(Similar to the effects of *Forsythiae Fructus* [Lianqiao, 连翘], do not use if the sore is not in the Shaoyang meridian) *Arctii Fructus* [Shuniuzi, 黍粘子] (3 fen, do not use if there is no swelling) *Laminariae Thallus* [Kunbu, 昆布] (2 fen, salty taste; use if the sore is extremely hard, as saltiness can soften hardness) *Bulbus Sparganii* [Jingsanleng, 京三棱] (processed, 2 fen) *Rhizoma Curcumae Longae* [Guangzhu, 广术] (3 fen. These two herbs are used when sores are extremely hard; do not use if not hard) *Notopterygii Rhizoma et Radix* [Qianghuo, 羌活], *Angelicae Dahuricae Radix* [Duhuo, 独活], *Saposhnikoviae Radix* [Fangfeng, 防风] (each 1 qian; these three herbs must be associated with Taiyang syndrome of the hand and foot presenting with back pain, neck stiffness, inability to look back, waist feeling like broken, neck feeling like pulled; *Saposhnikoviae Radix* is acrid and warm; even if the sore is above the diaphragm without Taiyang symptoms, it should still be used because it can dissipate nodules and remove wind pathogenic factors from the upper part) *Alpinae Oxyphyllae Fructus* [Yizhiren, 益智仁] (2 fen; add if excessive saliva indicates stomach disharmony, or if vomiting foam/food due to cold in the stomach; do not use otherwise) *Hordei Fructus Germinatus* [Maiya, 麦芽] (1 qian, promotes digestion and strengthens spleen) *Medicated Leaven* [Shenqu, 神曲] (fried, 2 fen, use for indigestion) *Coptidis Rhizoma* [Huanglian, 黄连] (fried, 3 fen, treats irritability and oppression) *Magnoliae Officinalis Cortex* [Houpo, 厚朴] (processed with ginger juice, 1.2 qian, add if abdominal distension occurs) *Phellodendri Cortex* [Huangbai, 黄柏] (fried, 3 fen, add if heat or weakness in legs/feet; if irritable and wanting to remove clothes, this indicates latent fire in the kidney, so it should definitely be added)
Grind all the above into fine powder. Soak steamed wheat cake in decoction, shape into cakes, dry them, and crush into particles the size of rice grains. Take 3 qian per dose, washed down with plain boiled water. If qi is not smooth, add *Citri Reticulatae Pericarpium* [Juhong, 橘红]; if severe, add a little *Aucklandiae Radix* [Muxiang, 木香]. Adjust according to the patient's deficiency or excess; do not let the dosage be too high to interfere with diet. This is the general principle of treatment. If the disease is only in the Yangming branch, remove *Bupleuri Radix* [Chaihu, 柴胡] and *Arctii Fructus* [Shuniuzi, 黍粘子], use the rest. If in the Shaoyang branch, termed "Madao Jia Ying" (Marble Knife with Goiter), remove *Angelicae Dahuricae Radix* [Duhuo, 独活], *Rhapontici Radix* [Loulu, 漏芦], *Bupleuri Radix* [Shengma, 升麻], *Puerariae Radix* [Gegen, 葛根], and add *Cistanches Herba* [Qumai, 瞿麦] 3 fen. If the patient is constitutionally weak but the disease onset shows strong qi and no shortness of breath, do not rely solely on their usual constitution; instead, treat it as an excess condition based on the disease change. Add *Scutellariae Radix* [Huangqin, 黄芩], *Coptidis Rhizoma* [Huanglian, 黄连], *Phellodendri Cortex* [Huangbai, 黄柏], *Anemarrhenae Rhizoma* [Zhimu, 知母], *Stephaniae Tetrandrae Radix* [Fangji, 防己], selecting based on whether the pathogen is in the Upper, Middle, or Lower Jiao. For example, add *Scutellariae Radix* [Huangqin, 黄芩] for Upper Jiao (half wine-washed, half raw); *Coptidis Rhizoma* [Huanglian, 黄连] for Middle Jiao (half wine-soaked, half raw); *Phellodendri Cortex* [Huangbai, 黄柏], *Anemarrhenae