The pulse at the Cun Kou (Cun position of the radial artery) is deep and slow, indicating cold vomiting; the pulse is floating, large, and rapid, indicating heat vomiting; the pulse is minute and fine with aphasia indicates a difficult-to-treat condition. When infants vomit immediately after breastfeeding, if accompanied by a chaotic pulse, there is no need for excessive worry.
Infantile vomiting is diverse and difficult to enumerate comprehensively. It includes cold vomiting, heat vomiting, food stagnation vomiting, and milk-injury vomiting. Zhu Fengyi advocated using Banxia (Pinelliae Rhizoma) and Shengjiang (Zingiberis Rhizoma Recens) as main herbs, but also distinguished between food stagnation in the Heart-Lung region, where pathogenic factors and food fail to descend and instead regurgitate. Zheng stated: Vomiting without diarrhea is termed Ni Tu (Rebellious Vomiting); if accompanied by phlegm inducing fright convulsions, the prognosis is critical. Treatment should involve weaning and feeding only thin rice gruel. Milk-injury vomiting occurs when vomiting follows immediately after breastfeeding, caused by unregulated feeding and weak Spleen Qi failing to transform and transport. Just as a small vessel overflows when full, breastfeeding must be regulated. If an infant vomits milk with body heat and the vomitus appears in clumps, this indicates Stomach Heat; accumulated heat may generate Wind. If the vomited milk is undigested, mixed with clear fluid, and accompanied by a cyan face and white lips, this indicates Stomach Cold. Vomiting due to consuming raw/cold foods or damaging stored milk, resulting in failure to retain food, requires warming the Stomach, dispelling wind-cold, and eliminating old cold. Li Zhong Tang (Regulate the Middle Decoction) and Ding Tu Yin (Stop Vomiting Drink) are appropriate. If other medicines are ineffective, Shen Xiang Yin (Ginseng and Aquilaria Drink) should be used. In cases of cold vomiting, common in winter due to wind-cold invasion, the mother may have contracted cold and transmitted it through breastfeeding, allowing cold qi to enter the infant's Stomach. Symptoms include vomiting preferring warmth, aversion to cold, and cold limbs. Cold vomiting必然 involves vomiting clear saliva. Treatment involves warm middle-jiao herbs, such as Dingxiang Wan (Clove Pill), Huoxiang Zhengqi San (Agastache Powder to Rectify the Qi) added with Dingxiang (Caryophylli Flos), Sizijunzi Tang (Four Gentlemen Decoction) added with Dingxiang, Juhong (Citri Reticulatae Pericarpium), Ganjiang (Zingiberis Rhizoma), Paoshengjiang (Prepared Zingiberis Rhizoma), and Jujubae Fructus (with pits removed) decocted together, taken before meals. For extreme cold, Fuzi (Aconiti Lateralis Radix Praeparata) is added to Li Zhong Tang (prepared, peeled, tip and navel removed), dosed according to deficiency or excess. Heat vomiting presents with a red face and red lips, infrequent but voluminous vomiting, dissipated yellowish milk clumps, severe generalized heat, or heat accumulation in the Stomach from summerheat or consumption of hot-natured foods, leading to mental discomfort and irritability. This is heat vomiting. Wuling San (Five-Ingredient Powder with Poria) minus Guizhi (Cinnamomi Ramulus) and Xiangru Yin (Elsholtzia Drink) are indicated. Furthermore, heat vomiting may resemble fright vomiting, characterized by yellow phlegm mixed with milk-food clumps, warmth on the forehead, five-palm heat, scanty dark urine, or dry heaving without substance, commonly seen in summer and autumn. Wuling San and Erchen Tang (Two-Aged Decoction) are used for treatment.
