Add five large dates (Da Zao, *Jujubae Fructus*). ○ If there is congenital insufficiency, add Ren Shen (Ginseng, *Panacis Ginseng Radix*) (2 qian). ○ If kidney yang is damaged by sexual excess and ministerial fire is insufficient, add Rou Gui (Cinnamon Bark, *Cinnamomi Cortex*), Chuan Bai (Phellodendron Bark, *Phellodendri Chinensis Cortex*), and Tan Na Qi (Sea Otter Testicle, *Mustela Sibirica Testis*).
Pulse diagnosis: Diseases of the Kidney meridian often present with a soft pulse. A tight and rapid pulse at both Chi positions indicates loss of restraint over ministerial fire; a pulsating left Chi position indicates Yang conflicting with Yin; floating external fire at the right Chi position with non-rising pulses at both Chi positions, weak upon deep pressure, are causes of flaccidity syndrome (Wei Zheng), for which astringent methods should be employed. Can a nearly extinct pulse be sought? Kidney diseases present with a soggy pulse. Impotence difficult to revive requires warming and tonifying methods. If due to excessive anger, with a string-taut and slow pulse, warming and dispersing methods should support it. When the root has departed, judgment must be made from the deep position.
Reading the works of various masters, one knows that Gynecology is established as a separate discipline from Men's Medicine. In *Jin Gui Yao Lue* (Essential Prescriptions of the Golden Cabinet), three chapters on gynecology are recorded: Pregnancy, Postpartum, and Miscellaneous Diseases, covering the general outline. Women's physiological essence and blood differ from men's, but the beginning and end of the twelve meridians and the fixed locations of the five Zang organs are the same. How can women's pulse positions be reversed? Therefore, it is said that women's diseases are mostly the same as men's, except for menstruation, pregnancy, and childbirth. Here, the differences are specifically listed and described in detail for our reference. For those that are the same, follow the treatment principles for men without repetition. Also, treatment methods already existing in *Jin Gui Yao Lue* are not repeated here; one should adhere to *Jin Gui Yao Lue*.
Regulating menstruation, leukorrhea, white turbid discharge, white strangury, Qi strangury, fatigue strangury, seven types of hernia, masses and accumulations, five accumulations.
Furthermore, "Jing" (Menstruation) means constancy. It circulates regularly, once a month, hence called "Yue Xin" (Monthly Trust). "Xin" means trust or agreement, like keeping an appointment with regularity. It is also said... then overflow... menstrual flow... generating blood... destination... pulse... Chong and Ren vessels... Dai vessel acts as... leakage and masses... continuous... women... yin... yang... regulate... Like a belt, continuous and unceasing, smooth and hard to stop. Treatment should soothe the Liver (Wood) and tonify the Spleen (Earth), regulating the Dai and Ren vessels. White turbid discharge (Bai Yin) refers to white material flowing after urination, seen in small amounts. Treatment should tonify the Heart and benefit the Spleen, while adjusting the Qi aspect. White strangury (Bai Lin) involves damp-heat accumulation in the Bladder, requiring lower abdominal distending pain before discharge. Treatment should use qi-transforming methods combined with clearing damp-heat. There are five types of strangury: First is Qi strangury, where Qi deficiency sinks, causing frequent urination with residual dripping. Treatment should tonify the Heart and boost Qi, combined with clearing damp-heat. Second is Fatigue strangury, occurring upon exertion, damaging the Spleen and Kidney. Treatment should use Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction). Thirdly, Blood strangury, Gravel strangury, and Paste strangury are all caused by long-standing damp-heat damaging Kidney Yin. Treatment should nourish the source of transformation, using Da Bu Yin Wan (Great Yin Tonifying Pill) for best results. In gynecology, judging early menstruation as Heat and delayed menstruation as Cold is not incorrect, but one should not be rigid. If menstruation is dripping and frequent, it may be blocked by Qi or stagnant by Blood stasis, so this rule cannot cover all cases. Blood depletion with internal heat also delays menstruation due to Yin/Blood consumption. If a woman has a full pulse and internal heat, with early menstruation, it belongs to Heat. If the pulse is weak and limbs are inverted, with late menstruation, it belongs to Cold. Thus, diagnosing women's diseases requires asking about Tian Gui (Heavenly Water/Menstrual Essence), the amount of Blood, waist/abdominal pain, menstrual color (purple, black, yellow, red), presence of stasis. Pre-menstrual pain indicates Qi stagnation; post-menstrual pain indicates Blood deficiency. One must investigate the deficiency/excess of the pulse and combine it with the severity of the disease. Internal heat presents with a rapid pulse; internal cold with a slow pulse; Blood deficiency with a choppy pulse lacking spirit; Qi deficiency with a deep and faint pulse; pain with a tight pulse; Blood stasis with a taut pulse; Blood loss with a hollow pulse. Ancients saying "women's Chi pulse is always strong" referred to normal pulses. Modern women often have sluggish and small Chi pulses because my hometown is in the Southeast, where damp-heat prevails, leading to Qi deficiency and Blood stagnation. The North has prevailing wind-cold, located in the West, where Blood deficiency and Qi prosperity are common. Thus, in the Northwest, only 10-20% suffer from leukorrhea due to severe cold. In the Southeast, 80-90% suffer from leukorrhea due to dampness prevalence. Fat people have more Blood stagnation leading to Cold; thin people have smoother Blood leading to Heat. Fat people find conception difficult but easy to give birth; thin people conceive easily but find childbirth difficult. The way to regulate menstruation: if Blood is abundant, tonify Qi; if Qi is abundant, tonify Blood. This ensures no bias in Blood and Qi, and no excess/deficiency in Yin and Yang. Why worry about difficulty conceiving or giving birth? I record these most urgent aspects of gynecology for reference.
