[Sanhuang Wan (Three-Yellow Pill)] Treats heat in the Sanjiao (Triple Burner).
Coptidis Rhizoma (Huanglian), Scutellariae Radix (Huangqin), Rhei Radix et Rhizoma (Dahuang) (equal parts).
Grind the above into powder. Make pills with refined honey, size of Cerasi Fructus (Wutongzi). Take 30 pills each time, swallow with warm water.
[Huanglian Jiedu Tang (Coptis Decoction to Relieve Toxicity)] Treats severe heat with extreme restlessness, delirious speech, and insomnia.
Coptidis Rhizoma (Huanglian) (7 qian 1/2), Phellodendri Cortex (Huangbai), Gardeniae Fructus (Zhizi) (each 0.5 liang), Scutellariae Radix (Huangqin) (1 liang).
Finely chop the four herbs into a coarse powder. Take 5 qian per dose, add 1.5 sheng of water, decoct to 1 sheng, remove dregs, and take hot. If not effective, take again. ○ Haicang added Saposhnikoviae Radix (Fangfeng) and Forsythiae Fructus (Lianqiao), named Jinhua Wan (Golden Flower Pill), for wind-heat; added Bupleuri Radix (Chaihu), for tidal fever in children; combined with Siwu Tang (Four-Substance Decoction) as Banban Tang (Half-Half Decoction), for tidal fever in women.
In the early days of the fourth month of the Jiayin year, I followed Wo'erduo to stay at Jiehe River. Chou Wumin fell ill for five to seven days, became manic and chaotic, stripped off his clothes and ran, shouting without distinguishing between relatives and strangers, holding milk and offering it to others. People at the time said he had "wind madness" (mental disorder). The shaman's prayers did not cure him but made it worse. The superior heard of this and ordered me to treat him. The pulse was six beats per respiration (rapid pulse), no bowel movement for several days, thirst craving milk. I reasoned: The northern lands are high and cold, with dense interstices (couli), so few people suffer from Shanghan (Cold Damage). However, at this time in the north, it is alternately cold and hot. Thus, he caught cold pathogens, failed to resolve them properly, and the condition transferred to the Yangming syndrome. Stomach excess led to delirium, and eating mutton aggravated the heat. The combination of two heats is called "excess yang leads to mania." When yang is excessive, purgation is indicated. I urgently prescribed Dachengqi Tang (Major Order the Qi Decoction) 1.5 liang, plus Coptidis Rhizoma (Huanglian) 2 qian, decocted in water and administered. That night, he had several bowel movements, passing more than twenty pieces of dry feces, and recovered after sweating. The next day, I visited again; his body was cool, pulse calm. Everyone rejoiced, saying, "Physician Luo Qianfu can indeed cure wind madness!" From this, he was heavily utilized. This shows that Shanghan differs from miscellaneous diseases; the Six Channels have different transmission patterns and variations. If one doubts during diagnosis and does not know the source of the disease, one will immediately face jealousy and slander. Alas! Those who covet profit and fame, and are ashamed of learning, when will such illnesses be cured?
Can Deifu, Counselor of Two Provinces, traveled to Dadu in March of the Shenxu year of Zhiyuan. He contracted Shanghan on the way, barely reaching Zhending, and stayed at Counselor Chang's residence. After several days of delay, the illness did not improve. Li Jingli and Ma Lushi of the General Office came to seek treatment. I went to examine him. The pulse in both hands was deep and rapid. Externally, however, his body was cool, limbs were cold (Jue), faint purple macules appeared on the skin, lips and gums were cracked and colorless, throat was dry, voice hoarse, he was silent and wanted to sleep but could not close his eyes, spirit was depressed and confused, tossing and turning uneasily. This syndrome is "the deeper the heat, the deeper the cold extremities," evolving into Huhuo syndrome (a condition characterized by internal heat-toxin causing mental confusion and mucosal lesions), which is most critical. Asking the attendants, they replied: "He started with headache, stiff body, fever, and aversion to cold after catching a cold in Neiqiu County. The doctor used Baijie San (Hundred-Resolution Powder) to induce sweating. Sweat soaked his back, yet the illness did not resolve. At every commandery we passed, the treatment was the same, inducing excessive sweating, leading to this state." I analyzed his account carefully. Additionally, he usually had accumulated heat from rich food inside, which had already dried up his fluids. Combined with excessive sweating, fluids were depleted again, transferring to the Yangming syndrome, hence the difficulty in defecation. I urgently used Dachengqi Tang to purge. After changing clothes, I administered Huanglian Jiedu Tang (Coptis Decoction to Relieve Toxicity) again, and the illness reduced by more than half. Then I gave Huanglian Xijiao Tang (Coptis and Rhinoceros Horn Decoction), and he stabilized after several days. From then on, Deifu's friendship with me grew stronger.
