In all cases of tooth swelling, if it is observed that the condition is not caused by stomach fire or wind-heat, but rather presents yin manifestations in various parts, this serves as sufficient evidence. This indicates that the Yuan Qi (Original Qi) is floating upward and failing to submerge; the urgent priority is to restore Yang, collect Qi, and seal and secure. If treated with methods that nourish Yin and clear fire, death will be hastened.
Ear Swelling Without Pain
In cases of ear swelling, the skin color must remain normal; even if slightly red, it often contains a bluish hue, and yin manifestations are definitely present throughout, providing sufficient proof. Large doses of Yang-restoring formulas should be used urgently. Do not mistakenly attribute this to wind-heat in the Liver and Gallbladder and treat it according to standard external contraction protocols, as this will hasten death.
-89 -
Throat Pain Craving Hot Drinks
In cases of throat pain where the patient craves hot drinks, it is certainly not due to wind-heat attacking upward. The pulse and voice will invariably lack spirit, and yin manifestations will be fully exposed. Yang-restoring herbs should be administered cold to save the patient, yielding very rapid results. This occurs because Yang floats upward and cannot reside peacefully in its home; receiving substances of similar nature (hot herbs) guides it back to its residence. Treating it according to wind-heat protocols will hasten danger.
Persistent Coughing
In patients with chronic illness or innate insufficiency, or those who have overused cooling and dispersing herbs, sudden abnormal coughing without respite, accompanied by complete yin manifestations, indicates that Yin pathogens are interfering with the Clear Orifices above, and the Yuan Yang faces the risk of escaping from the Lungs [1]. Urgent restoration of Yang and expulsion of Yin are required; as Yang becomes robust and Yin diminishes, the cough will cease naturally. One must not continue to use Yin-nourishing or generic cough-prescribing formulas. Failing to restore Yang out of fear is seeking one's own demise.
Asthma and Blue Lips
In patients with chronic illness or innate insufficiency, sudden onset of asthma and blue lips signifies floating Yuan Qi and impending collapse, a difficult-to-treat condition. Urgent restoration of Yang, descending of rebellious Qi, and collection are necessary. Only when the asthma subsides and the lip color turns red is there hope for life. Indecision and lack of trust in the treatment plan will lead to death.
Heart Pain Desiring Death
In cases of sudden heart pain desiring death, whether the face is red or lips are blue, Yin and Yang must be carefully distinguished without negligence. If heart pain is accompanied by a red face and a desire for cold drinks, with temporary relief, this is pathogenic heat invading the Heart; urgent clearing of fire is needed. If the face is red but the patient desires hot drinks, accompanied by blue-green signs on the lips and tongue, this is cold pathogen invading the Heart; urgent support of Yang is needed.
[1] "Shi" (trend/situation) was originally written as "Re" (heat), corrected here based on context.
-90 -
Abdominal Pain Desiring Death
In cases of abdominal pain desiring death, careful examination of various parts is essential. If the lips and tongue are blue-black, this indicates stagnation of Yin-cold and failure of Yang to circulate; urgent restoration of Yang is needed. If the tongue coating is yellow, breathing is coarse, urination and defecation are obstructed, and the whole body is ice-cold, this indicates internal attack by heat pathogen blocking the Clear Orifices; urgent dispersion and unblocking of stagnation are needed, such as using Wanying Lingtong Wan (Universal Efficacy Pill), also known as Duijin Wan, Lingbao Ruyi Wan, Chuandu Puji Wan, or Yushu Wanling Dan. Half blown into the nose, half taken orally, immediate effects are seen; this must not be ignored.
Bowel Sounds and Diarrhea
In patients with chronic illness or innate insufficiency presenting with bowel sounds like thunder and incessant diarrhea, this is due to decline of Mingmen Fire (Gate of Life Fire) and extreme cold of the Zang organs. Large doses of Yang-restoring formulas are urgently needed; treating with diuretics will certainly be ineffective. I have personally verified this in many cases.
