The condition known as "Lai" (leprosy-like skin disease or severe dermatosis) is rarely recorded in medical texts. I initially also considered it similar to scabies or tinea, and thus not worthy of special attention. Since the year Wuwu (1918), when I practiced medicine in Fengtian (now Shenyang), I encountered several severe cases of Lai. Some patients had suffered for years, spent enormous sums on medication without cure, but under my treatment, all were completely cured after taking only a few doses of medicine. Later, a Mr. Rong from Jinzhou County suffered from this condition; having taken local medications with no effect, he came to Fengtian seeking my medical care. After taking six doses, he was cured. Three years later, his condition suddenly relapsed, starting on the face. The lesions resembled stubborn tinea; when scratched open, they exuded yellow fluid, while the unbroken skin peeled off in patches. The itching was unbearable, causing extreme distress, even manifesting as erratic weeping and laughing. He took over ten days of local medication with no effect and returned to Fengtian for further treatment. His pulse was surging and full (Hong Shi). He reported extreme mental restlessness, worse at night; the more his skin itched, the more agitated he felt, keeping him awake all night. He stated that if not cured soon, he would not be able to endure it. Therefore, I prescribed a formula using Snake Slough (Shetui) four pieces, Cicada Slough (Chantui), Silkworm (Jiangcan), Whole Scorpion (Quanxie), and Licorice Root (Gancao) each two qian, Coptis Rhizome (Huanglian) and Saposhnikovia Root (Fangfeng) each three qian, Trichosanthes Root (Tianhuafen) six qian, and 12 seeds of Ailanthus Seed (Dafengzi), crushed together with their skins. Given his surging pulse and mental agitation, I added one and a half liang of fine powder of Raw Gypsum (Sheng Shigao). The mixture was decocted into two teacups of liquid, to be taken warm in two divided doses. After taking three consecutive doses, the areas on the face exuding yellow fluid formed crusts, old crusts fell off, and both itching and agitation were reduced by more than half; the surging quality of the pulse also decreased by half. Thus, I removed the Gypsum and added Gentian Root (Longdancao) three qian. After one dose, the vague, almost imperceptible lesions all over his body emerged fully, accompanied by itching. Continuing with the original formula for several more doses resulted in complete recovery. Regarding the herbs in the formula, most can be modified according to the syndrome, used or omitted as needed, but Snake Slough is essential because it is excellent at resolving toxicity (using toxicity to combat toxicity) and dispelling wind, and possesses the subtle efficacy of "treating the skin with the skin." If one fears the toxicity of Ailanthus Seed and wishes to avoid it, this herb may be omitted.
Nocturnal emission (Meng Yi) is a condition that most readily causes weakness of the Kidney Meridian. If this disease is not eradicated, taking kidney-tonifying medicines daily will be of no benefit. Although astringent substances such as Dragon Bone (Longgu), Oyster Shell (Muli), Cornus Fruit (Yurou), and Cherokee Rose Fruit (Jinyingzi) often show efficacy when taken, there is ultimately no absolute guarantee of success. This is a disorder characterized by lightness and reckless movement of the brain/mind (Naojin). Only Western medicines such as Potassium Bromide (Choubao Shui), which are sedatives/anesthetics for the nervous system, can truly calm the mind when used in small amounts. If combined with Longgu and Muli, therapeutic effects can be achieved readily. I have a commonly used prescription, which I record below:
Dragon Bone (Longgu) one liang, Oyster Shell (Muli) one liang, Purified Cornus Fruit (Jing Yurou) two liang, all ground into fine powder. Add fourteen grains (wa) of Western medicine Potassium Bromide (Choubao). Form into 100 pills using refined honey. Take seven pills just before going to sleep. After two months of administration, the disease can be permanently cured.
