[Formula Song] Stagnant Heat Dan Du (Erysipelas caused by stagnant heat) presents with redness resembling cinnabar and a wandering shape; it is mostly caused by excessive milk or food intake in infants, leading to food stagnation and the generation of internal heat. Compared to Chi You Dan (Wandering Red Erysipelas), its progression is slower. Treatment should first focus on resolving food stagnation, followed by clearing heat.
[Note] At the onset, this condition resembles Chi You Dan in appearance but spreads more slowly than Chi You Dan. This is due to excessive milk and food intake in infants, which the spleen and stomach cannot transform, causing heat to accumulate internally and manifest on the skin surface. Symptoms include fever, flushed face, sour breath, and yellow tongue coating. It is appropriate to administer Bao He Wan (Preserve Harmony Pill) to first resolve food stagnation. If symptoms include cracked lips and constipation, Yi Nian Jin (One Pinch of Gold) is appropriate to promote bowel movement. If the erysipelas persists or recurs, Xi Jiao San (Rhinoceros Horn Powder) is appropriate. Other treatment methods follow those for Chi You Dan.
Formula: Bao He Wan (Preserve Harmony Pill)
Composition: Bai Fu Ling (Poria cocos), Zhi Ban Xia (Prepared Pinellia ternata tuber), Shan Zha Rou (Crataegus pinnatifida fruit flesh), Chao Shen Qu (Stir-fried Medicated Leaven) (each 1 liang), Chen Pi (Citrus reticulata peel), Chao Luo Bo Zi (Stir-fried Raphanus sativus seed), Lian Qiao (Forsythia suspensa fruit, heart removed).
Grind the above ingredients into fine powder, make pills using rice gruel paste, sized like paulownia seeds. Take 30 pills each time, swallowed or dissolved in warm boiled water.
[Formula Song] Bao He Wan contains Poria and Pinellia, along with Citrus peel, Raphanus seed, Crataegus, Medicated Leaven, and Forsythia; taken as water-habituated pills, it effectively resolves milk and food accumulation.
Another Formula: Yi Nian Jin (One Pinch of Gold)
Composition: Ren Shen (Panax ginseng root), Da Huang (Rheum palmatum rhizome), Hei Chou (Black Semen Convolvuli), Bai Chou (White Semen Convolvuli), Bing Lang (Areca catechu seed) (equal parts).
Grind into fine powder. Take one "zi" (approx. 0.5-1g) each time, mixed with honey water.
[Formula Song] Yi Nian Jin treats food stagnation transforming into fire, indicated for cracked lips and constipation, promoting purgation. It contains Da Huang, Black and White Chou, Ren Shen; Bing Lang is powdered and must be added with honey.
Another Formula: Xi Jiao San (Rhinoceros Horn Powder)
Composition: Xi Jiao Xie (Rhinoceros horn shavings - *Note: Substitute with Water Buffalo Horn in modern practice*), Sheng Ma (Cimicifuga foetida rhizome), Fang Ji (Stephania tetrandra root), Shan Zhi (Gardenia jasminoides fruit), Pu Xiao (Mirabilite), Huang Qin (Scutellaria baicalensis root), Huang Qi (Astragalus membranaceus root) (each 1 qian), Niu Huang (Calculus Bovis) (5 fen).
Grind into fine powder. Take 5 fen each time, mixed with decoction of Zhu Ye (Bambusa leaves); adjust dosage according to the infant's condition.
[Formula Song] Xi Jiao San treats erysipelas with redness, combined with Cimicifuga, Stephania, and Gardenia; Mirabilite, Scutellaria, Astragalus, and Calculus Bovis are powdered and taken with Bamboo leaf decoction at any time.
[Formula Song] Infant sores and ulcers are often formed by "milk fire"; thick flavors cause fire stagnation, compounded by exposure to the Six Excesses (pathogenic factors). Treatment for swelling and ulceration follows the protocols for carbuncles and sores.
[Note] Any infantile sores or small boils are mostly caused by "fire from seven emotions" in the nursing mother, or by infants over one year old consuming excessive dry, fried, or rich foods, generating stagnant fire, further compounded by exposure to the Six Excesses (Wind, Cold, Summerheat, Dampness, Dryness, Fire). However, methods such as releasing the exterior, purging the interior, supporting the interior to expel pathogens, and detoxifying, as well as external treatments for the swelling and ulceration stages, all follow the standards set forth in the Carbuncles and Sores chapters. Infants possess a "pure yang" constitution, making them prone to fire syndromes; thus, potent agents are often required to overcome the disease. However, their muscles and viscera are delicate and fragile; treatment should cease immediately upon efficacy, avoiding overdose.
