[Danxi] The five types of jaundice need not be distinguished; they are all caused by damp-heat, similar to the fermentation of yeast. For mild cases, use Xiaowenzhong Wan (Minor Warming Middle Pill). For severe cases, use Dawenzhong Wan (Major Warming Middle Pill). If heat is predominant, add Huangqin (Scutellariae Radix) and Lianlian (Coptidis Rhizoma). If dampness is predominant, use Yinchen Wuling San (Artemisiae Scopariae and Five-Ingredient Poria Decoction) combined with herbs for food stagnation. If damp-heat damages stomach qi, and purgatives cause diarrhea, use Renshen (Ginseng Radix et Rhizoma), Yinchen (Artemisiae Scopariae Herba), Shanzhi (Gardeniae Fructus), and Gancao (Glycyrrhizae Radix et Rhizoma).
[Zhongjing] In all cases of jaundice, promoting urination is the primary treatment. If the pulse is floating, release the exterior through sweating; Guizhi Jia Huangqi Tang (Cinnamon Twig Decoction plus Astragalus) is appropriate. Gu Dan (Grain Jaundice) presents with aversion to cold, fever, anorexia, dizziness upon eating, and mental unease; if it persists and causes yellowing, it is Gu Dan, treated with Yinchenshao Tang (Artemisiae Scopariae and Rhubarb Decoction). For general jaundice, Yinchens Wuling San (Artemisiae Scopariae and Five-Ingredient Poria Decoction) is indicated. (Both formulas are found in Shanghan Lun.)
[Luo Tianyi] Fuling Chushi Tang (Poria Decoction for Removing Dampness) treats jaundice with qi-heat, vomiting, thirst with desire for cold drinks, yellowing of the body and face, difficult urination, inability to lie down comfortably, and lack of appetite.
Baifuling (Poria) (5 fen), Zexie (Alismatis Rhizoma) (3 fen), Yinchenshao (Artemisiae Scopariae Herba) (6 fen), Zhuling (Polyporus) (3 fen), Huangqin (Scutellariae Radix, raw), Lianlian (Coptidis Rhizoma), Shanzhi (Gardeniae Fructus), Fangji (Stephaniae Tetrandrae Radix), Baizhu (Atractylodis Macrocephalae Rhizoma), Cangzhu (Atractylodis Rhizoma), Chenpi (Citri Reticulatae Pericarpium), Qingpi (Citri Reticulatae Pericarpium Viride) (2 fen each).
Chop the herbs, decoct in water, and take on an empty stomach.
Wanyan Zhengqing, due to official duties and overwork, irregular diet, heart fire invading the spleen, and spleen qi deficiency, further injured by anger causing qi rebellion damaging the liver, developed epigastric fullness and stuffiness, fatigue in the four limbs, and body numbness. This progressed to yellowing of the body and eyes, slightly greenish, dark complexion, mental restlessness, palpitations, nausea with foul foam in the mouth, indigestion, passage of undigested food, oliguria with dark red-black urine, fever between Chen and Si hours (7-11 AM) resolving at dusk, worsening in April. His pulse was floating and moderate. The *Jingui Yaolue* states: "Floating pulse at Cun indicates wind, moderate pulse indicates Bi syndrome; Bi is not wind-stroke, but limbs suffer from vexation, spleen color turns yellow, stasis-heat has moved." Tight pulse at Fuyang with dizziness after eating indicates spleen injury; wind-cold combusting, grain qi fails to digest, stomach becomes turbid, turbid qi does not flow, urination stops, yin is affected by cold, heat flows into the bladder, body turns yellow, named Gu Dan. Treat with Yinchenshizi Tang (Artemisiae Scopariae and Gardeniae Decoction).
Yinchen (Artemisiae Scopariae Herba) (1 qian), Fuling (Poria) (5 fen), Shanzhiren (Gardeniae Fructus), Cangzhu (Atractylodis Rhizoma, stir-fried), Baizhu (Atractylodis Macrocephalae Rhizoma) (3 qian each), Huangqin (Scutellariae Radix, raw, 6 fen), Lianlian (Coptidis Rhizoma), Zhishi (Immature Bitter Orange Fruit, stir-fried), Zhuling (Polyporus), Zexie (Alismatis Rhizoma), Chenpi (Citri Reticulatae Pericarpium), Fangji (Stephaniae Tetrandrae Radix) (2 fen each), Qingpi (Citri Reticulatae Pericarpium Viride) (1 fen).