Hematochezia (Blood in Stool)
In patients with chronic illness or innate insufficiency, sudden incessant hematochezia indicates lack of fire in the Lower Jiao (Lower Burner) failing to control blood, with a trend of downward collapse. Large doses of Yang-restoring formulas are urgently needed, such as Fuzi Lizhong Tang (Aconite Regulate the Middle Decoction) or Huiyang Yin (Restore Yang Drink).
Hematuria (Blood in Urine)
This section is identical to the preceding one on hematochezia. I have personally verified this in many cases; the focus is always on restoring Yang, the efficacy of which is unfathomable, as there are no sufficient signs of pathogenic heat.
Continuous Sperm Leakage
Generally, individuals prone to sexual indulgence or with innate insufficiency suffer from spontaneous sperm leakage. This indicates severe depletion of Yuan Yang and loose sealing mechanisms. Large doses of Yang-restoring formulas are urgently needed, focusing on communicating Water and Fire. I have used Baitong Tang (White Tongue Decoction) to treat this disease with hundred percent success.
-91 -
Red Face After Noon
In cases of red face after noon or fever, the world generally attributes this to Yin deficiency. However, they do not know that in patients with chronic illness or innate insufficiency, Yang Qi declines daily, failing to suppress and collect Yin, while Yin pathogens grow stronger and float outward. Moreover, the afternoon is precisely when Yin is abundant; Yang attempts to submerge into Yin, but strong Yin does not accept it, forcing Yang outward. With Yuan Qi rising more than descending, a red face or night fever appears. These are all signs of Yin abundance. Treating as Yin deficiency will surely worsen the disease. I commonly treat this with methods of restoring Yang, collecting Qi, and communicating Upper and Lower, achieving hundred percent success.
Body Itching Desiring Death
In patients with chronic illness or innate insufficiency, sudden extreme body itching, or red spots covering the body resembling wind-rash, is actually not wind-rash. Wind-rash typically does not cause itching desiring death; it usually presents with fever, body ache, aversion to cold, and aversion to wind. In contrast, in chronic or constitutionally deficient patients, the onset is sudden, often without fever or body ache. Even if high fever is present, urine must be clear, thirst involves craving hot drinks, and yin manifestations are evident throughout. The pulse may show floating emptiness or tension like a rope. This condition urgently requires large doses of Yang-restoring and collecting methods. Treating as wind-rash will hasten death.
Profuse Sweating Like Rain
In patients with chronic illness or innate insufficiency, sudden profuse sweating like rain is a sign of Yang collapse. However, there are cases not involving Yang collapse. Profuse sweating that appears suddenly and stops within moments is not Yang collapse, but rather Yin pathogens exiting through the pores, indicating disease resolution. If the patient has faint breath and becomes cold immediately after sweating, it is true Yang collapse, considered untreatable. To save them, only the method of restoring Yang remains. Nevertheless, extreme Heat in the Yangming channel steaming outward can also cause profuse sweating like rain, provided there are sufficient signs of Yang syndrome symptoms. Here, however, all Yin manifestations are present and verifiable.
Profuse Sweating and Hiccups
In patients with chronic illness or innate insufficiency, or those who have overused harsh cooling herbs, sudden profuse sweating and hiccups indicate Yang collapsing externally and Qi of the Spleen and Kidneys terminating internally, a sign of death within days. Urgent restoration of Yang and descending of rebellious Qi are needed. After taking medicine, if sweating stops and hiccups cease, there is hope for life. If one still uses common anti-sweat herbs like Mahuanggen (Ephedrae Radix et Rhizoma) and Xiaomai (Tritici Fructus), or anti-hiccup herbs like Dingxiang (Caryophylli Flos) and Shidi (Diospyri Exocarpium), death will occur immediately.
Body Heat Without Spirit
In patients with chronic illness or innate insufficiency, or those who occasionally strain their mind, sudden high body heat without pain or discomfort, merely presenting as lethargy and lack of spirit, with no thirst or appetite, indicates Yuan Qi emitting outward. Restoration of Yang and collection is appropriate, curable in one dose. Mistaking this for fever and treating as external contraction will hasten danger; most people fail to recognize this.