Rhizoma* [Zhimu, 知母], *Stephaniae Tetrandrae Radix* [Fangji, 防己] for Lower Jiao. If constipation exacerbates pathogenic excess, add wine-processed *Rhei Radix et Rhizoma* [Dahuang, 大黄] to promote purgation. If blood dryness causes dry stool, add *Persicae Semen* [Taoren, 桃仁] paste and *Rhei Radix et Rhizoma* [Dahuang, 大黄]. If wind-stagnation causes dryness and blockage, add *Cannabis Semen* [Maren, 麻仁] and *Rhei Radix et Rhizoma* [Dahuang, 大黄] to moisten. If wind-dryness prevents bowel movement, add baked *Gleditsiae Sinensis Spina* [Zaorén, 皂角仁], *Rhei Radix et Rhizoma* [Dahuang, 大黄], *Gentianae Macrophytae Radix* [Qinjiao, 秦艽] to facilitate movement. If pulse is choppy and body feels qi-stagnated with constipation, add *Prunus mume* [Yuliren, 郁李仁] and *Rhei Radix et Rhizoma* [Dahuang, 大黄] to dispel wind-dryness. For Yin-cold syndrome causing constipation due to cold stagnation, use *Banliu Wan* (Half-Sulfur Pill) from the Bureau Formula, or decoct processed *Aconiti Lateralis Radix Praeparata* [Fuzi, 附子] and *Zingiberis Rhizoma* [Ganjiang, 干姜], and take cold. Generally, medication principles apply not only to sores but to all diseases where constitutional qi is weak; remove bitter-cold agents and add more *Ginseng Radix* [Renshen, 人参], *Astragali Radix* [Huangqi, 黄芪], *Glycyrrhizae Radix et Rhizoma* [Gancao, 甘草] to drain fire and tonify primal qi. The rest follows this pattern. [Ben Cao] Treats Shuluo (Scrofula).
*Tuber Aconiti Kusnezoffii* [Tufuzi, 土附子] (1 piece) *Halitum* [Shiyan, 食盐] (3 sheng) *Urine* [Xiaobian, 小便] (5 sheng)
Soak the above three together for half a month, then remove. Remove black skin of the tuber, dry in shade, grind to powder. Boil black beans until soft, grind into paste, coat the powder to make pills the size of paulownia seeds. Take 10 pills swallowed with wine, morning and evening.
[Danxi] For initial hard head/neck lesions not yet ulcerated, effect is miraculous. *Ostreae Concha* [Muli, 牡蛎] 4 liang, *Glycyrrhizae Radix et Rhizoma* [Gancao, 甘草] 2 liang, grind to powder. Take 1 da qian per dose, mixed with aged tea after meals, twice daily. Root cured in half a month. (From Chu Yushi's formula, Danxi used this for Shi Yi Da Guan's sore.) Effective for all men and women with scrofula. Calcine *Ostreae Concha* [Muli, 牡蛎] until red hot, remove from wet ground, place on paper overnight to remove fire toxicity. Take 4 liang, *Scrophulariae Radix* [Xuanshen, 玄参] 3 liang, grind together. Make pills with flour paste, size of paulownia seeds. Take 30 pills after meals and before bed with wine. Cure root by end of course. (From *Experience Formula*)
[Zhoubi] Treat submandibular scrofula the size of a plum pit; eliminate quickly. *Sargassum Herba* [Haizao, 海藻] 1 jin, wine 1 sheng, soak for several days, drink slowly.
[Yanyi] Lightly fry *Polygoni Orientalis Fructus* [Liaozizi, 蓼子], grind to fine powder. Mix with mint wine, take 2-3 qian. Effective for long-term scrofula.
[World Doctors] Treat scrofula. Use *Albiziae Cortex* [Yehé Cǎo, 夜合草] (leaves close at night, named Hemim in Materia Medica, common name Lianqianmu, grows in rice fields on water surface). Crush leaves to extract natural juice, drink it; apply dregs to affected area.
[Baoming Ji] Wenwu Gao treats scrofula.
Use 2 dou of ripe black mulberries, squeeze juice through cloth bag, boil in stone vessel into paste. Dissolve one spoon in boiled water, take three times daily. Red ones dry, grind to powder, mix with water.
