[Lingshu] When Ying (Nutrient) Qi becomes diseased, it manifests as alternating chills and fever, shortness of breath, and blood moving up and down in the body. (From *Shou Yao Gang Rou* Chapter)
[Zhang Zhongjing] states: If a woman suffers from Wind-Cold for seven or eight days and subsequently develops alternating chills and fever that occur at regular intervals, coinciding with the sudden cessation of her menses, this is Heat entering the Blood Chamber. The blood must be congealed, causing symptoms resembling malaria with regular paroxysms. Xiao Chai Hu Tang (Minor Bupleurum Decoction) is the primary treatment. (Formula see *Shanghan Lun*.)
[Li Dongyuan] Modifications of Xiao Chai Hu Tang: For alternating chills and fever with irregular menstruation, remove Ban Xia (Pinellia Tuber), and add Qin Jiao (Gentiana Macrophylla Root), Shao Yao (Peony Root), Dang Gui (Angelica Sinensis Root), Zhi Mu (Anemarrhena Rhizome), Di Gu Pi (Cortex Lycii), Mu Dan Pi (Cortex Moutan), Chuan Xiong (Ligusticum Chuanxiong Rhizome), Bai Zhu (Atractylodes Macrocephala Rhizome), and Fu Ling (Poria). If Xiao Chai Hu Tang and Si Wu Tang (Four Substances Decoction) are combined in equal parts, it is named Tiao Jing Tang (Regulate Menstruation Decoction). If there is no pregnancy but vomiting occurs, add Ban Xia. If there is no sweating, add Chai Hu (Bupleurum Root). If there is aversion to cold, add Gui Zhi (Cinnamon Twig). If there is sweating, add Di Gu Pi. If there is coughing, add Zi Wan (Aster Root). To unblock menstruation, add Jing San Leng (Sparganium Rhizome) and Guang Zhu (Curcuma Rhizome). For consumptive disease, add Bie Jia (Carapax Trionycis).
[Luo Tianyi] General Wanyan suffered from intermittent chills and fever. There were three to five macules on his wrists, and slight epistaxis. His pulses in both hands were deep and choppy. Pressing on his chest, diaphragm, and limbs revealed no significant heat. This was internal Cold injuring the Spleen/Stomach. Inquiry revealed he had previously caught a chill in a palace corner and drank iced cheese water when thirsty. The external contraction was mild, while the internal injury was severe; the external pathogen followed the internal pathology, both being Yin patterns. Hence, macules appeared first, followed by epistaxis, revealing an internal Yin syndrome. Intermittent chills and fever can also arise from the Spleen, not solely from Shaoyang. He was cured after taking several doses of Diao Zhong Tang (Regulate the Middle Decoction).
Diao Zhong Tang
Bai Fu Ling (Poria), Gan Jiang (Dried Ginger), Bai Zhu (Atractylodes Macrocephala Rhizome), Gan Cao (Licorice Root) (equal parts)
Crush these into pieces the size of hemp seeds. Take 5 qian per dose, decocted in 1.5 cups of water.
The Classics state: When Wind Qi is abundant between the skin and hair, it cannot circulate internally nor vent externally. Wind moves swiftly and changes frequently; when pores open, one feels chilly; when closed, one feels hot and stifled. Excess Cold leads to decreased appetite; excess Heat consumes muscle mass, causing shivering and inability to eat, termed "Chills and Fever." (Wind entering the skin causes chills and fever. Full text in *Feng Men*. "Li" may be read as "Shi Wei".) Exposure to wind generates chills and fever. (Full text in *Wu Zang*.) Excessive Wind causes chills and fever. (Full text in *Zhen Fa*.)
[He/Jin School Representative] Jie Feng Tang treats chills and fever due to Wind-Cold invasion, accompanied by dizziness, limb pain, numbness, and upper jiao stagnation with boiling sensation.
Ren Shen (Ginseng Root), Chuan Xiong (Ligusticum Chuanxiong Rhizome), Du Huo (Angelica Pubescens Root), Gan Cao (Licorice Root), Ma Huang (Ephedra Herb - remove joints, wash with boiling water, dry), Xi Xin (Asarum Root) (half liang)
Grind into powder. Take 3 qian per dose, decocted in 1 cup of water with 5 slices of fresh ginger and a few mint leaves, until reduced to 0.8 cup. Take at any time.
Fang Feng Tang treats chills and fever due to Wind invasion.
Fang Feng (Saposhnikovia Root), Gan Cao (Licorice Root), Huang Qin (Scutellaria Root), Gui Zhi (Cinnamon Twig), Dang Gui (Angelica Sinensis Root), Bai Fu Ling (Poria) (each 1 liang), Qin Jiao (Gentiana Macrophylla Root), Gan Ge (Pueraria Root) (each 1.5 liang), Xing Ren (Apricot Seed) (50 seeds)
Grind into a powder. Decoct with water, wine, ginger, and jujube, and take orally.
