Pulse
A thin and fine pulse in children indicates Gan-Lao (Gan disease with consumption). A white color of the pulse pattern on the Tiger's Mouth (Hukou, located at the radial side of the first web space) indicates Gan disease.
Disease
The various types of Gan disease are caused by the consumption of rich, sweet, and fatty foods. Since children have weak spleen and stomach functions, they are often overindulged due to maternal affection, without proper care methods. This leads to excessive eating of sweet, greasy, raw fruits, cold items, and all kinds of cooked and seasoned foods. Consumed morning and night, these gradually form stagnation. It is rare for such accumulated and fixed food not to develop into Gan disease. Alternatively, infants lacking breast milk may be fed porridge too early, damaging their physical form and Qi. Furthermore, prolonged suffering from malaria, dysentery, vomiting, or diarrhea can lead to various diseases; if mismanaged with purgative medicines, it damages the spleen and stomach, depletes body fluids, and results in symptoms such as fever, emaciation, sallow complexion, abdominal distension, visible subcutaneous veins, worm pain, diarrhea, and dysentery.
Treatment
The treatment should regulate the spleen and stomach, eliminate stagnation, resolve parasites, clear heat, and stop diarrhea and dysentery. Fei'er Wan (Fetus-Nourishing Pill) and Ganji Bing (Gan Stagnation Cake) are the primary formulas. These two formulas are most effective for all types of Gan disease regardless of cold or heat patterns. Generally, Gan disease arises from overeating, causing accumulation in the Spleen. If untreated, it transmits to other Zang organs, resulting in the Five Gan diseases. What are the Five Gan? They correspond to the Heart, Liver, Spleen, Lung, and Kidney.
If Gan is in the Heart: red face, dry mouth, teeth grinding, tongue protrusion, sores in the mouth and tongue, fever, and emaciation. Treat with Anshen Wan (Calming the Spirit Pill).
If Gan is in the Liver: blue-green face, tendons covering the eyes, head shaking, eye rubbing with excessive tearing, dry and upright hair, cyanotic tendons, brain heat, and weakness. Treat with Bugan Tang (Tonify Liver Decoction).
If Gan is in the Spleen: yellow face, fever, abdominal distension and enlargement, craving for eating soil, indigestion of water and grain, sour-smelling stool, lethargy, reduced appetite, and muscle wasting. Treat with Yihuang San (Yellow-Enriching Powder).
If Gan is in the Lung: pale face, cough, wheezing and reverse flow of Qi, sores in the mouth and nose, throat discomfort, high fever with aversion to cold, and runny clear nose. Treat with Qingfei Tang (Clear Lung Decoction).
If Gan is in the Kidney: black face, extreme muscle wasting, skin sores, fever, painful/difficult urination, cold hands and feet, bad breath, and dry thirst. Treat with Dihuang Wan (Rehmannia Pill).
Internal Gan presents with swollen eyes, variable stool color, or clear foamy stool, progressing to emaciation; this is a Cold pattern, treated with Muxiang Wan (Costus Pill).
External Gan presents with redness and erosion under the nose, self-rubbing of the nose, non-healing sores around the eyes. Treat with Lanxiang San (Perilla Leaf Powder).
Generally, Gan disease must be differentiated by cold/heat and fat/emaciation. Early-stage disease is Fat-Heat Gan, while chronic disease is Thin-Cold Gan. Cold patterns use Muxiang Wan; Heat patterns use Huanglian Wan. Clinical judgment is essential.
