Diarrhea occurring at any time should be treated with one liang of prepared Aconitum carmichaelii (Shu Wu Tou), one fen each of Magnolia officinalis bark (Houpo), dried ginger (Ganjiang), and licorice root (Ganzao), decocted with fresh ginger for administration. Currently, treatments for dysentery often employ formulas such as Zhuche Wan (Parking Tent Pill) or Huanglian Ejiao Wan (Coptis and Donkey-hide Gelatin Pill). Among these, only Coptis chinensis (Huanglian) possesses the property of "fattening the intestines" (a metaphorical term in this context likely referring to its drying dampness or potential cold nature affecting the gut), but its nature is inherently cold. If the accumulated pathogenic factors are mild, or in cases of lingering, intermittent dysentery, taking these remedies may yield results. However, if the condition is slightly more severe, these agents are insufficient for treatment.
[Worldly/Popular] Treatment for dysentery.
Platycladi Cacumen (Cebaiye, 1 qian) Licorice root (Ganzao, 3 qian) Papaveris Pericarpium (Yumiqiao, remove top and internal pith, wash three times with hot water, dry, stir-fry with honey until yellow, 1 qian; double dose for chronic cases)
Chop the above ingredients, decoct in water until cooked, clarify the liquid, add honey, and sip slowly. First administer Muxiang Binglang Wan (Aucklandia and Areca Nut Pill), then take this formula; the effect is miraculous.
[Bencao] Another prescription.
Black soybean (Heidou, 50 grains) Tangerine peel (Chenpi, half liang) Poppy capsule (Yingsuqiao, 14 pieces) Licorice root (Ganzao)
Take the four ingredients, half raw and half stir-fried, decoct in water, and take on an empty stomach. Completing one dose ensures efficacy without exception.
[Li Dongyuan] Baizhu Anwei San (Atractylodes Macrocephala Stomach-Anchoring Powder) Treats all types of diarrhea and dysentery, regardless of whether pus and blood are mixed, accompanied by tenesmus (Liji Houzhong), distressing pain, and uncontrolled frequency day and night. It also treats male Qi pain of the small intestine, female deficiency-cold below the umbilicus, postpartum "child pillow" pain (uterine lochia retention pain), and cases with weakness and persistent alternating chills and fever.
Papaveris Pericarpium (Yumiqiao, 3 liang, remove top and pedicle, boil overnight in vinegar) Poria cocos (Fuling) Atractylodes macrocephala (Baizhu, 1 liang each) Mume fructus (Wumei, take flesh, 1 qian, stir-fried) Plantaginis Semen (Cheqianzi, 1 liang)
Schisandrae Fructus (Wuweizi, half liang)
Pulverize the above into fine powder. Take 5 qian per dose, decoct in water, and take warm.
[Jing] A divine method for treating slippery intestines and chronic dysentery. Split one pomegranate, roast it in charcoal fire until smoke ceases, quickly remove it (do not let it turn to white ash), cover with a bowl overnight to eliminate fire toxicity, pulverize into powder, and mix with decoction from one sour pomegranate to take 2 qian.
[Shengji Zonglu] For dysentery persisting for a long time where intestinal mucus has been discharged. Use two Mume fructus (Wumei), one cup of water, decoct to obtain six-tenths volume, strain, and divide into two doses taken before meals.
[Shan's Method] Treat dysentery using Myrobalan (Wubeizi) made into pills. For red dysentery, swallow with Licorice decoction; for white dysentery, swallow with Dried Ginger decoction, 10 pills each.
[Li Dongyuan] Hezi Pi San (Persimmon Peel Powder) In the winter of the Gui-Mao year, during the Minor Snow solar term, the weather was cold as expected. An old servant of the Bai family in the Privy Council presented himself kneeling, his face dull and colorless, spirit particularly weak, saying: "I have suffered from rectal prolapse for a long time, medications have failed, and recently I have passed red and white purulent dysentery with tenesmus (Liji Houzhong), predominantly white with less red, unable to bear the suffering, begging for your treatment." I said: "This person does not consume rich meat and grain foods, likely eats much vegetable food, or has irregular diet. The weather is already cold, clothing and bedding are thin, cold invades the body, leading to prolapse due to inability to withstand it. This is Cold and Slippage. True Qi cannot secure, form cannot contain, this is Blood depletion. This is Cold-Slippage with Qi leakage and insecurity, causing downward prolapse of form. One must use astringents to stop prolapse and remove slippage, use great-hot agents to remove cold and tonify Yang, use Qi-tonifying herbs to raise Yang and benefit Qi, and use slightly sour flavors to secure Qi and draw it upward. Named Hezi Pi San."
