[Verse] Dingchuang (Ding sores on smallpox) differs from ordinary smallpox; commonly called "Thief Smallpox," its characteristics are purple-dark or black color and hardness like stone. Various symptoms arise simultaneously, making it difficult to suppurate. There are many types of Ding sores that must be memorized, and one must carefully examine where they manifest. Juanli Ding (Rolling Curtain Ding) arises at the base of the tongue, varying in size and easily identified. Huozhu Ding (Fire Pearl Ding) arises inside the nostrils, causing blockage and a sensation of fire spitting out, with a red face. Wangji Ding (Forget-Thirst Ding) arises on the eyelids, swelling like a toad with increased irritability and heat. Huhan Ding (Taming Tiger Ding) is seen in the ears, formed by Kidney toxin attacking the ear. Yanyao Ding (Swallow's Nest Ding) arises in both axillae, accompanied by facial redness, delirium, and worsened swelling and pain. Zhuming Ding (Life-Directing Ding) arises in the soles of the feet, with purple veins extending straight into the thighs. Touchang Ding (Penetrating Intestine Ding) erupts inside the anus, with pain similar to needle pricking. Qilong Ding (Black Dragon Ding) arises inside the urethral meatus, accompanied by fever, delirium, and urinary/fecal obstruction. Treatment should follow the specific type of Ding, naturally resolving all symptoms.
[Commentary] This condition has many names and treatments vary. Five to six days after the eruption of smallpox, three to five or six to seven lesions may appear mixed among other pustules. Their color is purple-dark, even black and hard like stone, leading to simultaneous onset of various symptoms and failure to suppurate. If Juanli Ding arises at the base of the tongue, small as a black bean or large as a grape, causing the child's tongue to curl and throat pain, urgently use a silver hook to puncture it, drain all bad blood, rinse with bitter tea, apply Bading San (Ding-Extracting Powder), and then blow a fine powder made of Bingpian (Borneol), Pengsha (Borax), Qingdai (Indigo Naturalis), Huanglian (Coptidis Rhizoma), Bohe (Menthae Haplocalycis Herba), Jingjie (Schizonepetae Spica), and Chao Jiangcan (Stewed Silkworm) into the lesion. Huozhu Ding arises inside the nostrils, causing blockage and a fiery sensation, with red face and eyes; also puncture with a silver hook, apply Huanglian Gao (Coptis Ointment) with Bingpian (Borneol) dropped into the nose, and internally administer Xiejing San (Metal-Draining Powder). Wangji Ding arises on the eyelids, swelling like a toad, with irritability, heat, and purple discoloration; puncture and apply juice from chewed Erythrina seeds (Yanzhi), and wash with a decoction of Gongying (Taraxaci Herba) and Juhua (Chrysanthemi Flos). Huhan Ding arises in the ears; when pus forms, puncture and apply Bading San (Ding-Extracting Powder). Yanyao Ding arises in the axillae, with swelling, hardness, facial redness, and delirium; if the Ding in the left axilla spreads subtly, the smallpox on the right side will sink and lose color, and vice versa; puncture to remove the root, apply Bading San (Ding-Extracting Powder), and take Xiaodu Yinzi (Detoxification Decoction). Zhuming Ding arises in both soles, swelling and hardening like coins, beans, or peppercorns, with purple veins extending to the thighs; puncture to drain bad blood, apply Tianluo Shui (Snail Water), then apply a paste of Shenhuo Cao (Herba Siegesbeckiae) and soaked, mashed Lüdou (Vigna radiata). Touchang Ding arises beside the anus, swelling and hardening like a cone around the sixth or seventh day; puncture, wash with a decoction of Jinyinhua (Lonicerae Japonicae Flos) and Fangfeng (Saposhnikoviae Radix), then apply a powder of Qingfen (Calomel), Zhenzhu (Margarita), Bingpian (Borneol), and Bailian (Belamcandae Rhizoma), and internally administer Huanglian Jiedu Tang (Coptis Decoction for Resolving Toxins). Qilong Ding arises inside the urethral meatus; around the fifth or sixth day, with fever, delirium, eye rolling, cold limbs, abdominal distension, and urinary retention/strangury, urgently use Chansu (Toad Venom), Niuhuang (Calculus Bovis), Bingpian (Borneol), and Shexiang (Moschus) ground into powder. Then, decoct Huanglian (Coptidis Rhizoma) with strong tea, let cool, take half a spoonful to mix with the powder, dip a soft rice stem in it, insert into the meatus, and take Xiaodu Yinzi (Detoxification Decoction) for excellent effect.