[Mai Jing] Food retention in the Large Intestine causes chills, fever, and paroxysmal symptoms resembling malaria. (Mild cases use digestion-promoting methods; severe cases use purgative methods. For heat-food retention, use Cheng Qi Tang formulas; for cold-food retention, use pill formulas. See *Su Shi Men*.)
Abdominal accumulation causing chills and fever should be treated according to the method for treating accumulations.
[Yun Qizi] Di Gu Pi San treats Wind in the Blood, weakness with thirst, and chills/fever.
Chai Hu (Bupleurum Root), Di Gu Pi (Cortex Lycii), Sang Bai Pi (Morus Root Bark), Zhi Ke (Immature Bitter Orange), Qian Hu (Peucedani Radix), Huang Qin (Scutellaria Root) (each 7.5 qian), Bai Fu Ling (Poria), Wu Jia Pi (Eucommia Bark), Ren Shen (Ginseng Root), Gan Cao (Licorice Root), Gui Xin (Cinnamon Wood), Shao Yao (Peony Root - white strip variety) (each half liang)
Cut into small pieces. Take 3 qian per dose with 3 slices of ginger, decocted in water.
Chai Hu San treats chills/fever with emaciation, joint pain, dry mouth, and irritability.
Chai Hu (Bupleurum Root), Huang Qin (Scutellaria Root), Chi Fu Ling (Red Poria), Bai Zhu (Atractylodes Macrocephala Rhizome) (each 2 liang), Ren Shen (Ginseng Root), Di Gu Pi (Cortex Lycii), Zhi Ke (Immature Bitter Orange - processed), Jie Geng (Platycodon Root), Sang Bai Pi (Morus Root Bark), Chi Shao Yao (Red Peony Root), Sheng Di Huang (Rehmannia Root) (each 7.5 qian), Mai Men Dong (Ophiopogon Tuber) (3 liang), Gan Cao (Licorice Root) (half liang)
Cut into small pieces. Take 4 qian per dose with 5 slices of ginger, decocted in water.
[Ji Sheng Fang] A nun suffered from aversion to wind, fatigue, alternating chills and fever, red face, irritability, and occasional spontaneous sweating. During an epidemic, physicians treated her chills/fever as Shang Han (Cold Damage), administering mixed Da and Xiao Chai Hu Tangs, which worsened her condition over several days. I examined her; the three pulse positions showed no exterior cold signs, only a Jueyin pulse that was wiry, long, and extended beyond the radial styloid process. She required Yin-suppressing herbs. I prepared this Di Huang Wan (Rehmannia Pill).
Sheng Di Huang (Rehmannia Root) (3 liang), Chai Hu (Bupleurum Root), Qin Jiao (Gentiana Macrophylla Root), Huang Qin (Scutellaria Root) (each half liang), Chi Shao Yao (Red Peony Root) (1 liang)
Grind into fine powder, mix with refined honey, and form pills the size of tung tree seeds. Take 30 pills per dose with sour plum soup, at any time, three times daily. In Song Dynasty, Chu Cheng prescribed special formulas for nuns and widows, likely because these women lived in isolation (Yin without Yang), had suppressed desires, leading to Yin-Yang conflict, alternating chills/fever resembling Wen Re (Warm Heat/Malaria), eventually becoming Lao (Consumption). Reading *Shiji: Cang Gong Zhuan*, Lady Han, attendant to King Jibei, suffered back pain and chills/fever. Physicians diagnosed it as generic chills/fever. Cang Gong said: The illness resulted from desiring a man but being unable to obtain one. How did he know? He palpated her Liver pulse emerging wiry above the Cun position. Men prioritize Jing (Essence); women prioritize Blood. When male Jing is abundant, sexual desire arises; when female Blood is abundant, conception occurs, governed by the Liver's storage of Blood. A Jueyin pulse wiry and extending above Cun to Yuji indicates abundant Yin Blood. Chu Cheng's words are indeed valid.
Although called Yin-Suppressing Di Huang Wan, this formula actually tonifies Yin and drains Yang.
[Zhu Danxi] Lady Zhao experienced chills followed by fever at night, subsiding during Chou and Yin hours. Upon waking, she felt thirsty, had poor appetite, undigested food in stool, abdominal pain, diarrhea, and fatigue. Stress triggered heat and irritability, red eyes, qi stagnation, aversion to wind/cold, and aversion to heat.