Papaveris Pericarpium (Yumiqiao, 5 fen, remove calyx, honey-fry) Dried Ginger (Ganjiang, 6 fen, processed) Tangerine peel (Chenpi, 5 fen) Persimmon peel (Hezi pi, 7 fen, baked, remove seed)
Chop the above, decoct in water and take, or mix with plain water and take. If taken on an empty stomach, take half dose first, then repeat; full recovery will occur.
[Luo Tianyi] Yangzang Tang (Nourish Viscera Decoction) Treats adults and children with weak spleen and stomach, disharmony of cold and heat, viscera affected by cold, passing red and white dysentery or pus and blood resembling fish brain, tenesmus (Liji Houzhong), abdominal pain, uncontrolled frequency day and night, chest and diaphragm oppression, hypochondriac distension, complete loss of appetite, rectal prolapse, bleeding from alcohol or damp-toxins, and cases where other medicines fail. All are suitable for this.
Ginseng Radix et Rhizoma (Renshen) Angelicae Sinensis Radix (Danggui, 6 qian each) Aucklandiae Radix (Muxiang, 1 liang 4 qian) Cinnamomi Cortex (Rougui, 8 qian) Myristicae Semen (Roukou, wrapped in flour, baked, half liang) Paeoniae Radix Alba (Baishaoyao, 1 liang 2 qian)
Licorice root (Ganzao, roasted, 6 qian) Atractylodis Macrocephalae Rhizoma (Baizhu, 6 qian) Papaveris Pericarpium (Yumiqiao, honey-roasted, 3 liang) Persimmon peel (Hezi pi, remove seed, 1 liang 2 qian)
Chop the above. Take two large qian per dose, decoct in water, strain, and take warm before meals. Elderly, pregnant women, and children with acute diarrhea should take it urgently for immediate cure. Avoid alcohol, wheat products, raw/cold items, fish/fowl, and greasy foods. For visceral slippage with nocturnal urgency and prolonged non-resolution, add four slices of Aconitum carmichaelii (Fuzi), decoct and take; the divine effect cannot be fully described.
[Baoming Ji] For diarrhea/dysentery with pus and blood, even rectal prolapse, use Diyug Shaoyao Tang (Sanguisorba and Peony Decoction).
Atractylodis Rhizoma Cangzhu (Cangzhu, 8 liang) Sanguisorbae Radix (Diyu) Selaginellae Herba (Juanbai) Paeoniae Radix (Shaoyao, 3 liang each)
Chop the above. Take 2 liang per dose, decoct in water, take warm. Stop when illness recedes.
[Worldly/Popular] For dysentery with involuntary bowel movements, anus open like a hollow cavity, mix scallion and Sichuan pepper powder, mash, and pack into the anal canal (Gudao). Also take astringent intestinal-strengthening agents like Papaveris Pericarpium (Yumiqiao) or Persimmon peel (Hezi pi); miraculous effect.
[Zhu Danxi] Chen Bofu, age twenty, impatient nature, fond of wine and sex, lived luxuriously. Due to worry and fear from affairs, he suffered from chronic malaria with chills and fever without fixed timing. Suddenly, one day he had high fever, passed extremely foul-smelling stagnant matter, anus extremely painful, groaning incessantly, anus sunken, making final arrangements. I said: "This is extreme deficiency. Pulse is wiry, large, and floating." I had tiles knocked round like copper coins, heated red, plunged into child's urine, quickly removed, dried, wrapped in paper, and applied to the painful area. As it was cold, fearing external cold invading deficiency, I gave a strong decoction of Ginseng Radix et Rhizoma (Renshen), Angelicae Sinensis Radix (Danggui), and Tangerine peel (Chenpi), advised a bland diet, and he recovered in half a month.
Chronic passage of red and white discharge, unbearable anal pain. Apply fried salt with heat, or apply roasted Fructus Aurantii Immaturus (Zhishi) with heat. (Zhouhou Beiji Fang)
Chronic dysentery with urgent anal pain, sometimes with chills and fever. Smoke with prepared Artemisia argyi (Shuai), yellow wax, and Persimmon peel (Hezi); miraculous. (Rihua Bencao)
Anal pain from dysentery due to heat flowing downward: Arecae Semen (Binglang), Aucklandiae Radix (Muxiang), Coptidis Rhizoma (Huanglian), Scutellariae Radix (Huangqin), add Dried Ginger (Ganjiang).
Yunqi (Five Movements and Six Qi) - Lower Bleeding has two types:
First, Heat assists Heart Blood excess leading to bleeding. Classic says: When Shaoyin governs the sky, excessive Heat causes people to suffer blood discharge, urine color changes. Also: When Shaoyang is in the spring, excessive Fire causes people to suffer red urine, severe cases bloody stool. Also: When Metal of the year is deficient,炎火 (flames/fire) prevails, people suffer bloody stool discharge. Also: When Jueyin governs the sky, Wind descends, Fire acts violently, red fluids flow down, treat with Salty-Cold methods.
