Patient Zhang (79 years old), with disordered Yin and Yang, experienced simultaneous vomiting and diarrhea. He could not swallow even a spoonful of water, his urination was obstructed, he felt thirsty with a dry tongue, and both pulses were weak and faint. This condition belongs to a critical and dangerous syndrome. Tentatively prescribing the Linqing Method (Clearing and Regulating Method) to observe the changes.
Poria (Fuling), Polyporus (Zuling), Tangerine Peel (Jupi), Magnolia Bark (Houpo), Coix Seed (Bian Dou), Patchouli (Guang Huo Xiang), White Peony Root (Baishao, stir-fried), Atractylodes Macrocephala (Yuzhu), Pinellia (Banxia), Uncaria (Gouteng).
After two doses, vomiting and diarrhea ceased, he advanced to thin rice gruel, the pulse became responsive, the tongue moistened, and tendons relaxed. It is appropriate to use aromatic herbs to awaken the Stomach and sweet/bland herbs to benefit the Spleen.
Codonopsis (Dangshen), Atractylodes Macrocephala (Yuzhu), Poria (Fuling), Broad-leaved Citrus Peel (Guangpi), Magnolia Bark (Houpo), Coix Seed (Bian Dou), Pinellia (Banxia), Amomum Fruit (Shake), Honey-fried Licorice (Zhigancao).
Patient Xu (28 years old) reported suffering from malaria twice. After bathing in a river, the quotidian malaria resolved, but immediately followed by inability to urinate. The abdomen was hard as stone, protruding like a mound, legs were swollen and shiny, breath was short, and spirit was dull. It had been seven days; the situation was critical and unbearable, worse than anything else. Previous physicians administered Chengqi Decoction and Gengyi Pills. Bowel movements slightly cleared, but urine was merely dripping. The pulse was wiry and forceful, clearly indicating malarial pathogen accumulation, with Bladder Qi failing to transform. Since the Foot-Shaoyang channel disease involves the Foot-Taiyang channel, treatment should drain Shaoyang and open Taiyang.
Bupleurum (Chaihu, 1 qian), Scutellaria (Huangqin, 1.5 qian), Green Citrus Peel (Qingpi, 1 qian), Tangerine Peel (Jupi, 1 qian), Poria (Fuling, 1.5 qian), Polyporus (Zuling, 1 qian), Alisma (Zexie, 1.5 qian), Lotus Stem (He Geng, 7 cun).
Taken in the evening, by late night sounds were heard in the abdomen. Previously instructed to stimulate the nose with Juncus (Dengxincao), after several sneezes, urine flowed like a torrent. By dawn, five voidings occurred, totaling about one dou, foul-smelling, and the illness seemed to vanish. The brilliance of this formula lies in the heavy use of Chaihu.
Patient Zhou (36 years old), food regurgitation after eating, dry stool. This results from irregular diet and labor injuring Middle Qi, as stated in the *Neijing*: "Disorder of the nine orifices all belong to Stomach disease."
Codonopsis (Dangshen), Ophiopogon (Maidong), Pinellia (Banxia), Chenpi, Poria (Fuling), Dendrobium (Chuanhu), Licorice (Gancao), Sugarcane Juice (Zheshuang).
Patient Shen (38 years old), pulse is deficient and rapid. Hemoptysis occurs upon exertion. This stems from labor injuring Spleen Qi, causing the Spleen to fail to control blood. The Classics state: "For deficiency/exhaustion, warm it." This syndrome must be warmed and nourished. If one sees rapid pulse, coughing, and hemoptysis, and immediately administers cold/cooling herbs, damaging Middle Qi, the illness will surely worsen.
Codonopsis (Dangshen, 2 qian), Atractylodes Macrocephala (Yuzhu, 2 qian), Astragalus (Huangqi, 2 qian, stir-fried), Angelica Sinensis Root (Guishen, 1.5 qian, charred), Cinnamon Twig Wood (Guizhimu, 0.5 qian), Schisandra (Wuweizi, 10 grains), Tangerine Peel (Jupi, 0.8 fen), Honey-fried Licorice (Zhigancao, 0.4 fen).
Add Ginger and Jujube.
Miss Chen (20 years old), began coughing and choking in late spring, hemoptysis in mid-summer, aversion to wind, tidal fever, rapid pulse. This is due to Lung affected by Wind-Heat, Qi unable to disperse outward, leading to reckless blood movement. It is not comparable to Yin deficiency with Fire rising; there is no need to worry. Simply lightly dispersing Taiyin Lung Qi will yield quick results.
Apricot Kernel (Xingren, 2 qian, crushed), Saposhnikovia (Fangfeng, 1 qian), Mint (Bohe, 1 qian), Mulberry Leaf (Sangye, 1.5 qian, honey-fried), Moutan Cortex (Danpi, 1.5 qian), Peucedanum (Qianhu, 1.5 qian), Forsythia (Lianqiao, 1.5 qian), Licorice (Gancao, 0.4 fen).
