Zhiji Erchen Tang (Bitter Orange, Platycodon and Two-Chen Decoction) & Jiegeng Tang (Platycodon Decoction)
15. Cough occurring during pregnancy is termed "Zisu" (Fetal Cough). If caused by Yin deficiency leading to phlegm-fluid retention combined with exogenous wind-cold pathogen, Zhike (Aurantii Fructus) and Jiegeng (Platocodonis Radix) should be added to Erchen Tang (Two-Chen Decoction), a basic formula for drying dampness and resolving phlegm; if caused by exogenous wind-cold, Jiegeng Tang can bring relief. For cases involving Zisu (Perillae Folium), Jiegeng (Platocodonis Radix), Mahuang (Ephedrae Herba), Sangbaipi (Mori Cortex), Xingren (Armeniacae Semen Amarum), Chifuling (Poria cum Rhizoma Polypori), Tiandong (Asparagi Radix), Baihe (Lilii Bulbus), Chuanbeimu (Fritillariae Cirrhosae Bulbus), and Qianhu (Peucedani Radix), which are suitable for chronic cough due to Yin deficiency, a moistening and clearing method is appropriate. Taking Maiwei Dihuang Tang (Ophiopogon and Lycium Pill of Six Ingredients modified) will lead to recovery.
[Note] Cough during pregnancy is called "Zisu". Prolonged coughing often injures the fetus. Etiologies differ between Yin deficiency with fire hyperactivity and upward reversal of phlegm-fluid, and common cold due to wind-cold. When caused by phlegm-fluid, treat with Erchen Tang plus Zhike and Jiegeng; when caused by wind-cold, use Jiegeng Tang, composed of Zisu Ye (Perillae Folium), Jiegeng (Platocodonis Radix), Mahuang (Ephedrae Herba), Sangbaipi (Mori Cortex), Xingren (Armeniacae Semen Amarum), Chifuling (Poria cum Rhizoma Polypori), Tiandong (Asparagi Radix), Baihe (Lilii Bulbus), Chuanbeimu (Fritillariae Cirrhosae Bulbus), and Qianhu (Peucedani Radix). If cough persists for a long time and belongs to Yin deficiency, nourish Yin and moisten the Lung for clearing and moistening treatment using Liuwei Dihuang Tang (Six-Ingredient Pill with Rehmannia), a basic formula for tonifying Kidney Yin.
The composition of Liuwei Dihuang Tang is found in the first volume of this book.
Siwu Tang (Four-Substance Decoction), Ejiao (Donkey-Hide Gelatin), & Wuling San (Five-Ingredient Powder)
16. When diet remains normal but there is irritability and inability to sleep, accompanied by urinary retention, this condition is termed "Zhuanbao" (Displacement of the Bladder/Uterine Pressure causing Urinary Retention). To save the emergency, apply Danxi's method of lifting the fetus: administer Siwu Tang (Four-Substance Decoction) augmented with Shengma (Cimicifugae Rhizoma), Renshen (Ginseng Radix et Rhizoma), Baizhu (Atractylodis Macrocephalae Rhizoma), and Chenpi (Citri Reticulatae Pericarpium). After ingestion, induce vomiting by probing the throat with a finger; after vomiting, take the medicine again and induce vomiting again. Repeat this three or four times, then the fetus will be lifted and urination will become smooth. If ineffective, it indicates fluid retention; use Wuling San (Five-Ingredient Powder) with added Ejiao to clear and promote diuresis.
[Note] During pregnancy, fetal pressure causes twisting of the bladder ligaments, resulting in urinary retention while diet remains normal, accompanied by irritability and insomnia. This is named "Zhuanbao". It is suitable to use Danxi's method of lifting the fetus: have the midwife apply sesame oil to her hands and manually lift the fetus up, so that urine will flow out naturally, providing temporary relief for the emergency. Then, decoct and administer Siwu Tang added with Shengma, Renshen, Baizhu, and Chenpi. After taking the medicine, probe the throat with a finger to induce vomiting; after vomiting, take the medicine again and induce vomiting again. Repeat this three to four times, the fetus is lifted, and urination becomes unobstructed. If this does not work, it implies the presence of fluid retention; use Wuling San with Ejiao to clear heat and promote diuresis.
The composition of Siwu Tang is found in the first volume of this book.