Hemoptysis with Body Heat
Patients with hemoptysis mostly suffer from Qi deficiency failing to control blood. Vomiting blood causes Qi mechanism to move outward, and Yuan Qi follows outward, resulting in body heat. Urgent restoration of Yang and collection is primary. One must not see hemoptysis and immediately assume Fire, applying cooling agents.
Vomiting with Body Heat
The Classics state: Vomiting leads to loss of Yang. Vomiting belongs to Taiyin. Patients with severe vomiting often have middle jiao Cold or Heat, or food obstruction. If, after vomiting, the whole body becomes extremely hot without sufficient signs of Taiyang, Shaoyang, or Yangming exterior syndromes, this belongs to Yuan Qi of the Spleen and Stomach emitting outward. Urgent collection of Middle Jiao Yuan Qi is primary. One must not continue to treat with Huoxiang Zhengqi San (Agastache Powder to Rectify the Qi). For this syndrome, I often use Gancao Ganjiang Tang (Licorice and Ginger Decoction) added with Sharen (Amomi Fructus Rotundus), achieving ten cures out of ten.
Diarrhea with Body Heat
In patients with chronic illness or innate insufficiency, sudden severe diarrhea followed gradually by high body heat belongs to Yang collapse. High heat indicates Yang exhaustion above; severe diarrhea indicates Yin collapse below. Urgent warming of the Middle and collection is primary. One must not see body heat and claim external contraction, nor see severe diarrhea and claim
-93 -
food stagnation. Using exterior-resolving, digestion-promoting, or diuretic methods will bring disaster immediately; this must not be ignored.
Body Heat After Noon
The Classics state: Yin deficiency generates internal heat. This refers to pathogenic Qi flourishing and Blood declining, not specifically referring to afternoon or night fever as Yin deficiency. Current practitioners fail to explore the principles of Yin-Yang balance and fullness/emptiness. Upon seeing afternoon or night fever, they immediately claim Yin deficiency and prescribe water-nourishing herbs. Their reasoning assumes that since afternoon belongs to Yin, it must be Yin deficiency, ignoring that afternoon and night are precisely when Yin is abundant, not a sign of Yin deficiency. Even if fever occurs, it mostly belongs to Yin abundance repelling Yang outward, preventing Yang from submerging and causing Yang to float outward, thus manifesting as body heat. Why? The True Qi of the human body begins to generate one Yang at Zi hour (11 PM–1 AM), passing through Chou, Yin, Mao, Chen, Si hours, Yang becomes extremely vigorous until Wu, Wei, Shen, You, Xu, Hai hours, when Yang declines and submerges. Now, blocked by Yin, it cannot descend, hence frequent fever. This concerns the fluctuation of Yin-Yang abundance and the entry/exit of Yuan Qi; it must not be ignored.
For this syndrome, regardless of night or afternoon fever, red face, red lips, empty pulse, craving hot drinks, or lack of spirit, I use Baitong Tang, with repeated successful treatments.
Bleeding from Skin and Hair
In patients with chronic illness or innate insufficiency, sudden bleeding from skin and hair indicates insufficiency of Defensive Yang protecting the exterior. Urgent restoration of Yang and collection is needed; delay is inadvisable.
Retraction of Scrotum
In patients with chronic illness or innate insufficiency, sudden retraction of the scrotum and abdominal pain indicates excessive Yin-cold in the Jueyin channel and extreme deficiency of Yang. Urgent restoration of Yang is needed. Alternatively, moxa can be burned on the Dantian or navel; or pepper powder wrapped and stuffed into the navel, blown into the abdomen by a strong person’s breath. Stop when pain ceases; this is also an excellent emergency method.