Chop, decoct once in long-flowing water, take warm before meals. Two doses will cure. Shanzhi and Yinchen clear damp-heat and resolve jaundice, serving as Sovereign herbs. Zhishi is bitter and cold, draining epigastric fullness; Lung governs Qi, heat damaged Qi causing numbness, so Huangqin clears fire and tonifies Qi as Minister. Ershu (Baizhu and Cangzhu) are bitter, sweet, and warm; Qingpi is bitter, pungent, and warm, clearing stomach damp-heat, draining stagnation, and nourishing righteous Qi. Fangji is bitter and cold, removing stagnant dampness from the twelve meridians. Zexie is salty and neutral; Fuling and Zhuling are sweet and neutral, guiding damp-heat from the bladder, promoting urination, and relieving oliguria.

[Danxi] Treating a 20-year-old who contracted jaundice from labor and rain exposure. Symptoms: sore legs, palpitations, bitter taste, weakness, yellow urine, floating and rapid pulse.

Jiudan (Wine Jaundice) may have no heat, clear speech, abdominal fullness, desire to vomit, dry nose. If pulse is floating, vomit first; if sinking and string-taut, purge first. [Zi He] Cai's daughter had jaundice with generalized edema, golden face and eyes, fatigue, anorexia, craving raw foods and ash.
First, use bitter emetics made from steamed dough balls to vomit one bowl of phlegm; then use Zhouche Wan (Boat Vehicle Pill) and Tongjing San (Unblocking Meridians Powder) to purge five to seven motions of black stool-like matter; then use Daoyin Wan (Guiding Drink Pill) to grind away food stagnation and qi constraint. Within a few days, she recovered completely.
Li Qilao had Yongshui Syndrome (Surging Water Syndrome): yellow face, asthma, swollen feet that pit and rebound, gurgling sound when walking, constant thirst, inability to sleep. Decoct in water; near completion, add Xiaoshi and take at once.
The Master said: Jaundice with fever, vexation, panting, chest fullness, and dry mouth results from using fire methods to force sweating during onset, combining two heats. However, jaundice arises from dampness; whole-body heat, yellow face and belly heat indicate interior heat, which must be purged.
[Luo Tianyi] Huanglian San (Coptis Powder) treats jaundice with constipation and internal heat obstruction; highly effective.
Huanglian (Coptidis Rhizoma) (2 liang), Dahuang (Rhei Radix et Rhizoma, vinegar-mixed, stir-fried, 2 liang), Huangqin (Scutellariae Radix), Gancao (Glycyrrhizae Radix et Rhizoma, honey-fried, 1 liang each).

[Zi He] Zhao Junyu of Anxi had jaundice with generalized yellowing. Doctors claimed it was Yangming pattern. You and Mazhi Ji studied under Zhang Dai Ren and discuss taking Dahuang; hard to discuss illness with you. Junyu returned displeased, suspected no other illness, took Sanhua Shenyao Wan (Three-Flower Divine Protection Pill) 80 pills, no effect. He realized his damp-heat was severe; even this pill failed. He took Zhouche Wan and Xunchuan San, purging one dou of hardened stool, yellow resolved overnight. He trusted Dai Ren's skill more.
Above, if pulse is sinking and abdomen is unharmonious, use purging.
[Zhongjing] For all jaundices with abdominal pain and vomiting, use Xiaochaihu Tang (Minor Bupleurum Decoction). (Formula in Shanghan Lun.)
[Sun Simiao] To treat jaundice. Use Chaihu (Bupleuri Radix) 1 liang (remove sprouts), Gancao (Glycyrrhizae Radix et Rhizoma) 1 fen (fine cut), as one dose. Boil in one bowl of water with one handful of Baimaogen (Imperatae Rhizoma) to 70%, remove dregs, take anytime.
For all jaundices, difficult urination indicates interior excess; promote urination or purge. No sweating indicates exterior excess; sweat or vomit. (Methods see Table/Interior Jaundice sections.) For men with yellowing and spontaneous loose stools, give Xujianzhong Tang (Minor Construct the Middle Decoction) for deficiency consumption. (Formula in Shanghan Lun.)
[Hai Zang] Internal contraction of cold-heat, physical labor, irregular diet, middle jiao turning cold, generates jaundice. Not external contraction; only Jianzhong Tang, Lizhong Tang, or Dajianzhong Tang are sufficient; Yinchen is unnecessary.
