When a disease presents with fever and signs of fire, and the pulse is surging (Hong) and rapid (Shu), it is treatable. If the pulse is deep (Chen) and minute (Wei), indicating an absence of fire, and if the pulse lacks root, the condition is critical.
If a patient has fever accompanied by symptoms of fire, this constitutes a Fire Disease (Huo Bing). In such cases, the pulse should be surging and rapid. Since the pulse matches the syndrome, treatment is feasible. However, if a deep and minute Yin-type pulse is obtained instead, it indicates the absence of fire. Fever persisting without actual fire suggests "Rootless Yang" (Xu Yang Wai Yue, floating yang due to deficiency). This represents a deficiency pattern manifesting as heat, so the prognosis is guarded/critical.
