Fupian (Processed Aconite Root) 60g, Ganjiang (Dried Ginger) 15g, Congbai (Scallion White) 4 segments (with root hairs).
After ingestion, the measles rash had fully erupted and turned bright red and vibrant. After one more dose, the rash color gradually faded to gray, the pulse became calm, the body temperature normalized, appetite increased, and spirit recovered vigorously; the patient was cured suddenly and completely.
Commentary: The patient had a weak constitution, appeared listless and devoid of spirit during fever, the measles eruption was slow, and the rash color was not fresh. After taking Baitong Tang (White Connective Decoction) for one or two doses, the measles were able to erupt completely, leading to a safe recovery. Although this treatment method is rarely seen in measles formularies, since the measles failed to vent outward and presented signs of Yin excess and Yang deficiency, administering Baitong Tang to support the Yang Qi of the Heart and Kidney resulted in rapid efficacy. If cold-natured drugs were mistakenly administered again, the healthy Qi would become more deficient and Yang weaker, lacking the strength to push toxins out, causing them to attack inward, inevitably leading to exterior collapse (Yang collapse). Therefore, regardless of sand-rash, measles, or smallpox, once the disease condition becomes severe, one must comprehend the spirit of "treating diseases requires seeking the root" from the *Neijing · Yin Yang Ying Xiang Da Lun*. One must carefully differentiate Yin and Yang. When symptoms present Yin characteristics, Yang must be rescued; when symptoms present Yang characteristics, Yin must be rescued; only then is there hope for recovery.
Two cases of critical measles requiring Yang-supporting and counter-reversal treatment, and one case of favorable progression.
Tao XX, male, 32 years old, from Jiangxi, residing in Shanghai. He had four sons and one daughter. In March 1932, during a measles epidemic, he sent his eldest and second sons (aged 7 to 9) to Suzhou to avoid infection. Unexpectedly, after arriving, both contracted measles and died successively due to the mistaken administration of cold-natured medicines.
The third and fourth sons, aged approximately 2 to 4, also contracted measles in Shanghai and were treated at a certain Guangdong Hospital. When their condition became severe, the hospital diagnosed it as "Pneumonia" and requested my consultation at the hospital.
Both children lay in a small bed together. Although the measles had erupted, the fever persisted, accompanied by wheezing, coughing, and phlegm rales. Their mouths were full of saliva and phlegm flowing out constantly, and the medication history was unknown. When fed with yellow fruit water, it flowed back out of the mouth. The face was cyanotic and dark (external manifestation of Yin), the malar regions were red (leakage of deficient Yang), lips were cyanotic-purple, the fingerprint (pediatric pulse diagnosis site) was cyanotic-black extending past the Wind and Qi gates (two gates), the pulse was urgent (sign of extreme cold), stools were watery diarrhea (water-cold dampening earth, Wood evil attacking Earth), and milk/food intake ceased (deficient-cold in Stomach, loss of transport authority). The hospital considered the condition critical, having no other methods, and felt helpless. Upon examination, I immediately informed them that the condition was critical, constituting a syndrome of extreme cold in the Three Yin channels, with cold-phlegm obstructing internally and True Yang leaking externally, showing tendencies of internal wind stirring or Yang collapse. Urgent treatment should focus on supporting Yang and suppressing Yin, primarily warming and expelling cold-phlegm. If vomiting of cold-phlegm occurs after medication, it is a sign of disease removal. If fever subsides and wheezing stops, critical condition can turn to safety. If treatment is delayed or cold-natured drugs are used again, it will surely lead to fatal danger. Due to the death of the first two sons and the critical illness of the next two, they were panicked, begged for rescue measures, trusted me completely, and stated they would bear no resentment even if unsuccessful. Thus, I prescribed Sini Erchen Tang (Four Reversal Two-Aged Phlegm Decoction) plus Dingxiang (Clove) and Rougui (Cinnamon Bark), with a small addition of Mahuang (Ephedra) and Xixin (Asarum), increasing dosages, given to both children together (as their conditions were identical, they shared one dose).
