If the pulse is slippery and large, it indicates Mania (Kuang Bing); if the pulse is choppy, small, deep, and faint, it indicates Epilepsy/Dementia-like Syndrome (Dian Bing). A small, rapid, and trembling pulse at the Cun position indicates Palpitations (Zhongchong), suggesting a prolonged course of illness. A small pulse on the right side indicates Spleen disease; the patient either suffers from Insomnia (Bu Mei) or Forgetfulness (Jian Wang). This is an internal injury caused by the Seven Emotions; urgently decoct and administer formulas that tonify the Heart. In internal injuries, if the condition worsens, it leads to the calamity of Mania. Patients with Dian Bing exhibit behaviors abnormal from their usual state. When Xi Zheng (Epileptic seizures) occurs, symptoms fluctuate between severe and mild.

In women, the Liver is the congenital foundation; only the Liver functions dominate. Within the inner chambers, what they see is rare and what they hear is scarce. They spend more time alone in wakefulness or sleep, leading to more stagnation, assisting the rise of Liver Yang. Various conditions arise, making regulation difficult. Occasionally, there are one or two individuals with gentle dispositions, secluded and chaste. Although they have no major ailments, they inevitably suffer from stagnation. Practitioners should distinguish between rigid and soft, Yin and Yang. For those who are Yin and soft, medicines should be light and thin. For those who are Yang and rigid, medicines should be dispersing and breaking. The Classics state: "For Wood stagnation, achieve dispersion; for Fire stagnation, achieve venting." This is the principle. What people today call "Liver Qi" generally encompasses all diseases of the Liver, regardless of deficiency or excess, treating them all with dispersing methods. No wonder treatment often fails. However, the Liver stores Blood. When Blood is deficient, Qi becomes hyperactive. Moreover, the Liver Meridian has abundant Qi but scant Blood. Once the Blood aspect is deficient, Qi lacks constraint, and various pathological changes arise. Some cases stem from Kidney Water deficiency failing to nourish Liver Wood, causing Liver Qi to become rampant. Some stem from Lung Metal deficiency failing to control Liver Wood, causing Wood and Fire to ascend. Some involve Liver Wood overacting on Spleen Earth. Some involve Liver Wood counter-attacking Lung Metal. Some involve Liver Wood assisting Heart Fire. Some involve excessive Liver Wood generating Wind. How can these be summarized merely as "Liver Qi"? When I treat these syndromes, I must seek the root cause. I inquire about the initial disease and how it transformed. If caused by anger, this is a primary Liver disease. If caused by worry and thought damaging the Spleen, leading to Liver Qi invading due to weakness, one should support Earth and drain Wood. In such cases, the pathology lies in the Spleen, not the Liver. I apply this principle accordingly.
