This sore is caused by damp-heat pathogenic factors and is the most severe among all sores. Diagnosis requires distinguishing between exterior/interior, deficiency/excess conditions. Initially, dispersing methods should be employed to attack and eliminate pathogenic toxins; subsequently, herbs that harmonize Qi and Blood are used to prevent recurrence. It is named "Yangmei Chuang" (Meadow Breadfruit Sore) because the shape of the sore is elevated and resembles a meadow breadfruit. In the North, it is called "Tian Chuang," which is incorrect.
For patients with Exterior Excess syndrome, it is appropriate to first release the exterior. (Jingfang Baidu San / Jing-Jie and Fang-Feng Decoction for Overcoming Toxicity.) For those with predominant Dampness, it is appropriate to first guide out the Dampness. (Qianghuo Cangzhu Shengshi Tang / Notopterygium and Atractylodes Decoction for Overcoming Dampness, Wuling San / Five-Ingredient Poria Decoction.)
For patients with Interior Excess syndrome, it is appropriate to first purge the interior. (Neishu Huanglian Tang / Internal Purging Coptis Decoction.) If both Exterior and Interior are in an Excess state, it is appropriate to simultaneously release the exterior and purge the interior. (Fangfeng Tongsheng San / Saposhnikovia Universal Relief Powder.) Initially, treatment must consider the patient's constitution (deficiency or excess) to determine the dosage intensity; after releasing the exterior and purging the interior, one should then regulate Qi and Blood, tonify Yuan Qi, and strictly avoid rich and greasy foods.
In cases of this sore, taking Calomelas (Qingfen) or Mercury (Shuiyin) tends to trap the toxin internally, leading to recurrent outbreaks; frequent use of Fangfeng Tongsheng San and other detoxifying agents further depletes Qi and Blood, resulting in many cases of physical deterioration.
(From *Secret Formulas*) Xian Yiliang Tang: Treats all types of Yangmei Chuang, effective regardless of duration or constitutional status (deficiency/excess).
Xian Yiliang (Two taels. Also known as Tu Fuling [Smilax Glabra Rhizome], also known as Lengfantuan. Use fresh rhizomes, clean off soil thoroughly, crush in a wooden or stone mortar. Add three bowls of water, decoct until two bowls remain, strain dregs, then add the following ingredients): Danggui (Angelicae Sinensis Radix), Sheng Dihuang (Rehmanniae Radix Recens), Fangfeng (Saposhnikoviae Radix) (each 8 fen); Jinyinhua (Lonicerae Japonicae Flos) (1 qian); Zaogao Ci (Gleditsiae Spina) (6 fen); Huanglian (Coptidis Rhizoma), Lianqiao (Forsythiae Fructus) (each 1 qian); Gancao (Glycyrrhizae Radix et Rhizoma) (4 fen); Mutong (Akebiae Caulis), Yiyiren (Coicis Semen) (each 8 fen); Baizhu (Atractylodis Macrocephalae Rhizoma), Baixianpi (Dictamni Cortex) (each 7 fen).
Chop these ingredients into pieces for one dose. Take two bowls of the prepared Xian Yiliang decoction, add twenty stalks of Dengxincao (Junci Medulla), and decoct until one bowl remains. Take after meals.
Rapid Method: Effective for Yangmei Chuang whether new or chronic, as well for carbuncles/toxins and E Zhang Feng (Goose Palm Wind/Eczema of hands). Cure within ten days.
Fulonggan (Dragon's House Liver/Fired Earth) (2 qian), Shuiyin (Mercury) (1 qian), Kufan (Alum Calcined) (3 fen), Baixianpi (Dictamni Cortex) (7 fen), Baicaoshuang (Soot from Hundred Herbs) (1 fen).