[Shengji Zonglu] Treat scrofula, swollen/hard/painful, chronic non-healing. Roast wild cat head/hoof bones with酥 (butter/oil) until yellow, grind to powder. Take 1 qian bish empty stomach with porridge drink.
[Ling Shu] Yellow Emperor asked: What qi causes cold-heat scrofula in neck/axilla? Qibo answered: These are toxins from mouse-lobe cold-heat, staying in vessels. Root in zang organs, manifest in neck/axilla. Floating in vessels, not internalized in muscles, externalizing as pus/blood is easy to remove. How to remove? Guide from root to tip to decline disease and cut off cold-heat. Carefully follow the path, slow insertion/removal. Small like wheat grain: one puncture sees effect, three punctures cure. (Cold-Heat Chapter)
This chapter uses meridians to treat scrofula tips via zang organ roots. Tips appearing below/behind ear, down neck to shoulder, or into Suken (supraclavicular fossa): treat Hand/Foot Shaoyang meridians, needle or moxa as follows.
[Bian Que] Scrofula: Tianjing, Jianjing.
[Cuyao] Scrofula: Tianjing (0.5 cun, moxa 7 cones, reducing method)
[Dongyuan] Axillary swelling, Madao Jia Ying, self-biting tongue/cheek, Tianyou swelling, cold-heat: Linqi, Qiuxu (each 1 fen, moxa 5 cones). Taichong (1 fen, moxa 3 cones). Axillary/neck swelling: Tianchi (parallel skin 0.3 cun, moxa 7 cones). If jaw swollen, add Houxi (2 fen, moxa 5 cones). Axillary swelling/Madao Jia Ying/Sore throat: Yangfu (0.5 cun, moxa 14 cones). Shenmai (1 fen, moxa 3 cones, immediate cure).
[Jia Yi Jing) Chest fullness, axillary swelling, Madao Scrofula, self-biting tongue/cheek, Tianyou swelling, limb acid pain, dizziness, occipital/jaw/cheek pain, dry eyes, body bi, chills, hypochondriac fullness, cold-heat, lateral costal/waist/abdomen/knee pain: Linqi governs. Madao swelling/scrofula: Yuanlin, Zhangmen, Zhigou governs. Emerging from chin to Jiache: treat Hand/Foot Yangming meridians, needle/moxa: Zusanli (Foot Yangming), Hegu (Hand Yangming).
[Danxi] Crush fresh *Phytolaccae Radix* [Shanglu, 商陆] root into cake, place on scrofula, moxa on cake. Replace when dry. Moxa 3-4 cakes. (From *Waitai Miyao*)
[World Doctors] Moxa directly on flesh over lesion 14 cones, miraculous effect.
[Dongyuan] Scrofula/Madao: Let one sore emerge first. Fix with square iron ring to prevent migration. Cut open, insert oil-paper wick to keep open, cutting off source. Fast effect. If not breaking or patient refuses, apply medicine, change every 3 days. Main formula: Longquan San (see Biao-Li section).
[World Doctors] Scrofula often on shoulder/neck, red/white, deep/superficial. Starts like bean, becomes nodule. Over years, size of plum or chicken egg, arranged in rows. Commonly called Pan She Luo (Coiled Snake Scrofula). Mental exertion/overthinking leads to pain/swelling. Early treatment best. Liuzhu Scrofula common in women. Irritable nature, qi stagnation, obsessive mind. Starts on neck, after rupture pus flows to limbs, systemic toxicity like plum/apricot. Unhealed ruptures connect holes, cold-heat pain, pus flow. Named Liuzhu Scrofula or Thousand-Year Sore. Take Tuoli Jiuku Shenyin Wan.
