Treats the concurrent occurrence of Shui Gu (Water Drum, abdominal distension due to fluid accumulation) and Qi Gu (Qi Drum, abdominal distension due to gas accumulation), and also treats Dan Fu Zhang (abdominal distension alone without obvious edema or gas), Dan Shui Gu Zhang (abdominal distension due solely to fluid accumulation), and Dan Qi Gu Zhang (abdominal distension due solely to gas).
Sheng Ji Nei Jin (Crataegus Pirentis Endocarpium, five qian, cleaned of stones and dregs, ground into fine powder) Sheng Bai Zhu (Atractylodis Macrocephalae Rhizoma, dosage adjusted according to timing) Xian Mao Gen (Imperatae Rhizoma, two liang, cut into small pieces)
First, decoct the Mao Gen in several teacups of water (do not over-decoct; after one boil, simmer on low heat until the Mao Gen sinks to the bottom, then the decoction is ready). Take one and a half cups first, add five slices of fresh ginger, and decoct the Ji Nei Jin powder. When reduced to half a cup, add one cup of the Mao Gen decoction. After boiling seven or eight times, clarify and take the clear liquid (whether one cup or slightly more) for administration. The remaining dregs should still be decocted with Mao Gen decoction and taken. Administer one dose per day, once in the morning and once in the evening. Upon initial administration, urine output increases immediately; after a few days, bowel movements also increase. If bowel movements reach two or three times daily, reduce Ji Nei Jin by one qian and add Sheng Bai Zhu by one qian. After another few days, if abdominal distension begins to subside but bowel movements remain frequent, reduce Ji Nei Jin by one qian and add Bai Zhu by one qian. After further days, if distension is reduced by more than half but bowel movements remain frequent, reduce Ji Nei Jin by one qian and add Bai Zhu by one qian again. Through such careful adjustment based on the pathogenesis, ensuring that the tonifying and breaking forces are appropriate for the patient's condition, complete recovery will naturally occur. If fresh Mao Gen is unavailable, one can substitute it with one liang of dried Mao Gen from the pharmacy. Without fresh Mao Gen, fresh ginger is not needed. The prepared Mao Gen decoction should be consumed within the same day; any leftover decoction after preparing medicine can be drunk warm as tea.
The efficacy of Ji Nei Jin has been discussed in detail in previous sections. As for Mao Gen, its ability to promote diuresis is common knowledge. However, when used in this formula, it is not only for its diuretic effect. Mao Gen emerges earliest in spring, inheriting the qi of the initial generation of Yang, which rises upward. Therefore, for any condition where Qi is stagnant and unsmooth, Mao Gen can facilitate its flow. Being adept at promoting diuresis and regulating Qi, it assists Ji Nei Jin to achieve remarkable results. Fresh ginger is added out of concern that the nature of fresh Mao Gen is slightly cold. Moreover, its pungent flavor regulates Qi, and its skin promotes diuresis. Subsequently, Bai Zhu is added and Ji Nei Jin is reduced because, as distension subsides, one should support the Righteous Qi to overcome the Pathogenic Factor, avoiding the exclusive use of opening and breaking agents that might damage the Righteous Qi. Some may suspect that Bai Zhu should have been added in small amounts initially. However, I have tested this, and early addition of Bai Zhu is indeed less effective than late addition.
One might ask: Mao Gen’s ability to clear heat and promote urination is common knowledge. Yet, regarding its claim to regulate stagnation in the Qi aspect, none of the various Materia Medica mention it. Have you used it alone, and do you have concrete evidence? I reply: Such experimental cases are too numerous to recount. I once treated a young unmarried woman who constantly felt heat in her heart region, with no effect from repeated medication. Upon my examination, all six pulses were deep and thin. During pulse diagnosis, I heard her sigh several times, indicating discomfort in the Qi aspect. Asking her, she reported constant dull pain in the heart and hypochondriac regions. I then instructed her to obtain fresh Mao Gen, cut half a jin into small pieces, decoct it through a few boils, and drink it as tea. Two days later, upon re-examination, her pulse had returned to the floating layer and was forceful upon heavy pressure; she no longer sighed. She reported no pain in the hypochondriac region, only feeling heat in the heart region. After drinking it for a few more days, the heat in her heart also resolved.
I also once treated a young man suffering from Pneumonic Plague. His throat, lips, and tongue were abnormally dry. His spirit was lethargic, appearing sleepy. His body surface was not hot. His pulse was deep and minute. He reported frequent mental restlessness. This indicated that the plague pathogen had blocked the Shaoyin channel, preventing Kidney Qi from ascending. He had not had a bowel movement for four days. I prescribed a large dose of Bai Hu Jia Ren Shen Tang (White Tiger Decoction plus Ginseng). First, decoct several liang of Mao Gen in water, using this decoction instead of plain water to prepare the medicine. Obtain three cups of juice and divide it into three doses for drinking. His pulse instantly rose, transforming into a surging and slippery appearance. His spirit recovered, his body became warm, and all symptoms began to improve. Instruct him to continue decocting the original prescription. For each dose, mix in one raw egg yolk. The disease was completely cured. Presumably, because Mao Gen grows near water, it inherits the energy of Cold Water. Furthermore, when emerging from the ground, it forms a sharp conical shape, allowing it to directly enter the Shaoyin channel, assist Kidney Qi in ascending, and harmonize with the Heart. Thus, the Heart beats with strength, causing the pulse to manifest as surging and slippery externally. Adding raw egg yolk nourishes the fluids of Shaoyin, allowing them to ascend with the Qi to resolve the upper Jiao heat generated by dryness and the subsequent irritability caused by heat, thereby curing all diseases. These two cases sufficiently demonstrate the efficacy of Mao Gen in regulating Qi.
