Huaisheng (Stir-fried with heat), Cebaiye (Biota Tops), Jingjiesui (Schizonepeta Spikes), Zhike (Aurantium Fructus, stir-baked with bran, equal parts)
Grind all into powder. Take 3 qian per dose, administered on an empty stomach.
This formula is indicated for hematochezia caused by Changfeng (Intestinal Wind) and Zangdu (Visceral Toxin).
Huaisheng and Cebaiye can cool the blood of the Large Intestine; Jingjie and Zhike can treat wind in the Large Intestine. It is effective for conditions where wind and heat struggle against each other.
Huanglian (Coptidis Rhizoma, 12 liang), Haojiu (Good wine, 5 sheng)
Boil until the wine is dry, grind into powder, and make into pills. Take 3 qian per dose.
This formula is indicated for hematochezia caused by damp-heat and alcohol toxin.
Huanglian is a bitter, cold, and drying substance. Its bitterness and coldness overcome heat; its dryness overcomes dampness. The reason it must be boiled with wine is that only wine can guide the medicine into the blood level.
Cebaiye (Single herb: harvested from the East in Spring, South in Summer, West in Autumn, North in Winter.)
Burn to ash, mix with water or appropriate liquid, and take 3 qian.
This formula is indicated for persistent hematochezia due to Zangdu (Visceral Toxin).
In the early stage of Zangdu, cooling blood is appropriate. Cooling blood agents are mostly bitter and cold; prolonged use leads to cold qi, making the blood even less secure. The method states: "Astringency can consolidate prolapse." Therefore, the astringent property of Biota ash is used to stop bleeding. When harvesting in four seasons, directions must be distinguished to obtain the concentrated qi. Dried persimmon cake burned to ash is also good.
Renshen (Ginseng Radix et Rhizoma, rhizome removed), Chugenbaipi (Toosendan Cortex albus, equal parts)
Grind into fine powder. Take 3 qian per dose.
This formula is indicated for hematochezia due to Zangdu with heat, persisting for a long time.
Zangdu refers to intestinal toxin. "With heat" implies carrying external heat or factors like drinking alcohol. If it persists for a long time, qi becomes deficient and insufficient to secure the blood. Therefore, the sweetness of Renshen is used to tonify qi, and the astringency of Chugen is used to secure blood. Tonification lifts, astringency seizes; this is a mixed/hegemonic treatment method.
Preface: Rectal proluo (Tuogang) is one disease, but there is a distinction between cold and heat. One who can further distinguish cold and heat within qi and blood is considered highly skilled. Here, three ancient formulas are examined, and their essentials are presented.
Renshen (Ginseng Radix et Rhizoma), Huang (likely Huangqi or Huangqin, literally "Yellow"), Chuanxiong (Ligusticum Rhizome), Danggui (Angelicae Sinensis Radix), Shengma (Cimicifugae Rhizome)
This formula is indicated for rectal prolapse due to chronic diarrhea.
Chronic diarrhea injures qi; excessive lower loss destroys yin, leading to deficiency of both qi and blood, causing the Guangchang (Large Intestine/Rectum) to vacate and prolapse. For qi deficiency, tonify with sweet-warm substances; thus, Renshen and Huang are used. For yin deficiency, nourish with rich/flavorful substances; thus, Chuanxiong and Danggui are used. For sinking conditions, lift them up; thus, Shengma is used.