*Radix Aconiti Carmichaelii* [Chuanwu, 川乌] (peeled, raw, 1 liang) *Radix Aconiti* [Wutou, 乌头] (5 liang) *Angelicae Sinensis Radix* [Danggui, 当归] (wine soaked overnight) *Myrrha* [Moyao, 没药] *Angelicae Dahuricae Radix* [Baizhi, 白芷] *Citri Reticulatae Pericarpium* [Chenpi, 陈皮] *Glycyrrhizae Radix et Rhizoma* [Gancao Jie, 甘草节] (each 1 liang) *Bombyx Batryticatus* [Chantui, 蝉蜕] (washed, 0.5 liang) *Gleditsiae Sinensis Fructus* [Da Zaorao, 大皂角] (7 pieces, peeled) *Curcumae Longae Rhizoma* [Jianghuang, 姜黄] (1.5 liang)
Crush gourd melon, decoct in 4 bowls water to 2 bowls, filter. Decoct Wutou/Chuanwu in filtrate until soft, mash to paste. Other herbs grind separately, mix with paste to make paulownia-sized pills. Take 60 pills with mint tea regardless of hunger/fullness.
If ulcerated with holes, fleshy growth like finger appears (black/white), due to wind/qi conflict. Use wind-dispelling/qi-regulating formula: Huaqi Tiaojing Tang.
Alternate Huaqi Tiaojing Tang and Shenyin Wan for Liuzhu Scrofula.
*Aucklandiae Radix* [Xiangfu, 香附] (wine soaked overnight, dried, 1 liang) *Citri Reticulatae Pericarpium* [Jupi, 橘皮] (2 liang) *Notopterygii Rhizoma et Radix* [Qianghuo, 羌活] (1 liang) *Angelicae Dahuricae Radix* [Baizhi, 白芷] (1 liang) *Glycyrrhizae Radix et Rhizoma* [Gancao, 甘草] (0.5 liang) *Ostreae Concha* [Muli, 牡蛎] (calcined, 0.5 liang) *Trichosanthis Radix* [Tianguafen, 天花粉] *Gleditsiae Sinensis Spina* [Zaocishi, 皂角刺] (each 0.5 liang)
Grind to powder. Take 2 qian with clear water anytime, 3 times daily.
If pulse is forceful, first use Zhuidu Shenyi Tang, then Jiuku Shenyin Wan.
*Cinnabar* [Chensha, 辰砂] *Dragon's Blood* [Xuejie, 血竭] (each 1 qian) *Moschus* [Shexiang, 麝香] (1 zi, fine powder) *Rhei Radix et Rhizoma* [Dahuang, 大黄] (0.5 liang) *Glycyrrhizae Radix et Rhizoma* [Da Gancao Jie, 大甘草节] (0.5 liang)
Coarse powder, decoct 1 bowl river water to 0.5 bowl. Mix with fine powder, take before bed.
Single-nest scrofula: single lesion on neck, hardest to treat. Take previous meds, will resolve over time. Occurring on fontanelle also hard to treat. Use toxic drugs, avoid prolonged soaking.
Lotus seed scrofula: capsule holds dozens, on sides of neck, movable by touch. Treatable. Hard/imovable means incurable. Chills/fever, dry thirst: very hard to treat, even sages cannot help.
Chongtai Scrofula: on neck, left/right. Single nest stacked top/bottom. Incurable by meds/acupuncture. Toxic spread causes severe swelling/pain/thirst/phlegm. High mortality. Early detection: use Xiaoxijiao Wan, Fenjin San.
Yanke Scrofula: shaped like swallow nest, incurable.
Lung pulse slightly choppy indicates Mouse Scrofula between neck/axilla. Lower body cannot support upper. Corresponds to sour taste. (See Deficiency/Excess methods. White color, choppy pulse.)
[Ling Shu] Yellow Emperor: How to determine life/death? Qibo: Look at eyes. Red vessels piercing pupil: 1 vessel = death in 1 year. 1.5 = 1.5 years. 2 = 2 years. 2.5 = 2.5 years. 3 = 3 years. Red vessels NOT piercing pupil: treatable. (Cold-Heat Chapter)
[Danxi] Scrofula starts in Shaoyang. Ignoring taboos spreads to Yangming. Rich food/stagnant qi create toxin/wind/heat. Must distinguish deficiency/excess. Excess is easier to treat, but requires strict diet/lifestyle changes. Belongs to Gallbladder meridian, governs decision-making, has Ministerial Fire, qi abundant/blood scarce. Women: if menstruation occurs without cold-heat, survival likely. Prolonged case turns to tidal fever, critical. Without abstinence/dietary blandness, even sages cannot cure.