Grind into an extremely fine powder. Mix with a small amount of Sesame Oil (Xiangyou) to form a paste, ensuring no visible mercury particles remain. In a windless place, apply a small amount of paste to the palms and soles, rubbing them against each other. Apply again and rub until the medicine is exhausted. After application, drink one to two bowls of hot Scallion Soup to induce sweating. Cover heavily with cotton quilts to promote sweating. After sweating, take one dose of Jiedu San (Detoxification Powder) or Neishu Huanglian Tang. Repeat every three days for three times over nine days. By the twelfth day, gradual recovery occurs, and recurrence ceases. If recovery is slow, repeat every five days with induced sweating. After cure, Banxia Bazhen Tang (Bistorta Eight-Tonifying Decoction) may be taken. If heat signs are present, add Huangqin (Scutellariae Radix) and Huanglian (Coptidis Rhizoma).
Fumigation Method: Xionghuang (Realgar), Yinzhu (Vermilion), Moyao (Myrrha), Zhusha (Cinnabar), Guanxiang (Official Fragrance/Musk substitute), Zaorou (Date Flesh - two dates, mashed).
Grind the first five ingredients into fine powder. Mix with mashed date flesh, roll into small strips, wrap tightly in rice paper. Ignite using a sesame oil lamp for fumigation. Place under blankets. Keep water in the mouth. Wrap the scrotum with cloth to protect from smoke. Perform once every other day. Complete three strips for a cure. Subsequently, continue taking Sanhuang Jiedu Tang (Three-Yellow Detoxification Decoction).
Yangmei Yonglou (Meadow Breadfruit Carbuncle and Fistula): Effective regardless of disease duration.
Tu Bixie (Smilacis Glabrae Rhizoma) (Five taels), Jinyinhua (Lonicerae Japonicae Flos) (One tael), Zaogao Ci (Gleditsiae Spina), Huajiao (Zanthoxylli Pericarpium), Niubangzi (Arctii Fructus), Yujin (Curcumae Longae Radix), Danggui (Angelicae Sinensis Radix) (each half tael), Heiqian (Black Lead) (Melted three taels, mixed with half tael of Shuiyin/Mercury).
Chop ingredients into pieces for five doses. Grind the black lead while hot into powder, dividing it into five portions. For each dose, decoct the herbs with two cups of water and one scallion root until one cup remains. Strain dregs, then add the lead powder and decoct until eight-tenths of a cup remains. Take after meals. Wash the lead powder with water before storage. This is because Yangmei Yonglou often results from accumulated toxicity due to Calomelas ingestion. Here, Mercury, Black Lead, and Zanthoxyllum are used to re-absorb and guide out the Calomelas toxicity, based on the principle of attracting like substances. Each course consists of five doses. The first dose induces sweating: decoct Jinyinhua (One tael), Fangfeng (Half tael), and Huajiao (Half tael) in two dou of water. Surround with mats to prevent drafts. Fumigate first, then wash, inducing sweat. If Jinyinhua is unavailable, Ren Dong Teng Ye (Honeysuckle Vine and Leaves) is superior. For chronic cases lasting 20-30 years, four courses (four sweat inductions) ensure complete cure. Avoid beef and distilled spirits. This is a miraculous formula, kept secret by others. I obtained it, treated patients with success, and thus record it in detail to share with the world, aiming to save the critically ill and benefit humanity.
(Wan Shi Formula): For Yangmei Chuang causing bone and joint pain, chronic and unhealed, providing immediate effect.
Tu Fuling (Smilacis Glabrae Rhizoma) (One tael), Huajiao (Zanthoxylli Pericarpium) (2 qian), Huaizi (Sophorae Fructus, stir-fried until charred) (2 qian), Yubiao (Fish Maw, fried until puffed) (3 qian), Mahuang (Ephedrae Herba), Qiancao (Rubiae Cordifoliae Radix), Xi Cha (Fine Tea) (each 1 qian), Wuyao (Linderae Radix), Ruxiang (Olibanum) (each 1 qian).
Add two cups of water, three slices of Ginger, three scallions. Decoct until one cup remains. Take orally in one go. For severe cases, several doses will bring a cure.
