In previous cases of epistaxis (nosebleed), recovery was mostly achieved by nourishing Yin and replenishing Qi. This indicates that it was not a excess-heat syndrome, but rather the movement of Yin blood caused by stomach injury due to overwork.
Currently, this young adult patient has insufficient muscle mass, experiences shortness of breath resembling asthma upon slight movement, and produces sticky, turbid saliva in the mouth. These are signs of kidney essence damage and failure of reception and storage functions. Urgent conservation is required; strict abstinence from alcohol and sexual indulgence will allow him to approach stability.
Formula: Rehmanniae Radix Preparata (Shudi), Ginseng Radix (Renshen), Corni Fructus (Yurou), Nelumbinis Semen (Hulian), Euryales Semen (Qianshi), Psoraleae Fructus (Buguzhi), Dioscoreae Rhizoma (Shanyao), made into powder pills.
Walking and moving excessively causes Yang Qi to float upward; alcohol generates dampness which easily transforms into heat. Rising heat invades and obscures the clear orifices (head and facial apertures), causing head heaviness, distension, and profuse nosebleeds, leading to upper ulcers that may become fistulas if prolonged. The common secular prescription often uses Xijiao Dihuang Tang (Rhino Horn and Rehmannia Decoction). However, alcohol nature first enters the Gallbladder channel, then the Stomach. Alcohol drinkers typically dislike sweet and cloying substances. Therefore, bitter descending methods are adopted, as bitterness can dry dampness.
Formula: Mori Folium (Sangye), Kudingcha Tea (Kudingcha), Forsythiae Fructum (Lianqiao), Lotus Leaf Margin (Heyebian), Moutan Cortex (Danpi), Belamcandae Rhizoma (Shegan).
The patient reports bleeding during spring, with nosebleeds inevitably following fatigue. Generally, in winter, atmospheric Qi hides and stores; young adults should naturally gather essence and spirit. If conservation is neglected, when Yang Qi rises and expands in mid-spring, it becomes the root of disease. This is all due to thin body essence and Qi. If Stomach Qi is strong and content with receiving, and one can self-regulate diet and rest quietly, then Spirit will be stored.
Formula: Rehmanniae Radix Preparata (Shudi), Corni Fructus (Yurou), Dioscoreae Rhizoma (Shanyao), Euryales Semen (Qianshi), Xianglian (Xianglian), Poria (Fuling), Rubus Chingii Fructus (Jinyingzi), Schisandrae Fructus (Wuweizi), Long Os (Qinglonggu), made into steamed dough paste pills.
Pulse is deficient and choppy on the left, slow and large on the right. Pain at the tailbone (Weilu) radiates to the spine, with blood after defecation, accompanied by palpitations and near-syncope. Combined with minimal food intake, this clearly indicates mutual damage of Middle and Lower Jiao, and total deficiency of the Eight Extraordinary Vessels. In the morning, take Qingnang Banlong Wan to strongly tonify Yutang and Guanyuan acupoints. In the evening, take Guipi Gao to nourish Ying Yin. Persist for years, and the physical form will naturally solidify.
Formula: Deer Antler Glue (Lurong), Deer Antler Frost (Lu Jiaoshuang), Deer Antler Gelatin (Lu Jiaojiao), Platycladi Seed (Baiziren), Rehmanniae Radix Preparata (Shudi), Allii Tuberosi Fructus (Jiu Zi), Cuscutae Semen (Tusizi), Red and White Poria (Chi Bai Fuling), Psoraleae Fructus (Buguzhi).
Pulse is small. Hematochezia has persisted for twenty years. The Classic says: "When Yin collaterals are injured, blood overflows internally." For sixteen years since onset, conception has been impossible. The patient describes heart pain and bearing-down sensation with blood discharge, regardless of whether before or after stool. Additionally, there is soreness in the spine and sacrum, yet menstruation still occurs. The dorsum of the foot and knees are often cold, while the bone marrow feels burning hot. This results from Yin fluid damage affecting Yang's inability to secure and close. Reviewing years of medication, cooling Liver herbs were mixed in arbitrarily. How does this follow ancient treatment principles?
It is proposed to adopt a method of lifting and securing through the Extraordinary Vessels.
Formula: Deer Antler Glue (Lurong), Deer Antler Frost (Lu Jiaoshuang), Lycii Fructus (Gouqi), Angelicae Sinensis Radix (Guishen), Purple Quartz (Zishi), Astragali Complanati Semen (Shayuan), Eucommiae Cortex (Duzhong), Stir-fried Anise Fruit (Da Hui), Psoraleae Fructus (Buguzhi), Hematitum (Yuyuliangshi), made into steamed dough paste pills.