Extreme Burning Sensation in Both Feet
In patients with chronic illness or innate insufficiency, or during night sleep or afternoon, extreme burning sensation in both feet, desiring to step on stone, accompanied by lethargy and lack of spirit, indicates Yuan Qi surging outward with a trend of Yang collapse. Urgent restoration of Yang and collection is primary. One must not rashly claim Yin deficiency and use Yin-nourishing drugs.
Swelling and Heat in Both Hands
In constitutionally deficient individuals, sudden swelling of both hands like basins, slight pain and redness, unbearable burning heat at night or afternoon, indicates Yin abundance forcing Yang, with Yang escaping from the hands. Urgent restoration of Yang and collection is primary.
Sudden Swelling of Both Breasts
In constitutionally deficient individuals, sudden swelling of both breasts with normal skin color indicates Yuan Qi escaping from the breasts. Do not treat as sores; prioritize restoration of Yang and collection.
Non-healing Wound Sores
Wounds that fail to heal for a long time mostly belong to severe damage of Yuan Qi, unable to resolve toxins and generate flesh. Only large doses of Yang-restoring formulas are suitable. As Yang returns and Qi flourishes, toxins dissipate naturally, and wounds close. Avoid nourishing Yin and using cooling herbs; do not treat the sore locally alone.
Flat Pockmarks (Smallpox)
Flat pockmarks are fundamentally due to lack of Fire. Only large doses of Yang-restoring formulas are suitable; Yin-nourishing herbs must not be combined.
Rectal Prolapse
In constitutionally deficient individuals, rectal prolapse due to severe diarrhea or dysentery indicates lack of fire in the Lower Origin, failing to retain. The method should restore Yang and collect Kidney Qi. Moxibustion on Baihui (GV20) is also a good method.
Uncontrolled Urination
In patients with chronic illness or innate insufficiency, sudden frequent urination dozens of times a day, each time clear, long, and abundant, indicates lack of fire in the Lower Origin. Urgent restoration of Yang and collection of Kidney Qi is needed; diuretics must not be used rashly.
Abdominal Pain Immediately Followed by Diarrhea
In patients with chronic illness or innate insufficiency, lower abdominal pain immediately followed by diarrhea or loose stools, ten-plus times a day, indicates decline of fire in the Lower Jiao and stagnation of Yin-cold Qi. Urgent restoration of Yang is needed. Do not solely focus on regulating Qi and separating turbidity.
Body Ache Without Fever
In patients with chronic illness or innate insufficiency, sudden body ache without fever indicates interior Cold; the method should warm the interior. However, after taking interior-warming herbs, some may experience increased body heat and severe ache; this is Yin pathogens dispersing, a sign of imminent recovery. Do not rashly use cooling herbs to stop it.
Body Heat Without Ache
In patients with chronic illness or innate insufficiency, or those who have overused harsh dispersing herbs, sudden body heat without pain, accompanied by sufficient yin manifestations in various parts, indicates Yang floating outward. Urgent restoration of Yang and collection is needed; do not rashly use Yin-nourishing or ascending/dispersing herbs.
Cold Exterior with Internal Heat
In patients with chronic illness or innate insufficiency, the exterior feels cold while the interior feels unbearably hot, desiring coolness for comfort. However, cool substances are rejected upon ingestion; the tongue is blue and slippery, the person lacks spirit, and urination/defecation are free. This is Yin tide emerging. Do not rashly use Yin-nourishing or cooling products; urgent large-dose Yang restoration is needed. As Yang returns, Yin tide extinguishes naturally. If it were truly an epidemic pattern of cold exterior and heat interior, the person would be irritable, thirsty for cold drinks, have obstructed urination/defecation, and possess spirit; then purgatives would be indicated, and Yang restoration would be dangerous.
Heat Exterior with Cold Interior
In chronic patients, sudden high body heat with severe internal cold, requiring multiple layers of blankets, indicates Yang floating outward and Cold blocking internally. Urgent restoration of Yang is needed; as Yang returns to its storage, the exterior naturally ceases to be hot, and the interior naturally ceases to be cold. Do not mistake this for exterior pathogen; giving exterior-resolving herbs will cause immediate Yuan Qi collapse. Such syndromes mostly lack sufficient signs of external contraction. Even if Taiyang exterior signs exist, adding small amounts of Guizhi (Cinnamomi Ramulus) and Ma Huang (Ephedrae Herba) to large-dose Yang-restoring formulas ensures safety.