Fupian (Processed Aconite Root) 100g, Ganjiang (Dried Ginger) 24g, Rougui (Cinnamon Bark) 10g (powdered, soaked in water and mixed in), Faxia (Prepared Pinellia) 10g, Guangpi (Aurantii Pericarpium) 6g, Fuling (Poria) 15g, Xixin (Asarum) 3g, Gongdingxiang (Public Clove) 6g, Zhi Marenrong (Honey-processed Ephedra Downy Part) 3g, Gancao (Licorice Root) 10g.
After taking this formula, both vomited about a bowl of saliva and phlegm, sweated profusely, and had watery diarrhea. On the second day's follow-up, fever had subsided by 70-80%, wheezing by 50-60%, oral saliva/phlegm reduced by 80-90%, and throat phlegm rales halved. They took slight amounts of milk/food. Following the original formula with increased dosage, removing Mahuang and Xixin, continued treatment.
Fupian (Processed Aconite Root) 130g, Ganjiang (Dried Ginger) 36g, Rougui (Cinnamon Bark) 10g (powdered, soaked in water and mixed in), [Chihong/Specific Herb] 6g, Fuling (Poria) 15g, Faxia (Prepared Pinellia) 10g, Gongdingxiang (Public Clove) 6g, Gancao (Licorice Root) 10g.
After the second formula, each vomited another bowl of saliva and phlegm. On the third day's follow-up, pulse was calm, body cool, wheezing stopped, diarrhea ceased, sleep and diet improved, cough reduced by 60-70%, and facial/fingerprint cyanosis receded. Following the original formula, removing Gongdingxiang, adding Xixin (Asarum), Wuweizi (Schisandra Berry), and Huangqi (Astragalus Root), taken for three consecutive doses, all diseases were cured.
His other daughter, over five years old, subsequently developed measles. Initially presenting with fever, clear nasal discharge, cough, vomiting, diarrhea, and teary eyes. Prescribed Guizhi Gegen Tang (Cinnamon Twig and Kudzu Root Decoction) plus Fangfeng (Saposhnikovia Root) and Bohe (Mint).
Guizhi (Cinnamon Twig) 10g, Hangshao (Hangzhou Peony Root) 10g, Gegen (Kudzu Root) 10g, Fangfeng (Saposhnikovia Root) 6g, Bohe (Mint) 6g, Gancao (Licorice Root) 6g, Shengjiang (Fresh Ginger) 10g, Dazao (Jujube Fruit) 2 pieces.
After taking the above formula, measles gradually erupted. For the second dose, Gegen was removed and Huangqi (Astragalus Root) 16g added; after ingestion, the body was thoroughly permeated. For the third dose, Huangqi Guizhi Wuwu Tang (Astragalus and Cinnamon Twig Five-Material Decoction) was used; after ingestion, rash faded, pulse calmed, body cooled, and recovery was peaceful.
Huangqi (Astragalus Root) 15g, Guizhi (Cinnamon Twig) 10g, Shengjiang (Fresh Ginger) 10g, Dazao (Jujube Fruit) 2 pieces, Hangshao (Hangzhou Peony Root) 10g.
Commentary: Among Tao's five children, the eldest and second sons could not escape despite isolation, dying due to mistaken cold-natured medicines. The third and fourth sons progressed to "Pneumonia" severely but survived due to Yang-returning and counter-reversal agents. The girl was treated appropriately with mild medicines in two or three doses. This shows that treating measles requires careful and prudent formulation and medication selection.
Li Nian'e of the Ming Dynasty, in his commentary on *Yin Yang Ying Xiang Da Lun* within *Neijing Zhi Yao*, regarding "Lesser Fire generates Qi," stated: "One must skillfully regulate; those in the world who adeptly use bitter-cold substances and favor destructive methods—are they not sinners against Qibo and Huangdi?" These are words for physicians to memorize diligently.
