[Danxi] Chen Ya'er, aged nineteen, presented with chickenpox/smallpox accompanied by red macules, vomiting, diarrhea, and thirst.
Prescription: Cangzhu (Atractylodis Rhizoma, 1 qian), Baizhu (Atractylodis Macrocephalae Rhizoma, 3 qian), Huangqin (Scutellariae Radix, 1.5 qian), Chenpi (Citri Reticulatae Pericarpium, 2 qian), Fuling (Poria), Zhigancao (Prepared Glycyrrhizae Radix et Rhizoma), Suosha (Amomi Fructus Rotundus), Danggui (Angelicae Sinensis Radix; each 1.5 qian).
Three slices of Shengjiang (Zingiberis Rhizoma Recens), decocted together and taken orally.
The son of Zhu Zhenheng suffered from smallpox at the age of six or seven, presenting with fever, slight thirst, and spontaneous diarrhea. A physician surnamed Xiao examined his pulse and prescribed Muxiang San (Aucklandiae Powder), adding ten grains of Dingxiang (Caryophylli Flos) to each dose. I privately harbored doubts. Observing that the eruption was delayed, I inquired and learned that although he had diarrhea, his qi was weak. Examining his excrement, it was foul, stagnant, and contained old accumulations, indicating heat steaming within the intestines and stomach, rather than a cold or deficiency pattern. Thus, I urgently stopped the previous medication (one dose having been administered). Subsequently, I used Huanglian Jiedu Tang (Coptis Decoction for Detoxification) added with Baizhu (Atractylodis Macrocephalae Rhizoma), administered for ten doses, to resolve the heat induced by Dingxiang. The diarrhea ceased, and the eruptions emerged. Afterwards, his skin often had slight heat, and carbuncles and furuncles appeared on his hands and feet. He was treated with cooling agents combined with tonifying herbs, and recovered after more than a month.
A man in his early teens contracted smallpox. During the febrile stage, the eruptions emerged densely at the base. He experienced vomiting, inability to eat, pain in the lower back, back, and joints, extreme thirst, sore throat, and abdominal pain. He could not eat anything for over half a month. His pulse was floating, wiry, surging, and rapid. I prescribed Shenren (Ginseng Radix), Huangqi (Astragali Radix), Danggui (Angelicae Sinensis Radix), Baizhu (Atractylodis Macrocephalae Rhizoma), Zhigancao (Prepared Glycyrrhizae Radix et Rhizoma), Chenpi (Citri Reticulatae Pericarpium), Banxia (Pinelliae Rhizoma), Fuling (Poria), and Shengjiang (Zingiberis Rhizoma Recens), decocted and taken orally. By the fifth day, the color of the eruptions became pale. I added a small amount of Guizhi (Cinnamomi Ramulus), and reused Danggui (Angelicae Sinensis Radix) and Huangqi (Astragali Radix) prepared with wine. By the seventh day, itching was severe. I added three grains of Dingxiang (Caryophylli Flos) and a small amount of Fuzi (Aconiti Lateralis Radix Praeparata), which stopped the itching. By the eighth and ninth days, thirst became intense, accompanied by abdominal diarrhea and severe itching. On the tenth day, chills occurred. I then used Baizhu (Atractylodis Macrocephalae Rhizoma) as the sovereign herb, with Huangqi (Astragali Radix), Chenpi (Citri Reticulatae Pericarpium), Fuling (Poria), Zhigancao (Prepared Glycyrrhizae Radix et Rhizoma), Huangqin (Scutellariae Radix), and Danggui (Angelicae Sinensis Radix) as minister herbs. By the eleventh day, the scabs had not formed; there was occasional itching and delirious speech. I adhered strictly to the previous prescription. After half a month, he naturally vomited a large amount of phlegm and recovered.
[Chen Wenzhong] Shi Yi Wei Muxiang San (Eleven-Ingredient Aucklandiae Powder) treats abdominal distension and thirst associated with rashes and smallpox, with efficacy like magic. (It is essential to carefully ascertain if it is a cold pattern: six pulses are deficient and fine, the body is not very hot, or there is aversion to cold, vomiting, abdominal pain, or coldness of the entire body and four limbs. Only in such cases can this formula be used. From *Jiyan*.)
Muxiang (Vladimiriae Radix), Dafupi (Arecae Pericarpium), Renshen (Ginseng Radix), Guangui (Cinnamomi Cortex), Chifuling (Poria cum Rubro), Qingpi (Citri Reticulatae Pericarpium Viride, white part removed), Qianhu (Peucedani Radix), Hezi Pi (Terminalia Chebulae Pericarpium), Banxia (Pinelliae Rhizoma), Dingxiang (Caryophylli Flos), Zhigancao (Prepared Glycyrrhizae Radix et Rhizoma; each 3 liang).
The above ingredients are chopped. Take 2 qian per dose, add one cup of water and three slices of Shengjiang (Zingiberis Rhizoma Recens), decoct, and take warm on an empty stomach.
Yigong San (Isolated Efficacy Powder) [from *Jiyan Fang*] treats smallpox where the head is warm and feet are cold, accompanied by abdominal distension, diarrhea, and thirst. (Formula see "Smallpox Eruption Not Rapid".)
Zhang Xunzhi stated: Vomiting, diarrhea, thirst, and reduced food intake indicate interior deficiency. Treat with Yigong San (Isolated Efficacy Powder) minus Guangui (Cinnamomi Cortex). For this condition involving diarrhea and thirst, honey water should not be consumed. If the erupted smallpox lacks luster, fails to rise, has non-red bases, or presents with diarrhea/thirst, abdominal distension, or shortness of breath, this indicates deficiency in both exterior and interior. Administer Yigong San (Isolated Efficacy Powder) with Seven-Ingredient Myristica Pill (Qi Wei Rou Dou Kou Wan) delivered via powder suspension. (Yigong San formula see "Smallpox Eruption Not Rapid".)