The previous formula regulated the exterior and descended rebellious Qi. With heat subsiding and cough calming, it is evident that hemoptysis often has external pathogen factors. Ignorant practitioners fail to investigate, indiscriminately using moistening/nourishing herbs, turning false symptoms into reality. Seeking the root is the essential principle of medicine. Now, only clear and nourish Lung and Stomach.
Ophiopogon (Maidong, 2 qian), Poria (Fuling, 2 qian), Coix Seed (Yiren, 3 qian), Red Tangerine Peel (Juhong, 0.8 fen), Apricot Kernel (Xingren, 2 qian, crushed), Glehnia (Beishashen, 2 qian), Forsythia (Lianqiao, 1.5 qian), Licorice (Gancao, 0.4 fen).
Patient Xu (45 years old), Ying-Qi injured by labor, newly affected by Cold. Fever, panting, hemoptysis. Condition is critical. Tentatively use exterior-releasing method to rescue interior rebellious Qi.
Fermented Soybean (Xiangchi, 3 qian), Perilla (Zisu, 1.5 qian), Apricot Kernel (Xingren, 3 qian), Peucedanum (Qianhu, 1.5 qian), Platycodon (Jiegeng, 1 qian), Tangerine Peel (Jupi, 1 qian), Moutan Cortex (Danpi, 1.5 qian), Forsythia (Lianqiao, 1.5 qian), Licorice (Gancao, 0.4 fen), Ginger Skin (Jiangpi, 0.4 fen), Shallot Head (Xicongtou, 5 pieces).
After two doses, heat eased, cough reduced, but hemoptysis persisted. Pulse left fine/soft, right floating/wiry/rapid. Tongue coating pale yellow. Pulse/symptoms indicate tonification is suitable, but due to right floating/wiry pulse, Wind pathogen remains. Adding one or two exterior-regulating herbs is prudent.
Codonopsis (Dangshen), Coix Seed (Yiren), Rehmannia (Shengdi), Charred Angelica (Guishentan), Ophiopogon (Maidong), Poria (Fuling), Licorice (Gancao), Tangerine Peel (Jupi), Pinellia (Banxia), Apricot Kernel (Xingren), Mint (Bohe).
After three doses, remove Xingren and Bohe, add Yuzhu, Huangqi, Zhigancao. After more than ten doses, recovery ensued.
Cough and Hemoptysis
Patient Chen (43 years old), cough and hemoptysis, sputum thick or thin. Sensation of Qi rising in the left abdomen, worse when lying down. Complexion pale, aversion to wind, pulse soft and slightly rapid. Previous physician first used Xingren and Bohe to disperse/descend Lung Qi, worsening the cough. Others attributed it to Dry-Fire insulting Metal, administering American Ginseng (Yangshen) and Maidong, along with heavy tonics, which greatly reduced the condition. I consulted a physician, believing this is Earth deficiency failing to generate Metal, Metal deficiency failing to control Wood, leading to Liver Qi rebellion, Stomach insulted by Wood, transport dysfunction, food failing to transform into essence, becoming phlegm-drool. Turbid Qi steams upward, condenses and ascends. As the Lung is the Canopy, how can it not be diseased? Thus, dispersing consumes Metal further, cooling/moistening empties the Mother. Treatment must nourish the source of transformation, pacify the overcoming element, to be effective.
Codonopsis (Dangshen, 3 qian, originally intended for Ren Shen but too expensive, substituted), Atractylodes Macrocephala (Yuzhu, 2 qian), Poria (Fuling, 1.5 qian), Honey-fried Licorice (Zhigancao, 0.4 fen), Tangerine Peel (Jupi, 1 qian), Pinellia (Banxia, 1.5 qian), Achyranthes (Niuxi, 1.5 qian), Tetsuka Herb (Tongcao, 0.7 fen), Moutan Cortex (Danpi, 1.5 qian), Mulberry Leaf (Sangye, 1 qian).
After ten doses, illness removed by more than half. Continue with Rensheng Shengmai San for three doses, then resume previous formula, removing Sangye and Danpi, adding Rougui, Huangqi, Yiren. Recovered.
Commentary: Improper treatment of this syndrome leads to daily muscle wasting and increasing phlegm. Within months, critical danger arrives. Hence, detailed discussion of pathology allows practitioners to grasp the boundaries of treatment.
Patient Cai (34 years old), chest distension, panting, cough, hemoptysis. Pulse large and rapid. Ancients say: Large pulse indicates exhaustion, rapid pulse indicates deficiency. Syndrome caused by labor injuring Spleen Yuan, Earth failing to generate Metal, Lung losing clear descent. Treatment should nourish the source of transformation.
Codonopsis (Dangshen, 2 qian), Astragalus (Huangqi, 3 qian), Atractylodes Macrocephala (Yuzhu, 1.5 qian), Angelica (Danggui, 1.5 qian, stir-fried charred), Tangerine Peel (Jupi, 1 qian), Ophiopogon (Maidong, 2 qian), Coix Seed (Yiren, 2 qian), Schisandra (Wuweizi, 10 grains), Honey-fried Licorice (Zhigancao, 0.4 fen).
