If the condition has not yet reached a critical state involving the head, and lies between deficiency and suspicion of phlegm, administer only thirty or fifty pills. The signs of life will be seen immediately, after which the dosage should be gradually increased until recovery is achieved. Concurrently, take herbs that generate fluids, resolve phlegm, warm the middle, and regulate qi. For all lingering diseases, whether in men or women suffering from non-Shanghan internal and external conditions; or due to excess in alcohol and sexual activity; or hematemesis; or delayed menstruation with vexation and mental confusion; or abdominal distension and hypochondriac pain with fatigue-induced dizziness and phlegm; or sudden walking in the midday sun causing latent summer-heat phlegm. Manifestations such as deviation of the mouth and eye, eye pain, ear fullness, nasal congestion, bone and joint soreness, dry heaving, nausea, various internal and external pains, where hundreds of medicines have failed and many physicians cannot identify the cause, shall be treated by modifying and administering the aforementioned method with good effect.
Generally, medication should be taken before retiring to bed. Swallow the pill with about one mouthful of hot water, then lie down immediately, allowing the pill to remain between the throat and diaphragm, descending slowly. If a disease arises suddenly during the day with unbearable pain requiring urgent elimination, one must strictly follow the supine administration method. For half a day thereafter, no food or drink should be consumed, nor should one rise, walk, sit, or speak. One must wait until the pill has expelled the upper jiao phlegm stagnation and foul matter past the diaphragm into the abdomen before moving; only then will it hit the target. Take twice nightly. If the next day three to five bowel movements of phlegm occur, continue the previous dose; if five to seven movements occur, or only two or three but the disease势头 subsides suddenly, reduce the dose by twenty pills the next night; if no foul matter was expelled after the first night's dose, increase by ten pills the next night. For robust individuals with excess disease, increase up to one hundred pills. Adjust according to deficiency and excess.
If after taking the medicine in a supine position, one feels thick, sticky obstruction in the throat making breathing difficult, this is due to phlegm and qi surging upward, a result of the medicine fighting the disease. After a short while, when the medicinal power prevails, calmness will naturally return. Often, long-standing disease results in solid phlegm accumulation between the lungs and stomach, or acute disease without widespread flooding; upon swallowing the medicine and lying supine, the whole body becomes quiet and comfortable, and the five zang organs become clear and peaceful. The next morning, pass stool once; subsequent passages consist entirely of phlegm, mucus, and foul matter. Observe the stool; stirring it with water reveals it consists entirely of phlegm flakes and viscous saliva. If there is slight abdominal pain or stiffness in the lower back and kidneys, this is due to stubborn phlegm and foul matter blocking qi and lubricating the intestines, causing tenesmus resembling dysentery, which passes quickly. If phlegm and saliva are easy to expel, the relief is indescribable, with saliva filling the mouth and the whole body feeling refreshed. Occasionally, there may be momentary fatigue because the body has been uneasy due to illness for days; suddenly overcome by the medicinal power, the qi and blood temporarily harmonize, like waking from drunkenness, emerging from a bath, or rising from sleep. This medicine does not cause severe diarrhea or intestinal scraping, but rather takes phlegm accumulation and foul matter, expelling them sequentially from the intestines and stomach, without injuring the fecal waste in the abdomen. The feces pushed out from the intestines and abdomen are those where the medicinal power reaches, thus removing the feces first. The remaining details cannot be fully described; the user will know through experience.
