When diagnosing the pulse of married women, one must first inquire when her menstruation is due and when it ends. If it occurs on schedule every month, it is unrelated to menstrual disorders or pregnancy. However, if menstruation has ceased for two or three months, unless it is due to natural causes related to Tian Gui (substances produced by kidney essence that promote reproductive maturity), it may be a sign of pregnancy. In medication, ingredients that harmonize Ying blood and stabilize the fetus should be added, so as not to contradict therapeutic principles. One must avoid using strongly heating and blood-breaking substances (such as Tao Ren [Peach Seed] and Fu Zi [Aconite Root Lateralis Preparata]), which could damage the fetus and endanger life.
As the disease is about to recede, patients often experience epigastric discomfort and restlessness in the chest, caused by floating fire in the Stomach channel surging upward; they feel relief only after eating food. However, during the illness, the essential fats and fluids of the zang-fu organs have been consumed, and the intestines and stomach become withered and thin. It is crucial to endure hunger for one full day (twenty-four hours) until the Stomach Qi gradually recovers. If food is given hastily, the Spleen Earth, being weak, may fail to digest and transform it. Consequently, formless residual pathogens may rely on formed grain qi as a basis for attachment, leading to stagnation and clustering, allowing minor flames to reignite fiercely. This results in high fever and mental confusion, making treatment more difficult than at the onset of the disease, often leading to deep-seated conditions that are incurable. Zhang Zhongjing referred to this as "Shi Fu" (recurrence due to improper diet), treating it primarily with Zhike Zhizi Tang (Bitter Orange and Gardenia Decoction, see formula below), plus herbs for resolving stagnation and clearing heat. Symptoms may take four to five days to gradually subside, and some cases may drag on without recovery. After enduring hunger for one day, one should first consume rice water for two to three days, followed by thin congee.
The way of Heaven and Earth involves Yang imparting and Yin receiving. Sexual arousal in both men and women arises from Xiang Huo (Ministerial Fire). When a woman engages in excessive sexual intercourse with men, the refined essence and pathogenic fire accumulate and ferment into toxicity, lying dormant in the Chong and Ren vessels. The Kidney opens into the two yin orifices (anterior and posterior), and the Liver governs free flow. Ministerial Fire resides internally within the Liver, and the body's Qi and Blood circulate through all bones and joints. During coitus, blood from the entire body is drawn to Ming Men (Gate of Life, located between the two kidneys, root of Yuan Qi) and transformed into semen for emission. Thus, joints relax, and the mind feels calm, but Yuan Qi is secretly depleted. Excessive lust leads to Essence depletion; Essence depletion leads to Qi deficiency; Qi deficiency leads to Shen weakness. This often results in Lao (consumptive disease) or Wei syndrome (flaccidity syndrome). For women, this represents long-term toxicity affecting the Liver and Kidneys. If a man engages in sexual activity with such women, the pathogenic toxin will invade inwardly when Qi and Blood are deficient after ejaculation. This is known as "Yu Ran Zhe" (those infected via sexual contact). The disease typically begins with small red bumps in the lower region, sometimes as large as a plum, accompanied by rotting and foul odor, bone and joint soreness, or dysuria. It can manifest throughout the body and is difficult to cure quickly. Long-term use of Xian Yi Liang San can lead to recovery (see formula below). If the disease arises from inhaling the odor of someone with syphilis or consuming filthy substances, it is considered caused by Qi transformation and is relatively easier to treat.
