The patient has historically suffered from Fanwei (regurgitation and vomiting of undigested food), which was originally manageable. However, in autumn, a sudden disturbance caused Jueyang (rebellious Yang qi from the lower jiao) to surge uncontrollably. Consequently, the patient ceased eating and had bowel obstruction, accompanied by persistent Xiaoke (wasting-thirst syndrome). If there is heart heat with vomiting of saliva, and a preference for sour and sweet flavors among the five tastes, it indicates that Liver Yin and Stomach fluids have been exhausted. It is difficult to tolerate drying herbs to open the orifices. The Stomach belongs to Yang Earth and should be treated with cooling and moistening methods; the Liver is a rigid organ and should be treated with softening and harmonizing methods. Therefore, sour and sweet flavors are used together to nourish Yin.
Wumei Meat (Fructus Mume), Renshen (Radix Ginseng), Xian Shengdi (Radix Rehmanniae Recens), Ejiao (Colla Corii Asini), Maidong Juice (Jus Radicis Ophiopogonis), Sheng Baishao (Radix Paeoniae Alba Cruda).
In the elderly, Blood and Qi gradually decline. They must rely on smooth bowel movements every few days so that food can enter the epigastrium without obstruction. This indicates that Stomach fluids are gradually drying up, and the mechanism of Stomach Qi descending is diminished; signs of Ye disease (dysphagia) are emerging. The illness results from excessive overwork, causing the Three Yang meridians to burn dry and scorch the body's fluids. Thus, medication has not achieved complete efficacy. It is proposed to use the method of Danxi.
Maidong Juice (Jus Radicis Ophiopogonis), Xian Shengdi Juice (Jus Radicis Rehmanniae Recens), Baiziren Juice (Jus Semen Platycladi), Tian Xingren Juice (Jus Semen Armeniacae Amarum), Hei Zhima Juice (Jus Sesami Nigris), Tusuzi Juice (Jus Fructus Lepidii et Descurainiae), Songziren Paste (Pasta Seminis Platycladi).
Due to mental and physical exertion, one naturally declines with age. For thirty years, the patient has taken Guizhi (Ramulus Cinnamomi), Fuzi (Radix Aconiti Lateralis Preparata), and Shengjiang (Rhizoma Zingiberis Recens). Now suffering from Ye disease where food cannot be swallowed and is vomited out from the diaphragm. This is because Qi in the Upper Jiao fails to transform, and body fluids fail to descend and irrigate downwards. Initially, bowel movements were hard and dry. According to the classics, excessive consumption of pungent and hot flavors leads to excess Liver Yang, depriving Lung Jin and Stomach Ye, resulting in Upper Jiao Dryness. Following Jiayan’s Qingzao (Clearing Dryness) method:
Maidong (Herba Ophiopogonis), Mahuang (not applicable here, likely Hemp Seed context implies Huren/Maren - text says Ma Ren, so Hemp Seed), Xian Shengdi (Radix Rehmanniae Recens), Tianshui Li (Sweet Pear), Sangye (Folium Mori), Shigao (Gypsum Fibrosum), Sheng Gancao (Radix Glycyrrhizae Preparata). *Correction: Text says Ma Ren (Hemp Seed).* -> Maidong (Herba Ophiopogonis), Huren (Semen Cannabis), Xian Shengdi (Radix Rehmanniae Recens), Tianshui Li (Pyrus bretschneideri var. culta), Sangye (Folium Mori), Shigao (Gypsum Fibrosum), Sheng Gancao (Radix Glycyrrhizae Preparata).
The pulse is wiry, small, and choppy. Food causes pain and rejection in the epigastrium upon entry, followed by vomiting of clear saliva before food can be taken again. The complexion is sallow, sclera veins are red and yellow; the patient was formerly fat but is now thin, attributed to injury from depression and anger. The Shenhuo (mineral fire/physiological fire) has transformed into Zhuanghuo (excessive pathological fire). Qi stagnation and phlegm accumulation increase daily, preventing the ascent of Clear Yang. The esophageal passage becomes narrow and constricted, unable to accept food, leading to Yige (dysphagia and regurgitation). The treatment method should employ bitter herbs to descend rebellious Qi and pungent herbs to unblock stagnation, assisted by herbs that benefit phlegm and clear the diaphragm. Do not use Doukou (Fructus Cardamomi) and Chenxiang (Lignum Aquilariae Resinatum) which deplete fluids.
Chuan Huanglian (Rhizoma Coptidis), Xingren (Semen Armeniacae Amarum), Jiegeng (Radix Platycodonis), Tu Gualou Pi (Pericarpium Trichosanthis), Banxia (Rhizoma Pinelliae Praeparatum), Juhong (Exocarpium Citri Reticulatae Rubrum), Zhuli (Nodus Bambusae in Taedia), Jiangzhi (Jus Zingiberis Recens).
