Treats conditions of Mingmen Fire Decline (deficiency of Kidney Yang failing to warm the Spleen Earth), resulting in Spleen-Stomach Deficiency-Cold, reduced appetite, and loose stools; or exhaustion of the Lower Yuan (extreme weakness of Kidney Qi), manifesting as pain around the umbilicus and abdomen, and frequent nocturia.
Shu Di Huang (Rehmanniae Radix Praeparata, steamed nine times until standard, mashed into paste) 8 liang, Gan Shan Yao (Dioscoreae Rhizoma) 4 liang, Shan Yu Rou (Corni Fructus) 4 liang, Bai Fu Ling (Poria) 3 liang, Dan Pi (Moutan Cortex) 3 liang, Ze Xie (Alismatis Rhizoma) 3 liang, Rou Gui (Cinnamomi Cortex) 1 liang, Fu Zi (Aconiti Lateralis Radix Praeparata) 1 liang. The above eight herbs are ground into powder, mixed with refined honey to form pills the size of tung tree seeds. Take 15 pills per dose, administered with wine, twice daily.
[Collected Annotations] Zhao Xianke stated: By observing the image of Kan (☵), one understands that the Kidney contains both Water and Fire functions. In contemporary cases, if yang rises easily after intercourse, it indicates Yin Deficiency with Fire Hyperactivity; if impotence occurs before intercourse, it indicates Mingmen Fire Decline. When True Water is exhausted, there is no cold even in mid-winter; when True Fire is extinguished, there is no heat even in mid-summer. In this formula, Shu Di Huang, Shan Yao, Ze Xie, Dan Pi, Fu Ling, and Shan Yu Rou are moistening and nourishing substances, used to strengthen the "Master of Water" (Kidney Yin); Rou Gui and Fu Zi are pungent and moistening substances, capable of supplementing Fire within Water, thus benefiting the "Origin of Fire" (Kidney Yang). When Water and Fire are nourished, Kidney Qi recovers.
Yu Chang stated: *Jin Gui Yao Lue* uses Ba Wei Wan to treat Jiao Qi (Gouty Arthritis/Paralysis) where pathogenic qi ascends into the lower abdomen causing numbness. Jiao Qi refers to Yin pathogenic qi; numbness in the lower abdomen is a precursor to the pathogen attacking the Heart, hence this formula is used to expel Yin pathogens. Its ability to treat deficiency fatigue with lumbar pain, lower abdominal tension, and dysuria arises because overexertion damages the Kidneys, causing Yin qi to rebel in the lower abdomen, obstructing the Qi transformation of the Bladder, leading to difficult urination. Thus, it is used to warm and nourish the Lower Jiao to consolidate Kidney Qi. For patients with shortness of breath accompanied by slight fluid retention, since fluids belong to the Yin category and obstruct Yang Qi in the chest, causing shortness of breath, this formula is used to guide fluids downward for excretion, thereby calming the Chest Middle. In Xiao Ke (Wasting-Thirst) disease, where drinking one dou results in urinating one dou, the Kidney Qi fails to control water, leading to excessive urination and a risk of source depletion; thus, urgent use of this formula is required to counteract this water flow. For Xiao patterns caused by Kidney Water sinking downward, and Ke patterns caused by failure of Kidney Qi to ascend, without using this method to store, protect, seal, and preserve, while steaming and activating Water Qi, from where else could treatment be sought! Later generations calling Ba Wei Wan the sacred medicine for treating Xiao Ke have grasped its essence.
Ke Qin stated: The Fire of Mingmen is the Yang within Water. The nature of Water is originally still, but its continuous flow like a river is due to the movement of Qi and the function of Fire, not referring to有形 (material forms). However, little Fire generates Qi, while excessive Fire consumes Qi; therefore, Fire must not be hyperactive nor deficient. What is meant by "Fire generates Earth" is the Little Fire (Physiological Fire) of the Kidney circulating within, blowing like breath. If Mingmen Fire declines, Little Fire is nearly extinguished. To warm the Yang of the Spleen and Stomach, one must first warm the Fire of Mingmen. This Shen Qi Pill incorporates Rou Gui and Fu Zi into Yin-nourishing agents at a ratio of one-tenth; the intent is not to strongly tonify Fire, but to gently generate Fire, which is essentially generating Kidney Qi. Hence, it is named Shen Qi (Kidney Qi) rather than Wen Shen (Warming Kidney), indicating that the Kidney prioritizes Qi; once the Kidney obtains Qi, Earth naturally generates. Furthermore, for those with insufficient form, warming with Qi cures diseases caused by Spleen-Stomach Deficiency-Cold; even for those with defunct Fire not returning to its origin, it guides them back to the root of storage and preservation. Cui Shi's Modified Ba Wei Wan replaces the pungent-hot Fu Zi with the sour-astringent Wu Wei Zi, suitable for Kidney Deficiency without Cold. *Qian Jin Fang* adds Xuan Shen (Scrophulariae Radix) salty-cold to assist Shu Di Huang in nourishing Kidney Yin, and Shao Yao (Paeoniae Radix Alba) sour-cold to assist Dan Pi in nourishing Liver Yin. Essentially, these act as locks and keys for Gui Zhi and Fu Zi. Using them to strengthen Water is excessive; using them to benefit Fire might be insufficient. *Ji Sheng Fang* adds Niu Xi (Achyranthis Bidentatae Radix) and Che Qian Zi (Plantaginis Semen) to treat Edema, doubles the dosage of Fu Ling to assist Di Huang, Shan Yao, and Zhu Yu, placing them on equal footing with Ze Xie, Dan Pi, Che Qian, and Niu Xi, adjusting according to symptoms, which is indeed appropriate. Yi Yin Shen Qi Pill adds Dang Gui (Angelicae Sinensis Radix), Wu Wei Zi, and Chai Hu (Bupleuri Radix) to Liu Wei Di Huang Wan to treat dim vision and blindness, making further refinements that are exquisite.
