The "Five Flaccidities" syndrome, also known as fetal deficiency (Tai Qie), is primarily caused by insufficient seminal essence of the father and declining qi and blood of the mother. It may result from conception when the mother's qi and blood are weak; from maternal illness during pregnancy; from excessive sexual indulgence by parents leading to physical weakness; from advanced parental age at conception; from prematurity (insufficient gestational months); or from surviving an attempted abortion with abortifacient herbs. These factors deplete true qi. After birth, the essence and qi are not充盈 (full), leading to flaccid sinews and bones, emaciated muscles, dull spirit and slow demeanor, resulting in weakness of the head, neck, hands, feet, and body, termed the "Five Flaccidities." The verse states: Innate qi is deficient and marrow is unfulfilled, making one susceptible to attack by the Six Exogenous Pathogens (Liu Yin). Weakness of the head, hands, feet, and body characterizes this syndrome, collectively named Five Flaccidities.
The "Five Stiffnesses" syndrome presents with a stiff neck unable to look down, qi stagnation in the chest and diaphragm, and cold, hard soles of the hands and feet, termed the "Five Stiffnesses." Another presentation involves a distended abdomen with visible blue veins and lack of flexibility, for which accumulation-resolving formulas are appropriate; hardness in the heart and abdomen also falls under this category.