Bai Zhu (Atractylodes Macrocephala Rhizome) (2 qian), Shao Yao (Peony Root - stir-fried) (1 qian), Chen Pi (Tangerine Peel) (1 qian), Huang Qin (Scutellaria Root) (half qian), Chao Bai (Stir-fried Phellodendron Bark) (3 qian), Zhi Gan Cao (Honey-fried Licorice Root) (2 fen), Chao Qin (Stir-fried Scutellaria Root) (3 fen), Mu Dan (Moutan Cortex) (3 fen), Mu Tong (Akebia Stem) (3 fen), Ren Shen (Ginseng Root) (half qian), Gui Shen (Angelica Sinensis Root tip) (2 qian), Suo Sha (Amomum Fruit) (3 fen)
Decoct and take with Bao He Wan (Preserve Harmony Pill) and Shi Chang Wan (Solidify Intestine Pill), 30 pills each.
Lady Lu (No. 14) developed chills/fever, cough, poor appetite, and dysuria/diarrhea after anger.
Suo Sha (Amomum Fruit) (3 fen), Gan Cao (Licorice Root) (3 fen), Ren Shen (Ginseng Root) (half qian), Bai Zhu (Atractylodes Macrocephala Rhizome) (1.5 qian), Lian Qiao (Forsythia Fruit) (1 qian), Chen Pi (Tangerine Peel) (1 qian), Fu Ling (Poria) (1 qian), 2 slices of ginger, decoct together.
Run Fei San (Moisten Lung Powder) treats cough. (See *Ke* section.) Da Chai Hu Tang (Major Bupleurum Decoction) treats interior binding-heat with alternating chills/fever. (See *Shanghan Lun*.)
For food retention, treat according to the method for food retention.
There are four types of Zang-Fu chills/fever:
1. Lung: The Classics state: Pulse comes rapid and weak, named Lung Bi chills/fever, caused by intercourse after drunkenness. Also: Lung pulse slightly urgent indicates Lung chills/fever.
2. Spleen: The Classics state: Spleen pulse very small indicates chills/fever.
3. Taiyang: The Classics state: Disease of the Three Yang channels presents with chills/fever.
4. Yang Wei Vessel: *Nan Jing* states: Disease of Yang Wei vessel involves suffering from chills/fever.
In Yun Qi (Five Movements and Six Qi) theory, there are two types of chills/fever:
1. Fire-Heat attacking Lung: The Classics state: When Shao Yin governs the year, Heat Qi descends, Lung Qi responds upward, causing panting, vomiting, and chills/fever. Also: When Shao Yang governs, Fire Qi descends, Lung Qi responds upward, causing chills/fever and swelling. Also: When Shao Yin governs, excessive Heat causes skin pain with chills/fever. Also: When Wood Qi is deficient, Dryness prevails, and the compensatory reaction causes chills/fever and sores, treated with cold agents.
2. Mixed Chills and Heat: The Classics state: When Yang Ming governs, weather is urgent, earth qi is bright, people suffer sudden chills/fever. In the third period, Dryness and Heat combine, causing chills/fever. Also: When Yang Ming governs, Dryness descends, Liver Qi responds upward, causing malaria-like chills/fever. Also: When Shao Yin governs, in the fourth period, Cold and Heat alternate, causing chills/fever. Treatment depends on the proportion of Cold and Heat.
[He Bing De/Zi He] Son of Qiu Tianxiang from Wuyang suffered chills/fever. Physicians treated him as Gu Zheng Lao (Bone Steaming Consumption) for half a year, worsening his condition. Dai Ren examined him; the Cun and Chi pulses directed towards Guan, and Guan pulse alone was large. Dai Ren said: This is Lung Abscess. Asked the wet nurse if there was pain; she said no. Dai Ren asked the child to remove clothes and raise arms, observing the hypochondrium; the right side was slightly higher. Pressing laterally, the child moved away to avoid pressure; pressing the left side caused no avoidance. Dai Ren said: There is an abscess in the Lung, already suppurating.
[Suwen] Methods for moxibustion in chills/fever: First, moxa Dachui (GV14) at the nape, using the patient's age as the number of cones. Next, moxa Juegu (coccyx), using age as cone count. Moxa any depressions on the back (e.g., Gaohuang points, indicating depression between bones/muscles). Moxa depressions in the shoulder when raising the arm (Jianyu LI15). Moxa between the costal margins (Jingmen GB25). Moxa the end of Juegu above the lateral malleolus (Yangfu GB38). Moxa between the 1st and 2nd toes (Xiaxi GB43). Moxa depressed vessels below (Chengjin BL56). Moxa behind the lateral malleolus (Kunlun BL60). Moxa hard, tendon-like pulsations above Clavicle (Quepen LI12) as indicated. Moxa depressions in the chest between bones (Tiantu CV22, uncertain). Moxa below the bone bundle at the palm (Yangchi TE4, uncertain). Moxa 3 cun below umbilicus (Guanyuan CV4) and arterial pulse at pubic hairline (Qichong ST30). Moxa 3 cun below knee in the gap (Zusanli ST36). Moxa arterial pulse on foot Yangming dorsum (Chongyang ST42). Moxa top of head (Baihui GV20). Total 29 sites.
