The *Neijing* (Yellow Emperor's Inner Canon) states: Pus and blood that are thick and sticky all belong to Fire. The Taiyin meridian governs diarrhea, while the Shaoyin meridian governs dysentery; this indicates that diarrhea occurs first, leading to the depletion of body fluids. Fire tends towards dryness, and the Kidney dislikes dryness. When pathogenic factors reside in the blood level of the lower jiao, it results in stools containing pus and blood. However, green and white colors indicate Cold, while red and yellow colors indicate Heat; therefore, careful examination is required during treatment. For Heat patterns, Jianzhong Wan (Firm the Middle Pill) or Doukou Xianglian Wan (Cardamom and Coptis Pill) should be used. For Cold patterns, Baijiaoxiang San (White Amber Powder) should be used. If there is more Heat than Cold, Shuizhu Muxiang Gao (Water-Boiled Agarwood Paste) is indicated. For deficiency-type slippery discharge with frequent occurrences, astringents should be employed, such as Yangzang Tang (Nourish the Viscera Decoction). When the pathogenic qi has largely subsided but the healthy qi has not yet recovered, the Spleen should be tonified. Just as after diarrhea and dysentery cease, if the Spleen and Stomach are weak and unable to tolerate food, one must not indiscriminately use harsh purgative agents. By nourishing the Spleen Qi, once Stomach Qi is sufficient, appetite will naturally return. It is advisable to use Yigong San (Extraordinary Power Powder) from Qian Yi’s formulas. If, however, excessive indulgence in food damages the Spleen and Stomach, leading to epigastric and abdominal fullness, vomiting, and nausea, this rule does not apply rigidly. Instead, Jupi Zhizhu Wan (Tangerine Peel and Immature Bitter Orange Pill) should be used flexibly; take one dose until relief is achieved, then stop. If dietary regulation causes no damage, the Stomach Qi will become harmonious.

In the eighth month of the Ding Si year, traveling with the army through Dengzhou, continuous rain prevailed, and many people suffered from dysentery. I obtained Baizhu Angwei San (Atractylodes Stomach-Calming Powder) and Shengbingzi (Holy Pills) from Gao Zhongkuan. They proved highly effective, so I record them here.
[Baizhu Angwei San] Treats all types of diarrhea and dysentery, regardless of mixed pus and blood, tenesmus, severe pain, and frequency day and night. Also treats intestinal Qi pain, cold deficiency below the umbilicus in women, postpartum uterine mass pain, and postpartum weakness with persistent chills and fever.

[Xuanqing Wan] Treats severe dysentery with frequent severe pain, chronic unhealed cases where other medicines fail. Requires emesis and purgation to expel damp-heat, relieve stagnation and fullness, and promote circulation of Qi and fluids. Take three times daily (empty stomach, midday, before sleep) until brisk purgation is achieved. Maintain with 15 pills daily. After several days, if digestion improves but dysentery persists, slowly increase dose to induce purgation again. After purgation, resume maintenance dose, adjusting according to condition until disease resolves. Then stop based on symptoms. For children, make mustard-seed-sized pills. To treat convulsive malnutrition and accumulated heat without purgation, maintain 10 pills daily.
[Ejiao Meilian Wan] Treats dysentery, regardless of duration, color (red, white, green, black), or associated pain.