Heavy Body and Aversion to Cold
In patients with chronic illness or innate insufficiency, sudden heavy body and aversion to cold indicate Yin abundance and Yang weakness. Urgent restoration of Yang is needed.
Stiff Body and Inability to Move
In patients with chronic illness or innate insufficiency, or those who have overused harsh dispersing herbs, sudden stiffness and inability to move indicate extreme decline of True Yang, insufficient Yang, failure of the Sovereign Command, excessive Yin Qi blocking meridians. Large-dose Yang restoration is suitable; as Yang flourishes and Yin diminishes, Righteous Qi recovers, and the Sovereign Command resumes function, curing the disease. People fail to recognize this, often diagnosing stroke and using wind-dispelling herbs, frequently causing deteriorated syndromes; this must not be ignored.
Light Feet and Heavy Head
In patients with chronic illness or innate insufficiency, sudden light feet and heavy head indicate Yin encroaching upward and Yang declining internally. Urgent restoration of Yang and collection of True Qi are needed; as Yang flourishes and Yin diminishes, head heaviness ceases, signaling hope for life.
Numb Feet and Soft Body
In patients with chronic illness or innate insufficiency, numb feet and soft body are common. This indicates severe Yang deficiency failing to permeate. Urgent sweet-warm Yang support is needed. As Yang becomes sufficient, the disease cures itself.
Asthma and Tense Pulse
In chronic patients, sudden asthma with tense pulse indicates Yang exhaustion above, a sign of imminent death. Urgent Yang restoration can save one or two out of ten. Unless it is a close relative, one must absolutely not prescribe; even if prescribing, one must clearly state the prognosis to avoid resentment. Do not see tense pulse and claim excessive Fire, then proceed to nourish Yin and descend Fire.
Hemoptysis and Large Pulse
In patients with hemoptysis, sudden洪大 (Surging-Large) pulse [1] indicates Yang exhaustion above, a critical sign of collapse. Current practitioners often claim: Hemoptysis belongs to Fire, Large pulse belongs to Fire; this is all due to failing to understand Yin and Yang clearly. [Marginal Note] If one knows that Blood is Water and Qi is Fire, one understands the error of Yin-nourishing and the efficacy of Jiang (Zingiberis Rhizoma) and Fu Zi (Aconiti Lateralis Radix Praeparata).
Deficiency Labor and Pulsation
In patients with extreme deficiency, the pulse should only be Deep-Fine. If Surging-Large, Fine-Rapid, Wiry, Tight, Tense, striking stone, like thick rope, or like sparrow pecking/cauldron boiling are seen, these are signs of death. Absolutely do not prescribe. If it is a close relative/friend with urgent circumstances, only large Sweet-Warm tonics should be used to support. If the pulse energy becomes peaceful, there is hope. Absolutely do not rashly use Yin-nourishing. Know that deficiency patients mostly belong to Qi deficiency; manifested symptoms often resemble Yin deficiency, but are actually not Yin deficiency.
I have seen deficiency patients often lacking Qi, reluctant to speak, heavy body, drowsy, tidal fever but no thirst, reduced diet, and lack of spirit in all activities. Clearly Yang deficiency, yet showing not a trace of Fire excess or Yin deficiency appearance.
Recent market habits diagnose such conditions as Yin deficiency, using mostly Sweet-Cold Yin-nourishing herbs. Few dare to use Acrid-Warm herbs. Thus, once becoming Deficiency Labor, nine out of ten die. Not dying of disease, but dying of medicine; not dying of medicine, but dying of physicians. All due to physicians not understanding Yin-Yang principles and patients fearing Acrid-Warm herbs, resulting in few survivors; truly a great regret.
