Individuals engaged in hard labor often have thick, coiled tendons in their legs and feet, indicating either full blood vessels within the tendons or excessive dampness. In cases where there is excessive heat in the blood level, even if external sores appear, mistakenly using knives or needles may cause uncontrollable bleeding that is incurable.
Swallow's nest (Yanwo) is formed by swiftlets attaching small fish to rocks, utilizing the vital qi contained in the swallow's mouth to clear and tonify Lung Yin. However, it is a "fa wu" (food that can trigger or exacerbate diseases) among seafood. If mistakenly consumed during external conditions, it can block pores, causing toxins to sink inward and not exit, leading to spreading swelling and unmanageable ulceration.
The Lung governs the skin and hair, while the Spleen governs the muscles. When these two organs are deficient, damp pathogens linger, transform into heat, and flow downward. Severe heat causes red skin and fever; severe dampness causes swelling, obesity, and aversion to cold. This mostly occurs in midsummer when yang qi expands and damp-heat spreads, affecting those with weak constitutions who lead sedentary lives. Symptoms include swelling followed by pain in the lower legs, known as "Damp Hong Kong Jiao" (Hong Kong Foot caused by damp-heat), representing blood injury affecting qi. Alternatively, it may start with slight redness and swelling at the popliteal fossa, or begin at the toes and lower legs, then spread to swell the entire leg or both lower legs; such cases can resolve within seven days. If the pathogen of Hong Kong Foot rushes upward, it indicates Water insulting Fire, manifesting as incessant vomiting and relentless panting, which is a adverse prognosis. Treatment should employ Danggui Niangong Tang (Angelica Ligusticum Chuanxiong Decoction). (Formula see later.)
If the lower legs and shins do not swell but hurt, with curling up and wasting/thinning, it is due to severe heat injuring the blood, known as "Dry Hong Kong Jiao" (Dry Hong Kong Foot), suitable for Liangqing Xengzao Tang (Cool Blood Dryness-Resolving Decoction). (Formula see later.)
During the shedding of necrotic tissue from external sores, if too much pus and blood are lost, righteous qi struggles to support. The base flesh of the lesion appears gray with no vitality. Or sudden changes occur: dryness without pus, surrounding skin turning red with halos, locking the wound opening. Puncture or cutting yields no pain; thirst and dry lips ensue; the mind is panicked; expression is dull; pulse is wiry and hard. These all indicate failure of Stomach Qi, exhaustion of blood, death of flesh, leakage of rootless floating fire, separation of Yin and Yang, with only four to five days of life remaining.
Deficiency of both Spleen and Kidney, compounded by overexertion damaging sinews and bones, causes dull pain in the waist and hips, aversion to cold, fever, poor appetite, and emaciation. A plum-sized protrusion/swelling appears in the spine; initially ignored, it gradually leads to kyphosis (hunchback) and neck retraction. Since Kidney decline causes bone flaccidity and Spleen damage causes muscle wasting, Guibei Tan (Tortoise Back Phlegm) has formed. Few recover; even if life is saved, the patient becomes disabled. Long-term administration of Bushen Tang (Kidney-Tonifying Decoction) is appropriate. (Formula see later.)
