Regarding the discipline of Shanghan (Cold Damage diseases), how many practitioners since Master Zhang Zhongjing have truly grasped its essence? I have examined that there are 397 methods and 113 formulas for treating Shanghan. This is because the disease changes unpredictably and repeatedly; without understanding Yunqi (Five Movements and Six Qi), one cannot know the waxing and waning of Yin and Yang; without distinguishing between Host and Guest pathogens, one cannot know the prosperity and decline of seasonal months. This is indeed difficult. One must master the Suwen (Plain Questions) and Nanjing (Classic of Difficult Issues), refine pulse diagnosis, and differentiate meridians before applying treatment. The general principles do not exceed the eight categories: Exterior, Interior, Yin, Yang, Deficiency, Excess, Cold, and Heat. Moreover, internal injuries often resemble external contractions; a slight error can lead to vast discrepancies. Li Dongyuan established the differentiation between internal injury and external contraction, which was thorough and complete. Zhu Danxi also stated that among those who came a thousand years later, only Li Dongyuan grasped the essence. Therefore, treatment methods should be carefully matched to symptoms: use sweating methods for exterior conditions and purging methods for interior conditions. If the application of sweating or purging for exterior or interior conditions is slightly inappropriate, it concerns life and death; how could this be a trivial matter?