Three cases of critical deficiency-cold syndrome following measles progressing to "Pneumonia"
Case 1:
Son of Luo XX, 3 years old, residing in Ruyi Alley, Kunming. The child contracted measles in March 1939 and was hospitalized. Due to progression to pneumonia, the condition became severe. The hospital deemed it critical and hopeless, discharging him home. I was then consulted. Although the measles had erupted, the pulse was deep, fine, and tight; the face showed cyanotic-darkness; lips and mouth were purple-black and scorched-dry; tongue coating was white, slippery, thick, and greasy; fingerprint was cyanotic-purple extending past the two gates; stools were watery diarrhea; urine was short and red. Coughing and wheezing were prominent, voice hoarse, fever in afternoon and night, poor appetite and spirit. Based on these symptoms, it had become a critical deficiency-cold syndrome of Taiyang and Shaoyin. Cold-phlegm and pathogenic Yin obstructed internally, insufficient Yang could not circulate, resulting in the above signs. Immediately, Sini Tang (Four Reversal Decoction) plus Rougui (Cinnamon Bark), Gongdingxiang (Public Clove), Faxia (Prepared Pinellia), and Fuling (Poria) was used to support Yang and suppress Yin.
Fupian (Processed Aconite Root) 36g, Ganjiang (Dried Ginger) 15g, Gancao (Licorice Root) 6g, Gongdingxiang (Public Clove) 3g, Faxia (Prepared Pinellia) 10g, Fuling (Poria) 15g.
Next day follow-up: Family reported immediate vomiting of considerable saliva/phlegm and several episodes of watery diarrhea upon taking medicine. Fever, cough, and wheezing reduced by 60-70%; child slept soundly; facial/lip cyanosis slightly receded; lips moister. Following original formula, Fupian increased to 60g, Dingxiang removed, Cangzhu (Atractylodes Rhizome) 6g added. Third day follow-up: Vomited phlegm twice more; fever subsided; urine longer; diarrhea stopped; wheezing ceased; could eat thin porridge. Facial/lip cyanosis receded 80-90%; lips turned red; white coating reduced by half; urine yellow but long; spirit/appetite increased; voice normalized; slight cough/wheeze remained. Sini Tang plus Sharen (Amomum Fruit) and Faxia taken for two doses.
Fupian (Processed Aconite Root) 60g, Ganjiang (Dried Ginger) 15g, Sharen (Amomum Fruit) 6g, Faxia (Prepared Pinellia) 10g.
Fifth day follow-up: Face turned red/rosy; tongue coating completely gone; cough/wheeze cured. Sini Tang plus Beiqi (Northern Astragalus) 16g, Lushen (Luodai Codonopsis) 13g, Sharen (Amomum Fruit) 6g, taken for two doses; appetite increased, spirit robust, all diseases cured.
Case 2: Daughter of Zhu XX, 5 years old, residing in Xiaodong Street, Kunming. Contracted measles in Spring 1939, hospitalized. After measles eruption, progressed to "Pneumonia," condition heavy, discharged home. Consulted me. Pulse deep and weak; face cyanotic-dark; lips/mouth pale-red and scorched; tongue coating white, thick, dry; no thirst; tidal fever at night; emaciated body and spirit; cough, shortness of breath, wheezing, abdominal pain, poor appetite. Based on symptoms, this belongs to innate insufficiency; after measles eruption, healthy Qi deficient and Yang weak, cold-dampness latent internally reversing upward to Lung, insufficient Yang unable to circulate. Method: Warm Middle, support Yang, raise Lung Qi, transform phlegm, stop cough. Main prescription: Sini Erchen Tang modified.
Fupian (Processed Aconite Root) 50g, Ganjiang (Dried Ginger) 15g, Faxia (Prepared Pinellia) 10g, Chenpi (Aged Tangerine Peel) 6g, Fuling (Poria) 12g, Rougui (Cinnamon Bark) 10g (powdered, soaked in water and mixed in), Sharen (Amomum Fruit) 6g, Xixin (Asarum) 3g, Wuwei Zi (Schisandra Berry) 2.5g, Gancao (Licorice Root) 6g.
Next day follow-up: Wheezing/cough reduced; lips/tongue moister; facial cyanosis slightly receded; diet slightly increased; night fever subsided. Take original formula for one more dose. Third day follow-up: Wheezing/cough stopped; spirit/diet increased; white coating receded 80-90%; lips/tongue red/rosy; facial cyanosis receded 70-80%. Continue Sini Tang plus Sharen, Rougui, Fuling for three doses; mouth full of fluids, appetite increased, spirit robust, all diseases cured.
Fupian (Processed Aconite Root) 50g, Ganjiang (Dried Ginger) 15g, Rougui (Cinnamon Bark) 6g (powdered, soaked in water and mixed in), Sharen (Amomum Fruit) 6g, Fuling (Poria) 12g, Gancao (Licorice Root) 6g.