Second, Cold attacks Heart Blood deficiency leading to bleeding. Classic says: When Taiyin governs the sky, excessive Cold causes blood changes internally, people suffer blood discharge. Also: Victory of Taiyang causes blood vessels to coagulate and stagnate, or result in blood discharge. Also: During Yangming governance, fourth Qi period, people suffer bloody stool, treat with various Hot methods.
Yunqi - White discharge belongs to Fire. Classic says: When Shaoyang governs the sky, guest victory leads to severe white discharge and hematuria.
Yunqi - Red and White discharge has three types.
First belongs to Fire. Classic says: When Shaoyang governs the sky, excessive Fire causes people to suffer discharge of red and white. Also: When Shaoyang is in the spring, excessive Fire causes people to suffer discharge of red and white, lower abdominal pain, discharge of red and white, treat with Sweet-Clear methods.
Second belongs to Wind: Classic says: Victory of Jueyin causes borborygmus and undigested stool diarrhea, lower abdomen discharges red and white, treat with Sour-Astringent methods.
Third belongs to Wind-Damp: Classic says: During Taiyang governance, fourth Qi period, Wind and Damp contend, people suffer discharge of red and white.
Yunqi - Tenesmus (Liji Houzhong) has two types:
First, Damp assists Spleen excess causing heaviness. Classic says: Victory of Taiyin causes lower abdominal fullness, heavy and stiff waist, internal discomfort, prone to watery diarrhea.
Second, Wind attacks Spleen deficiency causing heaviness. Classic says: When Jueyin governs the sky, excessive Wind causes people to suffer loose stools, masses, and urinary obstruction. "Mass" refers to tenesmus (Houzhong).
[Bencao] Jiuming Yannian Wan (Life-Saving Longevity Pill) Treats severe dysentery in men and women.
Coptidis Rhizoma (Huanglian) Dried Ginger (Ganjiang) Angelicae Sinensis Radix (Danggui) Colla Corii Asini (Ejiao)
Pulverize the above three items; boil Colla Corii Asini (Ejiao) in rice vinegar until completely dissolved, do not leave residue, then mix medicine with vinegar to make pills the size of paulownia seeds. Take 30 pills per dose, swallow with rice drink; very effective.
Another formula: Corni Fructus (Zhuyu), Coptidis Rhizoma (Huanglian), Colla Corii Asini (Ejiao), Atractylodis Macrocephalae Rhizoma (Baizhu) (equal parts, stir-fry together until yellow)
Pulverize into fine powder, make pills with wheat paste, size of paulownia seeds. Take 30 pills per dose, send down with aged rice drink. Children take 10 pills. (Chen Wuyue's version without Ejiao is named Wuji Wan.)
[Luo Tianyi] Ejiao Meilian Wan (Colla and Plum-Linked Pill) Treats dysentery, regardless of new or old, red or white, green or black, pain, etc.
Colla Corii Asini (Ejiao, stir-fry, grind extremely fine) Mume Fructus Carpel (Wumei Rou, stir-fry) Phellodendri Cortex (Huangbai, stir-fry) Coptidis Rhizoma (Huanglian) Angelicae Sinensis Radix (Danggui) Paeoniae Radix Rubra (Chishaoyao) Dried Ginger (Ganjiang, processed) Poria cocos with Red Core (Chifuling, equal parts each)
Pulverize, mix with Ejiao powder evenly, make water pills, size of paulownia seeds, take with rice drink.
Huanglian Wan (Coptis Pill) Treats red and white dysentery.
Evodiae Fructus (Wuzhuyu) Coptidis Rhizoma (Huanglian)
Soak both in good wine for three days, pulverize separately, make pills separately. For white dysentery, Evodia pill; for red dysentery, Coptis pill, take with Licorice decoction. For red and white dysentery, take both pills combined.
The above four formulas combine Cold and Hot, Astringent and Dispersing actions, suitable for dysentery mixed with Cold, Heat, Slippage, and Stagnation.
[Cui's Method] Dysentery prescription. Cut fresh ginger into hemp-seed sized pieces, mix with one or two bowls of tea, sip as desired until cured. For heat dysentery, keep ginger skin; for cold, remove skin and stir-fry. Another method: decoct Tangerine peel (Chenpi) and Artemisia argyi folium (Aiye) strongly and take.
For Zhixia (Dysentery), solely select the Foot-Taiyin channel. Classic says: The Spleen Foot-Taiyin meridian diseases include loose stools, masses, and watery diarrhea; treat based on excess/deficiency, heat/cold, sinking/depression.