The above dozens of items reveal Yuan Qi leaving its root and critical signs of Yang deficiency nearing collapse. Although symptoms differ, the source is one. If scholars carefully comprehend, they will have certainty in mind, recognizing instantly upon sight,

Medication will not be confused. Although not ten cures out of ten, it will not cost lives. But diseases have myriad forms, not exhausted by dozens of items. Scholars should explore the fluctuations and entry/exit of this Yuan Qi; even thousands of conditions cannot exceed this scope. I offer one more word: The three hundred and sixty bone joints of the human body each have Shen (Spirit) and Gui (Ghost). Shen is the spirit of Yang, the master of Qi. Saying "each joint" means Righteous Qi protects each. Gui is the spirit of Yin, the master of Blood. Saying "each joint" means True Yin protects each. Hence, former sages said: Ghosts and Spirits fill the universe. Universe refers to Heaven and Earth, and also to the human body. Whenever any joint shows Gui signs, it is Yin pathogen. Treat it with Shen. Shen is Yang, Fire, Qi. Treating Yin with Yang is increasing the source of Fire to eliminate Yin shadows. It is the meaning of supporting South and draining North, supplementing Fire and managing Water. Medication includes Guizhi (Cinnamomi Ramulus), Fuzi (Aconiti Lateralis Radix Praeparata), Ganjiang (Zingiberis Rhizoma), Sharen (Amomi Fructus Rotundus), following one lineage. Whenever any joint shows Xie Shen (Evil Spirit) causing harm, "Xie Shen" clearly means not the flourishing of Righteous Qi, but the flourishing of Pathogenic Qi; Pathogenic Qi is Evil Fire. Heaven and Earth are filled with Righteous Qi; Righteous Qi cannot harm, only Qi (when deviant) harms, hence called Xie Shen. It can be subdued by Gui. Gui is Yin, Blood, Water. Xie Shen is Evil Fire. Subduing Shen with Gui is using Water to treat Fire, nourishing Yin and descending Fire. Medication includes Sanhuang Shigao (Three Yellows Gypsum Formula), Da Chengqi Tang (Major Order the Qi Decoction), Xiao Chengqi Tang (Minor Order the Qi Decoction), etc. Current practitioners often say nourish Yin and descend Fire, establishing theories for Evil Fire injuring Yin, yet there is no true Yin deficiency. Even if claimed as Yin deficiency, it is Yang deficiency. Why? Yin and Yang are originally one Qi, inseparable. Hence, the Classics state: When Qi flourishes, Blood flourishes. When Qi declines, Blood declines. When Qi ascends, Blood ascends. When Qi descends, Blood descends. Where Qi is, Blood is. Where Qi perishes, Blood perishes. Understanding this principle reveals the purpose of "Heaven generates One Water" and the profound truth of Sweet-Warm and Acrid-Warm Yang restoration. Scholars need not speculate elsewhere; through the phrase "Ghosts and Spirits," comprehend the Yin-Yang of the whole body, follow the rules of medication from Yin or Yang, recognize the key of Righteous vs. Pathogenic, identify errors of various schools, and one can surpass ordinary levels, reaching divine transformation.
Essentials of Pulse Differentiation
Floating pulse governs Wind; Surging pulse governs Fire; Full pulse governs Heat; Rapid pulse governs Heat; Tight pulse governs Cold; Slippery pulse governs Phlegm; Deep pulse belongs to Yin; Slow pulse belongs to Cold; Fine pulse indicates insufficiency; Thready pulse indicates insufficiency; Empty pulse indicates insufficiency; Weak pulse indicates insufficiency.
Regarding the above pulse images, various books state: Floating governs Wind; Surging, Full, Rapid, Tight, Slippery govern Fire, Heat, Cold, Phlegm. I assert that Floating pulse cannot be hastily diagnosed as Wind. Surging, Large, Full, Rapid, Tight, Slippery cannot be hastily generalized as Fire, Heat, Cold, Phlegm. Deep, Slow, Fine, Thready, Empty, Weak, also cannot