Case 3:
Yang XX, residing in Dalvshuihe, Kunming. A female infant, half-year-old, contracted measles in Spring 1958. Rash faded, suddenly progressed to "Pneumonia," fever, wheezing/cough, throat phlegm rales, alar flaring, face showing cyanotic signs, fingerprint cyanotic-purple past two gates, stool greenish-water diarrhea, urine short/red. This is post-measures Yuan Yang internal deficiency, cold-phlegm obstruction, disconnect between Lung and Kidney Qi, failure of descending clarity. Prescribed Xiao Qinglong Tang (Minor Blue Dragon Decoction) plus Fupian.
Fupian (Processed Aconite Root) 30g, Ganjiang (Dried Ginger) 12g, Faxia (Prepared Pinellia) 6g, Xixin (Asarum) 3g, Mahuang (Ephedra) 3g, Wuweizi (Schisandra Berry) 1.5g, Guijian (Cinnamon Tip) 10g, Hangshao (Hangzhou Peony Root) 6g, Gancao (Licorice Root) 6g.
Immediately vomited about a cup of saliva/phlegm. Next day follow-up: Wheezing/cough slightly reduced, fever halved. Prescribed Sini Erchen Tang plus Rougui and small amount of Zhima Rong (Honey-processed Ephedra Downy Part), Xixin.
Fupian (Processed Aconite Root) 50g, Ganjiang (Dried Ginger) 12g, Faxia (Prepared Pinellia) 6g, Chenpi (Aged Tangerine Peel) 6g, Fuling (Poria) 13g, Rougui (Cinnamon Bark) 20g (powdered, soaked in water and mixed in), Gancao (Licorice Root) 10g, Zhima Rong (Honey-processed Ephedra Downy Part) 3g, Xixin (Asarum) 2.5g.
After ingestion, vomited considerable phlegm; wheezing/cough reduced 80-90%; alar flaring/phlegm rales stopped; stool turned yellow and loose; urine longer and pale yellow; slight thin porridge intake; facial/fingerprint turned red/rosy. Follow original formula, removing Mahuang, Xixin, Chenpi, take for two doses to cure.
Commentary: "Pneumonia" is a Western medical term; TCM classifies it into Lung Heat, Lung Cold, or Lung Dryness syndromes. Formulas must be flexible based on actual cold/heat/deficiency/excess conditions. If "Pneumonia" is seen without differentiating cold/heat, and cooling/antidote-clearing agents are arbitrarily used for anti-inflammation, significant delay ensues. The above three cases were all constitutional deficiency-cold with abundant damp-phlegm, forming severe cold-extreme pneumonia; thus, Yang-supporting and warming-transforming agents were main treatments, achieving complete success. If it were heat-toxin Lung Dryness inflammation, nourishing-Yin, clearing-Lung, generating-fluids, and moistening-dryness agents should be used to be effective.
Measles followed by Evil Heat Scorches Lung
Boy of Wang XX, 3 years old. After measles eruption, persistent fever, wheezing/cough with phlegm stagnation, scorched lips, white thick dry tongue coating, purple-red fingerprint, surging-numerate pulse, high fever with vexation/thirst, short red urine, dry nose, excessive eye discharge. This is post-measures True Yin internal deficiency, evil heat scorching Lung, fluid exhaustion and dryness. Prescribed Ren Shen Bai Hu Tang (Ginseng White Tiger Decoction) modified.
Mi Yangshen (Rice-Cultivated American Ginseng) 5g, Sheng Shigao (Raw Gypsum Fibrosum) 12g, Zhimu (Anemarrhena Rhizome) 10g, Gancao (Licorice Root) 6g, Cundong (Ophiopogon Tuber) 10g, Jianmu (Zanthoxylum Seed? Note: Likely typo for Zhimu or similar, kept as Jianmu) 3g, Wuwei Zi (Schisandra Berry) 5g, Jingmi (Glutinous Rice) a pinch.
Next day follow-up: Pulse calm, body cool, vexation/thirst/cough reduced. Prescribed Sheng Mai San (Generate Pulse Powder) modified.
