The *Bingji* (Pathogenesis) chapter states: For traumatic tetanus (Po Shang Feng), some cases arise from sudden exposure to wind-pathogen, which invades and spreads through the meridians, causing alternating chills and fever, opisthotonos (body arching backward), and trismus (lockjaw). In severe cases, if the pathogen enters the zang-organs, death ensues. Other cases arise from chronic non-healing sores; when Ying and Wei qi are deficient and fail to generate muscle tissue, leaving open wounds, wind-pathogens can also invade externally, resulting in traumatic tetanus. Both of these are external causes. Regarding chronic sores that do not heal, the world generally considers moxibustion as the superior treatment, termed "heat sores." However, they are unaware that heat-fire toxic pathogens residing within can transform diseases along the meridians, a multitude of conditions too numerous to recount. Mildly, it causes fever; severely, it generates wind and convulsions. Or it presents as opisthotonos, trismus, and strabismus. This belongs to the category of neither internal nor external causes. There are also cases of traumatic tetanus occurring without prior moxibustion. Because the sore is covered by a white scab, closing the wound, qi fails to circulate and vent freely, leading to yang-heat stagnation. Extreme heat generates wind. Or, even when the condition seems fully stabilized, this disease suddenly appears. All such cases result from deficiency of internal qi, with wind-heat stagnating in the five viscera and six fu-organs. These all belong to internal causes.
