air, if the syndrome originates from Qi affecting Blood, and then Blood affects Qi, Da Chengqi Tang should be used to purge. If it originates from Blood affecting Qi, and then Qi affects Blood, Shengdi Huang Huanglian Tang should be used. Both conditions involve constipation.
[Hua Tuo] Patients who pick at their seams while speaking deliriously cannot be cured. Patients whose Yin and Yang qi are exhausted, pulling at clothes, grasping at empty air, and speaking deliriously will die.
Joyful laughter belongs to Heart Fire (Heart-Fire). The Classic states: The Heart stores the Spirit (Shen); an excess of Shen leads to incessant laughter. It also states: In the viscera, it corresponds to the Heart; in sounds, it corresponds to laughter; in emotions, it corresponds to joy. Furthermore: When Essence and Qi converge in the Heart, there is joy. Also: Excessive Fire is called "He Xi" (Blazing Heat); in years of He Xi, diseases manifest as laughter, mania, and delusion. Additionally: The arrival of Shaoyin (Lesser Yin) energy manifests as joyful laughter.
[Li Dongyuan] Laughter represents flourishing, freshness, comfort, glory, and manifestation; this is the transformation of Fire. Therefore, joy is the emotion of Heart-Fire. Extreme joy turning into laughter is like excessive burning, producing the image of laughing. Thus, pathological laughter indicates an abundance of Heart-Fire.
[Zhang Zihe] Passing by an old residence, I encountered a woman suffering from incessant laughter for half a year. Everyone else had no method to treat her, so she sought treatment from Dai Ren (Zhang Zihe's pseudonym). Dai Ren said, "This is easy to treat." He took over two liang of block-shaped Cangyan (Halite/Rock Salt), heated it in fire until red hot, allowed it to cool, ground it fine, and decocted it in one large bowl of river water until it boiled three to five times. After cooling to warm temperature, it was divided into three doses to drink. A hairpin was used to stimulate the throat to induce vomiting, expelling five sheng of hot phlegm. Subsequently, a fire-purging agent was administered. Since Fire governs the bitter taste, detoxification is the principle. Within a few days, the laughter ceased. The *Neijing* states: "When Shen is in excess, laughter does not cease." What is referred to as Shen is Heart-Fire. Fire catches flame with wind, hence laughter is the image of Fire. Among the Five Elements, only Fire has the characteristic of laughter. I once treated an elderly man with incessant laughter and drooling. Using Huanglian Jiedu Tang (Coptis Decoction for Detoxification) added with Banxia Rhizoma Pinelliae Preparatum (Banxia), Zhuye Folium Bambusae (Zhuye), Zhuli Succus Bambusae (Zhuli), and Jiangzhi Succus Zingiberis Recens (Jiangzhi), the laughter stopped.
For acupuncture and moxibustion treatment of joyful laughter, exclusively select the Pericardium Meridian of Hand-Jueyin (Pericardium Channel). The Classic states: The Pericardium Meridian of Hand-Jueyin, when disturbed, causes red face, yellow eyes, and incessant laughter. One must carefully assess excess, deficiency, heat, cold, and sinking conditions to select acupoints accordingly.