[Zhongjing] Treating a 60-year-old woman with chills and tremors (Foot Taiyang Cold Water); yawning/sneezing (Foot Shaoyang Gallbladder); insufficient saliva (Foot Yangming deficiency); epigastric pain and jaundice (Hand Shaoyin cold, Foot Shaoyin blood stasis); body heat seeking fire (heat in skin, cold in bone); umbilical cold (Dantian cold); generalized and eye yellowing (cold-damp). Other symptoms confirm cold; dark red-black frequent urination confirms cold-damp excess. Heavy body like mountain, bed-bound indicates Yin-damp excess. Pulse: Right Guan/C/Mingmen string-taut and fine, pressing reveals surging/string-taut; string-taut/tight indicates cold, fine indicates Northern Cold Water. Mixed with excessive moderate indicates dampness producing yellow. Surging pulse indicates Heart Fire constrained. Left hand pressing to bone, returning solid indicates Ren/Gui Kidney prosperity. Six pulses void when pressed indicate Lower Jiao Yang deficiency. Treatment: Light agents to remove cold-damp, reduce surging pulse. Use Lizhong Jia Fuling Tang (Poria Decoction added to Lizhong Tang). (Note: Though urine is red, treat as cold based on other signs/pulse.)
Renshen (Ginseng Radix et Rhizoma), Baizhu (Atractylodis Macrocephalae Rhizoma), Gancao (Glycyrrhizae Radix et Rhizoma), Ganjiang (Zingiberis Rhizoma), Fuling (Poria).
Decoct, serve cold ("Heat by Cold" method). Counter Taiyang false heat with false cold; Ganjiang pungent-hot drains Bladder true qi. True matches true, false matches false. If ineffective, use Shufu Tang (Atractylodes and Aconite Decoction).
[Luo Tianyi] Yin Jaundice Case Study: June, Bingyin year. Continuous rain, damp epidemic. Han Junxiang of Zhending, overworked, drank cold tea, ate cold food. Headache, limb pain, heavy body, chest fullness, anorexia. Self-diagnosed external contraction. Used Tongsheng San twice, worsened. Doctor used Baijiesan to sweat. Four days later, Xiaochaihu Tang twice, added irritability. Six days later, Sanyi Chengqi Tang purged, extreme dry thirst. Added Baihu Jia Renshen Tang, Chaihu Yin, condition worsened. Changed doctor: Huanglian Jiedu Tang, Zhusha Gao, Zhibao Dan. After 17 days, condition worsened, developed yellow eyes/body, heavy limbs, back aversion to cold, cold skin, epigastric hardness/pain, gritty eyes, reluctance to speak, spontaneous sweating, free urination, incomplete bowel movements. I diagnosed tight/fine pulse, void on pressure, short Cun pulses. Cause: Heat season, cold drinks, excessive cold medicines, water attacking Heart, insulting Earth, imprisoning Mother, oppressing Son. Classic: Oppress invincible, attack victorious. Rain/cold-damp combine, Yin pattern jaundice. I used Yinchens Fuzi Ganjiang Tang (Artemisiae Scopariae, Aconite, and Ginger Decoction). Classic: Cold within, treat with sweet-hot, assist with bitter-pungent. Dampness excess, level with bitter-hot, leach with bland, dry with bitter. Fuzi/Ganjing pungent-sweet-hot scatter middle cold (Sovereign). Banxia/Caokou pungent-hot, Baizhu/Chenpi bitter-sweet-warm strengthen spleen/dry damp (Minister). Shengjiang pungent-warm scatters, Zexie bland-neutral leaches, Zhishi bitter-mild-cold drains fullness, Yinchen bitter-cold light floats, assisted by Jiang/Fu removes cold-damp from skin/subcutaneous tissue to resolve jaundice (Assistant/Envoy). One dose halved symptoms, two doses removed them. Followed by Lizhong Tang several doses, Qi restored. Objection: Jaundice is usually heat; now summer heat, why hot medicine? Answer: Reasonable. Cheng Wuji: Yin patterns: 1. External cold invading Yin meridians or cold food injuring Taiyin; 2. Initial Yang pattern treated with cold, excessive cold transforms Yang to Yin. Junxiang: Summer heat, cold food injures Spleen, excessive cold meds, Yin excess, Yang depletion, rain/cold-damp combine, produces jaundice. Zhongjing: Seek in cold-damp. Li Sishun: Sweating with excessive cold, purging harming ruler while chasing bandits. Sages' rules cannot be violated.

Above two formulas treat Deficiency-Heat Jaundice.
[Zhongjing] Jaundice with unchanged urine color, desire for loose stools, abdominal fullness, panting. Do not clear heat; clearing heat causes hiccups. Treat with Xiaobanxia Tang (Minor Pinellia Decoction). (Formula in Vomiting section. Unchanged urine means no heat in middle, thus clear/white, no heat to clear.) All jaundices with white urine cannot clear heat because there is no heat. If deficiency-cold signs exist, treat as deficiency consumption.
All jaundices with yellow-red urine are damp-heat. (Methods see Table/Interior/Deficiency Jaundice sections.)
