Alum (Mingfan, 明矾) half jin, boiled in one and a half dou of water until dissolved, used to soak the feet until the water covers the ankles.
[Note]
Patients with cold limbs (Jue, 厥) and high fever belong to Yang Jue Fu disease (Yang Jue Fu Bing, 阳厥府病). Soaking the feet in alum water externally is because Jue originates from the lower body; this method helps to consolidate and grasp the Yang Qi (Yang Qi, 阳气).
[Collected Notes]
Cheng Lin said: When rebellious Yang (Jue Yang, 厥阳) moves alone upwards, sudden death occurs accompanied by high fever. Qibo said: When Blood (Xue, 血) and Qi (Qi, 气) rush together upwards, it results in Great Jue (Da Jue, 大厥), leading to sudden death. Alum is an astringent and consolidating herb; soaking the feet in it can restrain the rebellious Qi.
Adopt the posture/action "Riding a Cow Facing the Face" (Qiniu Linmian, 骑牛临面), crush Scallion White (Xie, 薤) to extract juice and instill into the ears, blow Soap Pod powder (Zaogao, 皂角) into the nose; immediate effect is expected.
Moxibustion on the four corners behind the fingernails and toenails, fourteen moxa cones per point, followed by administration of Wudu Zhugaosan (Five Poisons Various Ointment Powders, 五毒诸膏散) (which contains Croton Seed, Badou, 巴豆).
[Note]
For sudden death with open mouth and opisthotonos (Fanzhe, 反折), use moxibustion to unblock Yang Qi, but must also administer Wudu Zhugao Fang (Five Poisons Ointment Formula, 五毒诸膏方).
One sheng of horse manure, boiled in three dou of water to obtain two dou of liquid, used to wash the patient's body; take one sheng of cow dung (thin feces), mix with warm wine, and pour into the mouth; moxibustion at one cun below the heart, three cun above the navel, and four cun below the navel, one hundred cones per point, will cure the condition.
[Note]
Shi Jue (Corpse Jue, 尸厥) presents with closed eyes, open mouth with loss of bowel control, opisthotonos, and flaccid limbs that cannot be moved; this belongs to Yin Jue Zang disease (Yin Jue Zang Bing, 阴厥藏病). For such symptoms, the "Riding a Cow Facing the Face" method is used to suppress evil pathogens. Instilling Scallion White juice into the ears and blowing Soap Pod powder into the nose opens all orifices, allowing blocked passages to become patent. Moxibustion behind the nails unblocks external Yang Qi. Horse manure induces vomiting; moxibustion at Zhongwan (CV12) and Guanyuan (CV4) unblocks internal Yang Qi.
One pill of dog feces, crushed to extract juice, and administered orally. If fresh feces are unavailable, boil dry feces in water to extract the juice.
[Formula Analysis]
All feces have the property of dispersing Yang Qi; using dog feces also aims to disperse Yang Qi.
Ephedrae Herba (Mahuang, 麻黄) [devoid of nodes] 3 liang, Semen Armeniacae Amarum (Xingren, 杏仁) [devoid of skin and tips] 70 pieces, Glycyrrhizae Radix et Rhizoma (Gancao, 甘草) [roasted] 1 liang.
These three ingredients are boiled in eight sheng of water to obtain three sheng of decoction. Strain the dregs, and allow the patient to swallow the liquid in divided doses. This formula treats various cases of Ke Wu (Guest Malevolence, 客忤).
[Note]
In cases of Zhong E (Poisoning by Evil Qi, 中恶) and Ke Wu causing constipation and interior excess, Zhang Zhongjing uses Beiji Wan (Emergency Rescue Pill, 备急丸). It follows that for exterior excess without sweating, Beiji Wan should not be used to purge the interior; instead, Huanhun Tang (Return Soul Decoction, 还魂汤) should be used to unblock the exterior Yang. Purging the interior suppresses Yin Qi; unblocking the exterior supports Yang Qi. Ignorant practitioners mistakenly believe Mahuang only induces sweating in the Taiyang channel; they fail to realize that if applied without warming coverings to induce sweat, it enters the Taiyin channel to unblock Yang. Once Yang Qi flows freely, the Hun (Soul) returns.
Another Formula:
Leek Roots (Jiu Gen, 韭根) one handful, Chinese Plum Fruit (Wumei, 乌梅) 27 pieces, Evodiae Fructus (Wuzhuyu, 吴茱萸) [stir-fried] half jin.
Boil these three ingredients in one dou of water. Place the patient's comb into the boiling liquid for three boils. If the comb floats, the patient will survive; if it sinks, the patient will die. Boil down to three sheng, strain, and drink in divided doses.
[Formula Analysis]
The treatment using Leek Roots, Evodiae Fructus, and Chinese Plum Fruit is also a method to gather and consolidate Yang Qi. Floating represents Yang, sinking represents Yin; this illustrates the principle that when Yang generates, Yin dies.
To rescue someone who has died from hanging: If the time elapsed is from morning to evening, even if the body is cold, treatment is definitely effective; if from evening to morning, although the duration is short, it is difficult to treat. This is likely due to the abundance or deficiency of Qi at those times. However, in summer, nights are shorter than days and it is hot, so treatment should still be effective. It is also stated that if the epigastrium remains slightly warm, treatment is possible even after more than a day.
Slowly untie the person; do not cut the rope. Lay them flat on bedding placed above and below. One person stands on their shoulders, gently pulling their hair tightly without letting go. Another person presses and kneads the chest, moving it repeatedly. A third person massages the arms and legs, flexing and extending them. If the body is stiff, gradually force flexion and extension while pressing the abdomen. After about the time it takes to cook a meal, breath may return from the mouth, respiration resumes, and eyes open. Continue traction and pressure without releasing, but avoid exhausting the patient. Shortly after, give a small amount of Guizhi Tang (Cinnamon Twig Decoction, 桂枝汤) or thin rice gruel to moisten the throat, allowing gradual swallowing. Once stabilized, have two people use tubes to blow air into both ears. This method is most effective and rarely fails.
[Note]
From morning to evening, Yang Qi is abundant; since Yang governs life, treatment is effective even if the body is cold. From evening to morning, Yin Qi is abundant; since Yin governs death, treatment is difficult. Cases of hanging that are curable often involve prior anger or conflict where Qi was vigorous and not dispersed. Although evening-to-morning cases are difficult, summer conditions (short nights, heat) imply abundant Yang, making treatment viable. The statement about slight warmth in the epigastrium indicates that even after a day, treatment is possible. Zhang Zhongjing’s earnest warnings reflect his benevolent heart, fearing people might abandon treatment due to its complexity. Clinical trials show an 80-90% success rate, proving these words true. "Xianxian" means tightly. Kneading the chest, pressing the abdomen, and massaging/extending limbs are methods to guide Qi.
For sudden death from Heatstroke (Yi, 暍): Do not expose the patient to cold; exposure to cold causes death. Treatment method:
Wrap a grass belt around the umbilicus of the heatstroke victim. Have two or three persons urinate into the center to generate warmth. Alternatively, mix hot mud with grass strips, or place heated tile bottoms or chariot wheels against the patient. Urine must flow away. This method is for situations where no hot water is available on the road. To generate warmth, multiple persons may urinate. If hot water is available, use it instead of mud or chariot wheels, as these objects are cold. In summer, heating mud or chariot wheels makes them usable.
[Note]
Patients dying suddenly from heatstroke should not be placed in cool places, as this traps heat inside. Similarly, frostbite victims should not be poured with hot water, as this traps cold inside, causing greater harm.