This formula is my original creation, named Peiyuan Yifei Tang (Nourish Source Benefit Lung Decoction), performing miracles in treating labor-induced hemoptysis.
Patient Cao (55 years old), aversion to cold, cough, hemoptysis. Both pulses wiry and soft. This is labor-fatigue injuring Spleen, accumulated Cold injuring Lung. Treatment should warm-tonify Hand and Foot Taiyin (Lung/Spleen), slightly assisting in dispersing exogenous pathogens.
Codonopsis (Dangshen), Steamed Atractylodes (Zheng Yuzhu), Poria (Fuling), Tangerine Peel (Jupi), Peucedanum (Qianhu), Angelica (Guishen), Coix Seed (Yiren), Cinnamon Twig Wood (Guizhimu), Perilla (Zisu), Honey-fried Licorice (Zhigancao), Baked Ginger (Weijiang), Jujube (Dazao).
After two doses, bleeding stopped, cough reduced.
Patient Wu (21 years old), previous administration of muscle-releasing herbs resulted in sweating and heat reduction, but hemoptysis persisted. Investigating the cause: severe Cold pathogen injury, Lung Qi unsmooth, leading to reckless blood movement. Tentatively again disperse and promote Yuanfu (pores) to resolve interior rebellion.
Apricot Kernel (Xingren), Peucedanum (Qianhu), Perilla Leaf (Suye), Saposhnikovia (Fangfeng), Tangerine Peel (Jupi), Gypsum (Shigao), Licorice (Gancao).
This formula substitutes Qinglong Tang. After two doses, cough eased, hemoptysis stopped, chest widened, appetite increased, vitality returned.
Patient Zhao (20 years old), night fever, night sweats, cough with red sputum. Pulse wiry and rapid. Syndrome belongs to Labor Consumption (Lao Que). Naturally requires protecting Zheng Qi, assisting with herbs, hoping to extend lifespan.
Glehnia (Beishashen), Ophiopogon (Maidong), Poria Spirit (Fushen), Coix Seed (Yiren), Achyranthes (Niuxi), White Peony (Baishao), Mulberry Leaf (Sangye), Uncaria (Gouteng), Imperata Rhizome (Maogen).
Previous treatment with Clearing Metal Harmonizing Liver methods cured night fever/sweats; now treat only cough. Investigating the source, it is labor-induced. Use Tonifying Earth Generating Metal method, implying nourishing the source of transformation.
Codonopsis (Dangshen), Steamed Atractylodes (Zheng Yuzhu), Poria (Fuling), Pinellia (Banxia), Schisandra (Wuweizi), Ophiopogon (Maidong), Coix Seed (Yiren), Tangerine Peel (Jupi), Honey-fried Licorice (Zhigancao), Imperata Rhizome (Maogen).
Ancients state that using Yigong San to conclude cough treatment prevents recurrence, known as Tonifying Earth to Generate Metal.
Ms. Yao (24 years old), cough started last winter, extending to February, then spitting offensive-smelling red sputum. Pulse extremely fine and rapid, spontaneous sweating. Repeatedly changing doctors, all diagnosed Yin Deficiency, administering Siwu Tang and Liuwei Dihuang Wan. Later doctor diagnosed Lung Abscess. Now seeking specialist care. Family knows I can treat difficult cases and invited me. Observing pulse/symptoms: if Yin Deficiency, there must be dryness, how can there be sweat? Internal abscess in hypochondrium must hurt, pulse must be large and flooding, neither present. In my view, it is Lung affected by exogenous pathogen. This organ is most delicate; chronic cough damages membranes, causing red sputum. Furthermore, Earth generates Metal, Child steals Mother Qi. Offensive sputum belongs to Spleen deficiency. Observe worldly foul matter covered by earth overnight; its odor dissipates quickly. Treatment must address branch then root. First disperse Lung pathogen.
Apricot Kernel (Xingren), Mint (Bohe), Saposhnikovia (Fangfeng), Red Tangerine Peel (Juhong), Platycodon (Jiegeng), Mulberry Bark (Sangpi), Forsythia (Lianqiao), Licorice (Gancao).
After two doses, cough significantly reduced. Switch to Tonifying Earth Generating Metal method, slightly benefiting Lung. Liujunzi Tang added with Yiren, Bian Dou, Shanyao, Xingren, Qianhu. After four doses, less sputum, foul odor gone, cough cured. Original formula remove last five items, add Maidong, Danggui, Dihuang, regulate and restore Yuan.
Miss Pu (11 years old), chronic cough injuring Lung, sputum contains blood. Left pulse wiry and rapid. Liver Fire insulting Metal.
Moutan Cortex (Danpi), Mulberry Leaf (Sangye), Uncaria (Gouteng), White Peony (Baishao), Coix Seed (Yiren), Poria Spirit (Fushen), Apricot Kernel (Xingren), Red Tangerine Peel (Juhong), Ophiopogon (Maidong), Licorice (Gancao), Glehnia (Shashen).
After four doses, halved. Changed method and recovered.
