The Stomach governs the generation of vital Qi, and the Spleen governs transportation and transformation. When the Spleen and Stomach are deficient, the Qi from grain and food fails to fill the body, leaving the Spleen further deprived of its source. The Spleen distributes Qi to the four limbs; since the received Qi is deficient, the four limbs become fatigued and weak, unable to move, thus resulting in lassitude and a desire to sleep. There are also cases where excessive eating injures the Spleen, making it difficult for the Spleen to transform and transport; after every full meal, one feels sleepy and drowsy. This condition occurs temporarily after eating and resolves after digestion. For such cases, one should tonify the Spleen and Stomach while simultaneously employing methods to promote digestion and resolve stagnation. If both the Spleen and Stomach are deficient, with reduced food intake, decline in form and Qi, and constant fatigue, one must strongly tonify the Spleen and Stomach to nourish their source of transformation; at this time, purgative or digestive agents should not be used lightly or rashly. After prolonged tonification, once the Spleen and Stomach become robust, spirit and energy will naturally suffice, and there will be no fatigue.
Danxi stated: In cases where dampness invades the Spleen and Stomach, causing heaviness, weakness, lassitude, and somnolence, formulas such as Er Chen Tang (Two-Cured Decoction), Cang Zhu (Atractylodis Rhizoma), Bai Zhu (Atractylodis Macrocephalae Rhizoma), and Liu Jun Zi Tang (Six-Gentlemen Decoction) should be used as the main prescription. If deficiency is inherent, formulas like Sheng Yang Chu Shi Tang (Raise Yang and Dispel Dampness Decoction) may be employed.
In obese individuals, lassitude is often due to Qi deficiency accompanied by phlegm; Si Jun Zi Tang (Four-Gentlemen Decoction), Er Chen Tang (Two-Cured Decoction) added with Cang Zhu (Atractylodis Rhizoma), and Yi Wei Tang (Benefit the Stomach Decoction) are suitable. In thin individuals, it is often due to heat; Huang Qin (Scutellariae Radix) and Bai Zhu (Atractylodis Macrocephalae Rhizoma) are suitable. For those whose excessive diet leads to inadequate transportation and transformation, Jia Wei Bao He Wan (Modified Preserve Harmony Pill) and Zhong He Tang (Harmonize the Middle Decoction) are appropriate.
A pulse that is deep, faint, and forceless indicates deficiency-related lassitude. A fine and soggy pulse at the Right Guan position indicates dampness. A wiry pulse at the Right Guan position indicates food retention; a slippery and large pulse indicates lethargy due to phlegm-fire.