Abdominal pain and borborygmus occur at night; testicular ptosis has existed for a long time. Bleeding occurred in the first lunar month, with bright red blood throughout the month. This pain is definitely not caused by accumulation masses, but by Ying blood damage and cold invasion, where Liver Wood insults Spleen Earth, causing fourteen years of lingering illness. Nourishing and fortifying Earth should use Sweet flavors; draining Wood and dispersing stagnation should use Acrid flavors. Control diet and abstain from desire; efficacy can be seen in one hundred days.
Formula: Ginseng Radix (Renshen), Stir-fried Angelicae Sinensis Radix (Chao Danggui), Stir-fried Paeoniae Radix Alba (Chao Baishao), Cinnamomi Cortex (Rougui), Prepared Zingiberis Rhizoma (Pao Jiang), Poria (Fuling), Honey-fried Licorice Root (Zhigancao), Southern Jujube (Nanzao).
Pulse is small, left is rapid. Dry stool with lower bleeding indicates Liver collateral heat surging, causing blood to be restless. The physician administered Shen, Dang, Gui, etc., which are Sweet-Acrid and warm. Ignoring that Ministerial Fire resides in the Liver and Gallbladder, flame flies with wind, obscuring the clear orifices above, causing nasal congestion, dizziness, and incessant choking cough. One error after another leaves complications of Middle Jiao Jue. Lower deficiency leads to Upper excess; Yin damage leads to Yang floating. This is an absolute truth.
Formula: Lonicerae Flos Cor (Lianqiao Xin), Bambusae Folium Cor (Zhuye Xin), Fresh Rehmanniae Radix (Xian Shengdi), Scrophulariae Radix (Yuanshen), Moutan Cortex (Danpi), Dendrobii Caulis (Chuan Hu).
Second visit: Lower bleeding damages Yin and causes leakage, with deficient Yang rising. The head is the clear orifice; Shen, Zhu, Gui are Acrid-Sweet and assist Yang upwards, causing nasal congestion and deafness. After using clearing herbs for five to six days, the right pulse has become small, but the left remains fine and rapid. This is the original aspect of Yin deficiency. The more deficient the lower, the more excessive the upper. It is proposed to use a formula to nourish Water to control Fire.
Formula: Rehmanniae Radix (Shengdi), Scrophulariae Radix (Yuanshen), Asparagi Tuber (Tiandong), Dendrobii Caulis (Chuan Hu), Poria Cum Pinellia (Fushen), Stir-fried Achyranthis Bidentatae Radix (Chao Niuxi).
Last year, phlegm-fluid asthma prevented lying flat. Recovery was achieved using Acrid-Warm methods to unblock Yang and seize fluids. This reveals weak internal Spleen Yang and failed transport function. Steam from water and grain creates fluid pathogens from dampness. Dampness belongs to Yin; long-term stagnation transforms into heat. Heat entering collaterals causes blood to bleed spontaneously. However, the constitution remains Yang deficient. Most prescriptions for intestinal redness contain Bitter-Cold herbs, such as Zanglian Wan, which do not fit the constitution. Do not seek speed.
Formula: Raw Atractylodis Macrocephalae Rhizoma (Sheng Baizhu), Poria (Fuling), Alismatis Rhizoma (Zexie), Sophorae Flavescentis Radix Charred (Diyutan), Mori Folium (Sangye), Moutan Cortex (Danpi).
Pulse is slow, soggy, and weak. Insufficient Yang Qi. Excessive drinking leads to dampness dominance, loose slippery stools, seemingly uncontrollable. Blood after stool is red-purple, mixed with clots. Logically, this is Shaoyin Kidney failing to control retention, while Yangming Stomach channel only opens without closing. Historically, treating Fu organs uses "unblocking as tonification," which differs greatly from tonifying Zang organs. First, propose warming the Stomach and unblocking Yang.
Formula: Raw Atractylodis Rhizoma (Sheng Maozhu), Ginseng Radix (Renshen), Poria (Fuling), Citri Reticulatae Pericarpium (Xinhui Pi), Magnoliae Officinalis Cortex (N Houpu), Prepared Aconiti Lateralis Radix (Pao Fuzi), Prepared Zingiberis Rhizoma Charred (Pao Jiangtan), Sophorae Flavescentis Radix Charred (Diyutan).