[Zhang Yuansu] Involuntary diarrhea/dysentery, lower abdominal pain, tenesmus, pus and blood in stool: Dantian (1.5 cun). Fuliuxiao (KI7) Xiaochangshu (BL27) (Moxibustion 7 cones). If not resolved, take Tianshu (ST25) Fu'ai (ST26) (below Stomach 1 cun 5 fen). Cold dysentery abdominal pain, discharge of red and white: Guanyuan (CV4) Qionggu (BL29) (Moxibustion 50 cones each). Diarrhea/dysentery with flatus and pus/blood: Xiayao (50 cones, point located in center of Baku, on spine, moxa, named Sanzonggu, apply three times). Pus/blood, chronic dysentery with heaviness: Xiaochangshu (BL27) (Moxibustion).
[Worldly/Popular] Tenesmus (Liji Houzhong): Hegu (LI4) Waiguan (TE5)
[Jiayi Jing) Pus and blood in stool, cold in middle, undigested food, abdominal pain: Fu'ai (ST26) governs. Loose stools/masses, abdominal pain, organ bi, Dijī (SP8) governs.
[Zhu Danxi] My clan uncle had solid constitution, suffered malaria and dysentery, relied on strength and ability to eat, showing no caution. One day he summoned me: "Though healthy and able to eat, I suffer from sweating. Can you stop this sweat?" I said: "Malaria requires sweating to heal; the concern is only his robustness and appetite. This is not dysentery; it is Stomach Heat consuming rapidly, Spleen disease failing to transform, food and stagnation power is excessive. At this time, regulate diet to nourish Stomach Qi, reduce going out/in to avoid wind-cold, wait for sweat to penetrate and settle." Uncle said: "Secular belief is no one dies of fullness from dysentery; I can eat now, why worry?" I said: "Being able to eat in dysentery means Stomach Qi is not diseased, hence 'not dying', but does not mean one can eat indiscriminately without restraint." He did not listen, ate excessively, consumed fruits, for over a month. Later forced to seek treatment, impossible to handle, languished for another month and died. Neijing considers arrogant and unreasonable behavior as incurable disease, truly so.
Another Zhou surname patient, solid constitution, suffered dysentery, ate well and got hungry easily, ate indiscriminately for five months. I reprimanded him: "During illness, one should regulate and nourish oneself, how can one indulge flavors and harm the body?" I taught him to only eat熟 radish congee, provide mild treatment, and he recovered in half a month.
[Luo Tianyi] If diarrhea/dysentery stops, Spleen/Stomach is weak, difficult to tolerate food/drink, do not uniformly use attacking/draining agents; instead, tonify and nourish sufficiently, naturally appetite returns. Use Yigong San (Extraordinary Power Powder) from Qian Yi's formulas. However, if excessive indulgence in food damages Spleen/Stomach, causing epigastric/abdominal fullness, nausea, vomiting, this case is excluded. Temporarily use Jupi Zhizhu Wan (Tangerine Peel and Atractylodes Pill), one dose brings relief, do not take again. If diet is regulated without damage, Stomach Qi will naturally harmonize.
[Zhu Danxi] For severe chronic dysentery, cold-cooling medicines must be combined with ascending/dispersing and hot agents. In initial dysentery, do not use Ginseng (Renshen) or Atractylodes (Baizhu), except for Qi-deficient patients.
[Yanyi] A man suffered bloody dysentery in summer, doctor wrongly used cold medicines to counteract, specifically using Aucklandiae Radix (Muxiang), Coptidis Rhizoma (Huanglian), and Colla Corii Asini (Ejiao). These drugs are acceptable if used at onset, but if illness is prolonged and intestines are empty/weak, they are unsuitable; strictly beware. Sanguisorbae Radix (Diyu) is deep, cold, bitter; usable only for lower jiao heat and bloody dysentery. For deficiency-cold persons and watery white dysentery, do not use lightly.
[Zhu Danxi] Ancient formulas use Magnolia officinalis bark (Houpo) specifically to drain Qi of stagnation. However, Houpo is greatly warm and disperses Qi; long-term use greatly weakens the person. Remove once stagnant Qi moves slightly. If residual stagnation remains, use stir-fried Aurantii Fructus Immaturus (Zhike) and Tangerine peel (Chenpi). However, Zhike also consumes Qi; though slower than Houpo, it is heavier than Chenpi. If stagnant Qi retreats by half, remove it, use only Chenpi to harmonize other drugs. However, Chenpi without white pith has both tonifying and draining capabilities. If used as assistant to Renshen/Baizhu, it acts purely as a tonifying drug.
