1. Diseases during pregnancy include morning sickness (Ezu, nausea and vomiting), uterine obstruction (Baoku), swelling and fullness, Qi stagnation with vexation, hanging pain (Zixuan), eclampsia (Zixian), cough (Zisou), urinary retention due to bladder displacement (Zhuanbao), and painful urination during pregnancy (Zilin). There is also abnormal menstruation during pregnancy (Jijing) and fetal leakage (Tailou) leading to fetal insecurity. Additionally, there are spontaneous abortion (Xiaochan), retained dead fetus (Sitaizisi), fetal growth retardation (Taiwei buchang), aphonia during pregnancy (Ziyin), visceral dryness (Zangzao), and phantom pregnancy (Guotai). Other diseases should be treated by referring to the treatment of miscellaneous diseases, but one must constantly protect the root of the fetus.
[Note] This refers to conditions during pregnancy, including Ezu, Baoku, Zishen (swelling), Ziman (fullness), Zifan (vexation), Zixuan, Zixian, Zisou, Zhuanbao, Zilin, Jijing, Tailou, Tai dong bu an (fetal movement insecurity), Xiaochan/Duotai (spontaneous/induced abortion), Zisi fuzhong (dead fetus in uterus), Taiwei buchang, Ziyin, Zangzao, Guotai, etc. All these syndromes should be carefully differentiated and memorized. For other pre-natal conditions such as Cold Damage (Shanghan), food stagnation, malaria-dysentery, cholera-like disease (Huoluan), and diarrhea, treatment should be referenced from the section on miscellaneous diseases. However, it is essential to constantly protect the fetal root to avoid erroneous interventions.
2. Nausea and vomiting are called Ezu. If it is merely selective eating and the condition resolves after a certain period, no treatment is needed. In severe cases, medication is required, primarily addressing weak Stomach Qi, while distinguishing between rebellious Fetal Qi, phlegm-fluid retention, heat, or cold.
[Note] After a woman has been pregnant for over a month, frequent vomiting is named Ezu. If there are no other illnesses and she is merely selective in food, her preferences should be accommodated. Mild cases resolve spontaneously without medication after a period of time; severe cases require medicinal treatment. The primary cause is considered weak Stomach Qi. One must further examine whether it is caused by rebellious Fetal Qi, phlegm-fluid retention, or accompanied by heat or cold, and treat accordingly.
Baosheng Tang (Preserve Life Decoction)
3. When Ezu is caused solely by rebellious Fetal Qi with vomiting and no other accompanying symptoms, use Baosheng Tang. It contains Amomi Fructus (Sharen), Atractodis Rhizoma (Baizhu), Cyperi Rhizoma (Xiangfu), Linderae Radix (Wuyao), Citri Reticulatae Pericarpium (Chenpi), and Glycyrrhizae Radix et Rhizoma (Gancao). Add Ginseng Radix et Rhizoma (Renshen) or Aurantii Fructus (Zhike) as appropriate, using Zingiberis Rhizoma Recens (Shengjiang) as a guide.
[Note] Ezu caused by rebellious Fetal Qi occurs when, after conception, the uterine opening closes, internal organ Qi is obstructed, and this Qi rebels upward along with the Fetal Qi into the Stomach, causing nausea and vomiting. If caused by chronic Stomach deficiency, even without phlegm-fluids, mixed cold and heat may present as Ezu; Baosheng Tang is suitable. This formula consists of Sharen, Baizhu, Xiangfu, Wuyao, Chenpi, and Gancao. Zingiberis Rhizoma Recens (Shengjiang) is used as a guide to stop vomiting. If Qi is deficient, add Ginseng Radix et Rhizoma (Renshen) appropriately; if Qi is excess, add Aurantii Fructus (Zhike) appropriately.
Jiawei Liujun Tang (Modified Six Gentlemen Decoction)
4. Ezu caused by phlegm-fluids presents with vomiting of phlegm and water, vexation, and dizziness; use Jiawei Liujun Tang. It includes Eriobotryae Folium (Pipaye), Agastaches Herba (Huoxiang), Inulae Flos (Xuanfuhua), Amomi Fructus (Suosha), and Aurantii Fructus (Zhike). If there is heat with constipation, add Scutellariae Radix (Huangqin) and Rheum Radix et Rhizoma (Dahuang); if cold, add Cinnamomi Cortex (Rougui) and Zingiberis Rhizoma (Ganjiang).
[Note] Ezu caused by phlegm-fluids involves vomiting copious phlegm and water, accompanied by mental vexation and dizziness. This indicates chronic Stomach deficiency with accumulation of phlegm-fluids in the middle jiao. Jiawei Liujun Tang is indicated, which is Liujun Tang added with Eriobotryae Folium (Pipaye), Agastaches Herba (Huoxiang), Inulae Flos (Xuanfuhua), Amomi Fructus (Suosha), and Aurantii Fructus (Zhike). If Stomach heat causes constipation, add Scutellariae Radix (Huangqin) and Rheum Radix et Rhizoma (Dahuang) to promote purgation; if Stomach cold with preference for warmth, add Cinnamomi Cortex (Rougui) and Zingiberis Rhizoma (Ganjiang) to warm.
Liujun Tang formula is seen in the first volume.
Jiawei Wendan Tang (Modified Warm the Gallbladder Decoction)
5. Ezu caused by heat involves aversion to food, desire for cool drinks, mental vexation, and oppression; use Wendan Tang. It contains Citri Reticulatae Pericarpium (Chenpi), Pinelliae Rhizoma (Banxia), Poria (Fuling), Glycyrrhizae Radix et Rhizoma (Gancao), Aurantii Fructus (Zhishi), and Bamboo Shavings (Zhuru). Add Scutellariae Radix (Huangqin), Coptidis Rhizoma (Huanglian), Phragmitis Rhizoma (Lugen), and Ophiopogonis Tuber (Maidong), using Zingiberis Rhizoma Recens (Shengjiang) as a guide.
[Note] Ezu caused by Stomach heat involves vomiting, mental heat and vexation, oppression, and a desire to drink cool fluids. Jiawei Wendan Tang is indicated. The base formula is Chenpi, Banxia, Fuling, Gancao, Zhishi, and Zhuru, known as Wendan Tang. Added are Huangqin, Huanglian, Lugen, and Maidong, with Zingiberis Rhizoma Recens (Shengjiang) as a guide.
