Classification: General Discussion on Manipulation Techniques
Attributes: The term "manipulation technique" refers to the use of both hands to position injured bones and tendons, restoring them to their original state.
[1] However, injuries vary in severity, and manipulation techniques have specific indications. The speed of recovery and whether disabilities remain depend entirely on whether the manipulation is applied appropriately; if it is misapplied or fails to fully adhere to the principles, outcomes may suffer.
[2] Since the skeletal structure of the entire body is not uniform, and the arrangement and affiliation of the Twelve Sinew Channels (Twelve Jing-Jin) differ from one another, one must be familiar with the physical constitution and recognize anatomical locations beforehand. Upon clinical examination, when external contact triggers an understanding of the pathology, skill arises internally. The hand moves according to the mind, and methods are executed through the hand. Some techniques involve traction to separate and then recombine parts; others involve pushing to bring fragments together for reduction; some correct deformities, while others fill defects. Whether bones are fractured transversely, comminuted, or obliquely, or whether tendons are lax, contracted, twisted, or displaced—even if these conditions lie deep within the muscles—palpation allows the practitioner to fully comprehend the situation. When the application of method alleviates pain to the point that the patient is unaware of suffering, it can truly be called a manipulation technique.
[3] Moreover, injured areas often relate to life itself. For instance, the seven orifices connect upward to the brain marrow; the diaphragm is near the Heart (the Sovereign Mind); injuries to the four extremities cause pain that penetrates the heart. Even if a patient’s Yuan Qi (Original Qi) is naturally robust, allowing blood stasis to dissipate and recovery to occur within a set period, manipulation should still not be applied recklessly. If the patient’s Yuan Qi is inherently weak, an injury may already be beyond support; if manipulation is further mishandled, recovery becomes nearly impossible. This is why extreme caution is required.
[4] Therefore, practitioners of bone-setting must possess mental clarity and manual dexterity. Having understood the condition and mastered the appropriate manipulation techniques, treatment will naturally yield frequent success. Indeed, the hand is composed of flesh and blood; its subtle capacity for flexible movement—through curling and extending, adjusting height and depth, controlling speed, varying pressure, and opening and closing—can address the patient’s Qi and Blood stagnation, swelling and pain in skin and flesh, contracture and fracture of bones and tendons, as well as emotional distress and desires. Compared to using rigid instruments that constrain the body, this approach is vastly superior. Thus, manipulation techniques are indeed the primary priority in bone-setting.
