Cold qi entering the bladder forms cold stranguria (Han Lin), characterized by obstruction of urination, distension and unbearable discomfort, continuous dribbling of urine, and dull abdominal pain. It can be treated with Wuling San (Five-Ling Decoction) with increased dosage of Rougui (Cinnamon Twig) and Xiaohuixiang (Fennel).
[Note]
Cold stranguria is caused by wind-cold pathogens invading the bladder, leading to cold accumulation in the lower jiao.
Its manifestations include urinary obstruction, unbearable distending pain, intermittent dribbling, and dull pain in the lower abdomen.
It must be treated with Wuling San (Five-Ling Decoction) with added Rougui (Cinnamon Twig) and Xiaohuixiang (Fennel); the stranguria will heal spontaneously.
(Wuling San [Five-Ling Decoction]) Baizhu (Stir-baked with soil) (Atractylodis Macrocephalae Rhizoma), Zexie (Alismatis Rhizoma), Zhuling (Polyporus), Rougui (Cinnamomi Cortex), Xiaohuixiang (Foeniculi Fructus), Chifuling (Poria cum Roselle), decocted in water and taken orally.
(Verse for Prescription) Wuling San treats cold stranguria; it contains Baizhu, Zexie, and Zhuling; use double the amount of Rougui, along with Xiaohuixiang and Chifuling.

Heat accumulation in the bladder forms heat stranguria (Re Lin); Shiwai Daochi Tang (Ten-Ingredient Guide-to-the-Heart Decoction) yields remarkable results. If there is lower abdominal fullness and constipation, Baj Zheng San (Eight-Right Decoction) should be taken urgently without delay.
[Note]
Heat stranguria is formed by heat accumulation in the bladder, presenting as difficult urination, dribbling, and painful dysuria. It is primarily treated with Shiwai Daochi Tang (Ten-Ingredient Guide-to-the-Heart Decoction).
If accompanied by lower abdominal fullness, pain radiating to the umbilicus, and dry stool constipation, it is primarily treated with Baj Zheng San (Eight-Right Decoction).
(Shiwai Daochi Tang [Ten-Ingredient Guide-to-the-Heart Decoction]) Dihuang (Rehmanniae Radix), Shanzhizi (Gardeniae Fructus), Mutong (Akebiae Caulis), Qumai (Dianthi Herba), Huashi (Talcum), Danzhuye (Lophatheri Folium), Yinchenshao (Artemisiae Scopariae Herba), Huangqin (Scutellariae Radix), Gancao (Licorizae Radix et Rhizoma, raw), Zhuling (Polyporus), decocted in water and taken orally.
(Verse for Prescription)
The ten-ingredient Daochi medicine is most effective; Dihuang, Shanzhizi, and Mutong are combined; Qumai, Huashi, and Danzhuye are added; Yinchenshao, Huangqin, Gancao, and Zhuling follow.
(Baj Zheng San [Eight-Right Decoction]) See the section on "Urinary Retention" for the formula.

Stranguria Section
Long-term accumulation of damp-heat forms stone stranguria (Shi Lin), where urine passes like sand and stones causing penile pain. For mild cases, Kuizi San (Hibiscus Seed Powder) is required; for severe cases, Baj Zheng San (Eight-Right Decoction) can be used subsequently.
[Note]
Stone stranguria involves pain in the urethra during urination, often passing sand-like or stone-like particles. It is caused by long-term heat accumulation in the bladder,
similar to how a kettle develops white scale at the bottom after prolonged heating over fire. Mild cases are treated with Zhu Kuizi San (Boiled Hibiscus Seed Powder), while severe cases are treated with Baj Zheng San (Eight-Right Decoction).
(Kuizi San [Hibiscus Seed Powder]) Sangpi (Mori Cortex, stir-fried), Qumai (Dianthi Herba), Zhizi (Gardeniae Fructus), Chifuling (Poria cum Roselle), Mutong (Akebiae Caulis), Cheqianzi (Plantaginis Semen), Gancao (Licorizae Radix et Rhizoma, raw), Kuizi (Abutilonis Semen), decocted in water and taken orally.
(Verse for Prescription)
Kuizi San treats stone stranguria; it consists of Sangpi, Qumai, and Shanzhiren (Gardeniae Semen); Chiling, Mutong, and Cheqianzi are added; Gancao and Kuizi are mixed evenly.
(Baj Zheng San [Eight-Right Decoction]) See the section on "Urinary Retention" for the formula.