Anger moves Liver Qi, causing collateral blood to descend. Pain relief upon pressure indicates deficiency. Liver Wood ascending must invade the Stomach gate, causing distension and vomiting. Clear Yang sinks, doors lose storage, leading to tenesmus and hematochezia. Shen, Zhu, Pao Jiang are Acrid-Sweet and warm, being Taiyin Spleen herbs. How can they harmonize Liver and Stomach? Danxi said: Ascending Qi originates from the Liver. Feeling cold is not true cold.
Formula: Zhuche Wan.
Alternating chills and fever like malaria, incessant hematochezia. Mass in left hypochondrium, attacking and reversing endlessly causing pain. Pulse is wiry, rapid, and choppy. Wiry indicates Wind, Rapid indicates Heat, Choppy indicates Qi knotting. This is Liver and Spleen Qi depressed and unexpressed. Yang Harmony in the chest is suppressed into Fire, so Spirit is unclear. Liver corresponds to Wind; Liver Qi movement brings Wind, hence surface manifestations of chills and fever. The left half of the body is governed by Liver and Kidney. Liver Wind rebels, hence left hypochondriac pain and mass. Liver stores blood; Liver injury means blood is not guarded. Plus Wind dominance and Heat excess, this is the cause of blood loss.
Formula: Raw Polygoni Multiflori Caulis (Sheng Shouwu), Coptidis Rhizoma (Huanglian), Bupleuri Radix (Chaihu), Scutellariae Radix (Huangqin), Anemarrhenae Rhizoma (Zhimu), Immature Bitter Orange (Zhishi), Magnoliae Officinalis Cortex (N Houpu).
Hematuria has lasted three months. Previous lifting and tonifying methods failed. Right pulse is choppy and spiritless; Left Guan pulse alone is wiry. Urethral pain. Large amount of blood clots, haggard appearance, frequent belching. Based on pulse and symptoms, this is Liver and Spleen accumulated heat. Liver heat makes Yin fire restless, causing spontaneous Yin blood movement. Blood is stored by the Liver, and Sanjiao Fire resides in the Liver. Hence hematuria and urethral pain. Spleen heat causes internal dampness obstruction, preventing vital Qi from extending. Spleen transforms damp earth, and Sanjiao Qi operates in the Spleen. Hence frequent belching and haggard appearance. Method should benefit Liver Yin so Fire calms; drain Spleen dampness so Qi harmonizes.
Formula: Rehmanniae Radix (Shengdi), Paeoniae Radix Alba (Baishao), Smilacis Glabrae Rhizoma (Bixie), Moutan Cortex (Danpi), Glycyrrhizae Radix et Rhizoma (Gancao), Plantaginis Semen (Cheqian).
Childhood hematochezia was due to dietary irregularity, heat steaming collaterals, causing gastrointestinal disease. Anal hemorrhoids are also caused by internal steaming of damp-heat. Severe heat fails to fill Yin fluids; Wind-Heat rises causing dry choking cough. Method should provide Sweet-Cold agents so Metal (Lung) and Water originate from the same source. Moreover, Lung heat transfers to Large Intestine; Kidney opens into the two yin openings.
Formula: Fresh Rehmanniae Radix (Xian Shengdi), Lycii Cortex (Digupi), Ophiopogonis Tuber (Maimendong), Lonicerae Japonicae Flos (Jinyinhua), Bean Skin (Dopi), Fat Anemarrhenae Rhizoma (Fei Zhimu).
Typhoid blood stasis involves pathogen entering interior. The Inner Classic states: When Yin collaterals are injured, blood overflows downward. Yin relates to Zang diseases; Yin Qi leaks from below, Yang Qi loses attachment and floats upward, causing confusion and madness. This is a collapse syndrome. Especially at the critical juncture of Major Snow. Reviewing medical records, today's medicine includes Qianghuo, Fangfeng, Changpu root. Previously, Chaihu and Fangfeng were taken repeatedly. Body Yin and Yang have been exhausted by these demonic obstacles. How could there be peace? Although urgently gathering scattered Qi, it may not be stable enough to retrieve.
Formula: Ginseng Radix (Renshen), Poria with Tuckahoe (Fushen), Hematitum (Yuyuliang), Chaenomelis Fructus (Mu Gua), Schisandrae Fructus (Wuweizi), Triticum Aestivum (Xiaomai).
