Clause: In the condition of "Yanyue" (Crescent-shaped) internal cataract, the pupil exhibits a crescent-shaped opacity, with a bay of white qi arising from below. This is caused by brain wind and accumulated heat pouring downward into the eyes, as well as deficiency in both the Liver and Kidney leading to impaired vision.
For treatment, Fangfeng (Saposhnikovia Root) and Huangqin (Scutellaria Root) should be added to Tongming San; the formula also contains Renshen (Ginseng), Baifuling (Poria), Xixin (Asarum), and Chongweizi (Eclipta Fruit).
If using Zhiyi Wan (specifically Wudan Yanyue Zhuanyi Pill), Shijueming (Concha Arcae), Shexiang (Musk), Qingyudan (Green Fish Gallbladder), Liyudan (Carp Gallbladder), Qingyangdan (Blue Sheep Gallbladder), Niudan (Ox Gallbladder), and Xiongdan (Bear Gallbladder) are required.
Formula: Yanyue Tongming San Decoction
Fangfeng (Saposhnikovia Radix) 3g, Huangqin (Scutellariae Radix) 3g, Renshen (Ginseng Radix) 3g, Fuling (Poria) 3g, each 1 qian; Xixin (Asari Radix) 1.5g; Chongweizi (Ecliptae Fructus) 6g.
Grind the above herbs into coarse powder. Add two cups of water, decoct until one cup remains. Strain and take warm after night meal.
Wudan Yanyue Zhuanyi Pill Formula
Shijueming (Concha Arcae) 30g, Shexiang (Moschus) a small amount, Qingyudan (Bile of Cyprinus carpio var. auratus) seven units, Liyudan (Bile of Cyprinus carpio) seven units, Qingyangdan (Bile of Ovis aries) seven units, Niudan (Bile of Bos taurus) 15g, Xiongdan (Bile of Ursus arctos) 0.3g.
Grind the above herbs into fine powder and bind with wheat paste to make pills, sized like paulownia seeds. Take 1.5g with clear tea on an empty stomach.
[Note] [1] "Yanyue" internal cataract refers to a bay of white qi appearing in the upper half inside the pupil, faintly resembling the shape of a new moon, hanging downwards.
[2] This arises because wind and accumulated heat from the brain pour into the eyes, causing internal cataracts; it is a syndrome of exhaustion in both the Liver and Kidney. It is appropriate to administer Tongming San and Zhuanyi Wan.
Clause: The protrusion of "Xuanluo" (Spiral-shell) external opacity resembles a shell, with changes in the color of the black cornea and extreme pain. The shape is pointed and raised like a shell, with a bluish-black color, caused by accumulated heat in the Liver Meridian and coagulation of blood stasis.
For mild cases, Xie Nao Tang (Brain-Draining Decoction) is suitable, containing Fangfeng (Saposhnikovia Root), Xixin (Asarum), Jiegeng (Platycodon Root), Chishao (Red Peony Root), Tiandong (Asparagus Tuber), Wuweizi (Schisandra Berry), and Chongweizi (Eclipta Fruit); for severe cases, Xie Gan Yinzi (Liver-Draining Infusion) is used, containing Mangxiao (Natrii Sulfas), Dahuang (Rhubarb Root), Jiegeng (Platycodon Root), Chaihu (Bupleurum Root), Huangqin (Scutellaria Root), Zhimu (Anemarrhena Rhizome), Xixin (Asarum), and Cheqianzi (Plantago Seed).
Formula: Xie Nao Tang Decoction
Fangfeng (Saposhnikovia Radix) 6g, Xixin (Asari Radix) 1.5g, Jiegeng (Platycodonis Radix) 3g, Chishao (Paeoniae Radix Rubra) 3g, Tiandong (Asparagi Radix, heart removed) 3g, Wuweizi (Schisandrae Chinensis Fructus) 1.5g, Chongweizi (Ecliptae Fructus) 6g.
Grind the above herbs into coarse powder. Add two cups of water, decoct until one cup remains. Strain and take warm after meals.
Xie Gan Yinzi Infusion
Mangxiao (Natrii Sulfas), Dahuang (Rhei Radix et Rhizoma), Jiegeng (Platycodonis Radix), Chaihu (Bupleuri Radix), Huangqin (Scutellariae Radix), Zhimu (Anemarrhenae Rhizoma, stir-fried), Xixin (Asari Radix), Cheqianzi (Plantaginis Semen), each 3g.
Grind the above herbs into coarse powder. Add two cups of water, decoct until one cup remains. Strain and take warm after meals.
[Note] [1] "Xuanluo" external opacity refers to the black cornea within the Qi Wheel (white sclera) changing color to blue-white, resembling a snail shell. Its color is initially blue and turns black over time, and its shape is pointed and round. This is caused by extreme hyperactivity of accumulated heat in the Liver Meridian and stagnation of blood stasis.
[2] Mild cases are suitable for Xie Nao Tang, while severe cases use Xie Gan Yinzi.
