For postpartum abdominal pain, Danggui Shengyang Roushou Tang (Dongqu, Ginger, and Mutton Decoction) is the primary treatment; it also treats abdominal cold colic and deficiency-fatigue insufficiency.
[Annotation]
Sudden acute abdominal pain after childbirth is caused by postpartum deficiency-cold. Danggui Shengyang Roushou Tang (Dongqu, Ginger, and Mutton Decoction) is used to tonify deficiency, dispel cold, and relieve pain. It also treats deficiency-fatigue insufficiency and cold colic abdominal pain because these conditions are also due to deficiency and cold.
[Collected Annotations]
Cheng Lin stated: Postpartum blood deficiency with cold leads to acute abdominal pain. The *Neijing* states: "Substances with thick flavors belong to Yin." Dongqu (*Radix Angelicae Sinensis*) and mutton are substances with thick flavors; they are used to tonify the Yin of the postpartum state. Ginger (*Rhizoma Zingiberis Recens*) is added to dispel cold in the abdomen, so the pain stops naturally. Pungent flavors can dispel cold, and tonification can remove weakness; these three ingredients form a pungent-warm tonic formula, thus it also treats deficiency-fatigue and cold colic.
Wei Litong stated: Abdominal pain during pregnancy is due to uterine obstruction and blood-cold. Abdominal pain after childbirth is due to interior deficiency and blood-cold. One is obstruction, the other is deficiency; therefore, the treatment methods differ.

For postpartum abdominal pain accompanied by vexation, fullness, and inability to lie down, Zhishi Shaoyao San (Immature Bitter Orange and White Peony Powder) is the primary treatment.
[Annotation]
Postpartum abdominal pain without vexation or fullness indicates interior deficiency; current symptoms of abdominal pain with vexation, fullness, and inability to lie down indicate interior excess. Pain caused by Qi stagnation and Blood coagulation requires Zhishi (*Fructus Aurantii Immaturus*) to break Qi stagnation and Shaoyao (*Radix Paeoniae Alba*) to regulate abdominal pain. Zhishi is stir-fried until black because the Qi of postpartum women is not solid. It also treats carbuncles and pus, which arise from Blood being coagulated by Qi and decomposing over time. Wheat gruel (*Mai Zhou*) is used as a vehicle to prevent damage to the postpartum woman's Stomach Qi.
[Collected Annotations]
You Yi stated: When postpartum abdominal pain progresses to vexation, fullness, and inability to lie down, it indicates that Blood stagnation has transformed into Heat, and pathology below is affecting the upper body; this differs from deficiency-cold abdominal pain.
Zhishi (*Fructus Aurantii Immaturus*) (burned until black, but not excessively) and Shaoyao (*Radix Paeoniae Alba*) in equal parts.
These two ingredients are pounded into a powder. Take one *fangcun bi* (a measure approx. 2-3 grams) per dose, taken three times daily. It also treats carbuncles and pus, administered with wheat gruel (*Mai Zhou*).

The Master said: For postpartum abdominal pain, the method dictates using Zhishi Shaoyao San (Immature Bitter Orange and White Peony Powder). If it does not heal, it is because there is dried Blood adhering below the umbilicus; Xiayuxue Tang (Downward Draining Blood Decoction) should be used. It also treats irregular menstruation.

Postpartum abdominal pain due to Qi stagnation and Blood coagulation is regulated by Zhishi Shaoyao San (Immature Bitter Orange and White Peony Powder). If it does not heal after administration, it is because Heat scorching has dried the Blood, causing it to adhere below the umbilicus and cause pain; this cannot be treated by Zhishi (*Fructus Aurantii Immaturus*) and Shaoyao (*Radix Paeoniae Alba*). It is appropriate to drain Blood stasis, primarily using Xiayuxue Tang (Downward Draining Blood Decoction) to purge Heat and drain Blood stasis. It also treats amenorrhea, which is also due to Heat scorching and drying of the Blood.
