Treats women with alternating chills and fever (Han Re Wang Lai), or chills preceding fever, with heat resolving after sweating; or irregular menstruation (Yue Shi Bu Tiao) with scanty menstrual flow.
Sheng Huang Qi (Raw Astragali Radix, 5 qian) Sheng Di Huang (Raw Rehmanniae Radix, 6 qian) Xuan Shen (Scrophulariae Radix, 4 qian) Zhi Mu (Anemarrhenae Rhizoma, 4 qian) Dang Gui (Angelicae Sinensis Radix, 3 qian) Xiang Fu (Cyperi Rhizoma, 3 qian, vinegar-fried)
Chai Hu (Bupleuri Radix, 1.5 qian) Gan Cao (Glycyrrhizae Radix et Rhizoma, 1.5 qian)
If there is excessive sweating, replace Chai Hu with Yin Chen (Artemisiae Scopariae Herba) and add several qian of Yu Rou (Corni Fructus). If heat predominates, add several qian of Sheng Hang Shao (Raw Paeoniae Radix Alba). If chills predominate, add several qian of Sheng Jiang (Zingiberis Rhizoma Recens).
Regarding the syndrome of alternating chills and fever in women, medical texts offer various explanations. Some claim that deficiency in the Yang aspect causes chills in the forenoon, while deficiency in the Yin aspect causes fever in the afternoon. However, Yang qi is vigorous before noon, and declines after noon as Yin qi gradually increases. When Yang qi is vigorous, deficient areas can temporarily become filled. Why do the symptoms manifest inversely proportional to time? Others state that when the disease is in the Shaoyang channel, there is alternating chills and fever, similar to external pathogenic factors affecting Shaoyang; combining with Taiyang causes chills, and combining with Yangming causes fever. But internal injury diseases originally lack external pathogens. What combines with Taiyang and Yangming to produce chills and fever? Some argue that Liver deficiency causes intermittent fever and chills. I have verified this theory. Following the principle from *Shennong Bencao Jing* that Corni Fructus treats chills and fever, I used a single heavy dose of Corni Fructus (pitted, 2 liang) decocted in water, which cured the patient immediately (see case study under Laifu Tang). However, this represents extreme deficiency of Liver Wood with impending internal wind stirring, which cannot be equated with ordinary chills and fever. Generally, the Qi transformation of the human body is intimately connected with the Qi transformation of the seasons. The forenoon of a day corresponds to spring and summer of a year. Human Yang qi responds and activates to distribute throughout the body. Women are prone to worry and overthinking, leading to stagnation and blockage in organs and channels, obstructing Yang qi from reaching the exterior. Alternatively, due to activation, it may sink inward, or if activation is incomplete, the pushing against channels causes further blockage. Thus, cold pervades the body. When Yang qi accumulates to the extreme, it must eventually erupt violently. The mechanism of eruption clashes with the force of suppression within organs and channels, generating heat. This explains why morning chills transform into afternoon fever. Huang Qi is the chief herb for the Qi aspect; it tonifies Qi and raises Qi. Assisted by the uplifting nature of Chai Hu and the unblocking nature of Xiang Fu, the suppressed Yang qi is freely released. Since blood follows Qi, Qi stagnation leads to Blood stasis; thus, those with alternating chills and fever often have irregular menstruation and deficient Blood. Use Dang Gui to regulate it, Di Huang to tonify it, and Zhi Mu, Yuan Shen (Panacis Ginseng Radix Rubra or Codonopsis Radix depending on context, here likely referring to a specific variant or typo for Ren Shen/Dang Shen, but literally Yuan Shen), and Gan Cao to transform sweet and bitter into Yin to assist it, thereby nourishing the menstrual blood. Moreover, cooling herbs like Di Huang and Zhi Mu complement the warm-natured Huang Qi, making them excellent for harmonizing Yin and Yang and regulating chills and fever. As for the "afternoon fever" mentioned in texts, "Ribu" refers to the Shen hour, when the Foot-Shaoyin Kidney channel governs. If the person has Yin deficiency of the Kidney channel, the Fire of the Kidney channel may act at this time; this decoction can also treat it. Reduce Huang Qi by half, and use 1 liang of Di Huang. For amenorrhea complicated by masses (Zheng Jia) blocking Qi transformation causing chills and fever, use Lichong Tang later. For chest Great Qi sinking causing chills and fever, where the patient feels shortness of breath, use my proposed Shengxian Tang; see the treatment cases after the formula for reference.
[Attached Formula] Westerners' Rust Cinchona Pills are very effective for treating women's irregular menstruation, emaciation with cough and asthma, or intermittent chills and fever. The formula uses Iron Rust (Tie Xiu) and Myrrha (Moyao, avoid fire) each 1 qian, Cinchonae Cortex (Jinji Na Shuang) and Zanthoxyli Pericarpium (Hua Jiao) each 0.5 qian. Grind into fine powder, make pills with refined honey, 60 pills per batch. Take 3 to 5 pills per dose.
Iron rust is formed by the combination of iron and oxygen. Human blood turns red upon receiving oxygen. Iron rust contains oxygen, is red like blood, and smells slightly metallic/bloody, so it can nourish the Blood aspect and circulate meridians. Furthermore, human blood actually contains iron; using iron rust to tonify blood has the wonder of "tonifying iron with iron." Cinchona bark has functions in treating malaria. It is used here because it treats anemia well and enters the Hand and Foot Shaoyang channels to harmonize chills and fever. Zanthoxyli Pericarpium is added as an assistant because Cinchona is cold in nature; the pungent and hot Zanthoxyli balances it to achieve harmony.
East Asians have Zhongjiang Tang, which effectively regulates women's menses. My student Gao XX sent me the formula without dosages. I determined the dosages, and it was very effective. I list it below for reference.
Yan Hu Suo (Corydalis Rhizoma, vinegar-fried) 3 qian, Dang Gui (Angelicae Sinensis Radix) 6 qian, Guan Gui (Cinnamomi Cortex) 2 qian, Gan Cao (Glycyrrhizae Radix et Rhizoma) 2 qian, Ding Xiang (Eugeniae Flos) 2 qian, Shan Zha He (Crataegi Fructus core, vinegar-fried) 3 qian, Yu Jin (Curcumae Radix, vinegar-fried) 2 qian, Sha Shen (Glehniae Radix or Adenophorae Radix) 4 qian, Xu Duan (Dipsaci Radix, wine-fried) 3 qian, Rou Kou (Myristicae Semen) stir-fried with Chi Shi Zhi 3 qian (discard Chi Shi Zhi), Ku Shen (Sophorae Flavescentis Radix) 3 qian, Huai Niu Xi (Achyranthis Bidentatae Radix) 3 qian. Total 12 ingredients, crushed into coarse dregs, divided into 3 doses. For each dose, steep in boiling water in a covered bowl for about half an hour, then drink the liquid. Repeat this steeping method 2 to 3 times, finally decocting with water. Use one dose daily; after several doses, the menses will naturally regulate. This formula contains cooling and warming, tonifying and breaking, astringent and slippery herbs. The dosages I determined ensure balanced efficacy, so it can be taken for any woman's menstrual irregularity, especially effective for leukorrhea.
