Jin, male, 68 years old. On the 20th day of the 3rd month in the Guiyou year. The patient had a chronic history of Phlegm-Fluid Retention (Tan Yin), with occasional episodes of vomiting. This episode still manifested as signs and symptoms of Phlegm-Fluid Retention. However, the attending physician failed to recognize this and instead used Bitter-Cold herbs to "firm the Yin" (a misapplication where bitter-cold properties damage Yang rather than nourish Yin). No wonder there was nothing left to sustain the body. Based on the symptom of "vomiting immediately after eating," it is determined to be a case of "Lack of Fire" (deficiency of Spleen-Stomach Yang and Mingmen Fire).
Prescription: Dan Wu Zhu Yu (Evodiae Fructus, lightly processed) (5 qian), Sheng Yi Yi Ren (Coicis Semen, raw) (6 qian), Gan Jiang (Zingiberis Rhizoma, dried) (5 qian), Jiang Zhi (Zingiberis Rhizoma juice, added to each dose) (3 spoons), Ban Xia (Pinelliae Rhizoma) (8 qian), Guang Pi (Citri Reticulatae Pericarpium) (3 qian).
Decoct 5 cups of water to obtain 2 cups. Take half in the morning and half in the evening. Decoct the dregs again with water to obtain 1 cup, and consume the entire first dose's liquid.
On the 23rd day. The previous prescription has already shown efficacy. However, the pulse is slow and tight. Continue with methods to unblock and warm the Stomach Yang.
Prescription: Dan Wu Zhu Yu (Evodiae Fructus, lightly processed) (3 qian), Sheng Jiang (Zingiberis Rhizoma, fresh) (5 slices), Yi Yi Ren (Coicis Semen) (3 qian), Ren Shen (Ginseng Radix et Rhizoma) (1.5 liang), Fu Ling (Poria) (2 qian), Ban Xia (Pinelliae Rhizoma) (3 qian). Do not restrict the number of doses; adjust according to symptoms.
Heng, female, 27 years old. Initially, due to severe fright, Liver Qi rebelled upward, causing frequent vomiting. Subsequently, due to incorrect use of tonifying herbs, severe continuous vomiting occurred, so intense that she attempted suicide by cutting her throat with scissors due to unbearable distress. Gradually, she could not swallow even rice grains. Her body became emaciated like firewood, and her breath was faint and weak. Nevertheless, she still experienced intermittent dry heaves. Her four limbs were ice-cold, and her pulse was wiry, fine as a thread, yet forceful. Treatment followed the method of Wu Mei Wan.
Prescription: Chuan Jiao Tan (Sichuan Pepper Charcoal) (4 qian), Huang Qin Tan (Scutellariae Radix Charcoal) (1 qian), Jiang Zhi (Zingiberis Rhizoma juice, added to dose) (3 spoons), Ban Xia (Pinelliae Rhizoma) (4 qian), Ya Lian (Coptidis Rhizoma, stir-fried with ginger juice) (2 qian), Wu Mei Rou (Mume Fructus pulp) (5 qian), Liao Shen (Ginseng Radix et Rhizoma from Liaoning) (3 qian), Wu Zhu Yu (Evodiae Fructus) (3 qian), Yun Ling Kuai (Poria chunks) (5 qian).
After taking two doses, she could consume rice soup. After four doses, she could eat congee. After seven doses, she was completely cured. Later, treatment focused on harmonizing the Liver and Stomach until full recovery and peace were achieved.
Zhou, male, 65 years old. On the 25th day of the 10th month in the Jiazi year. In elderly patients, slight Yang deficiency leads to the accumulation of turbid Yin, resulting in Chest Bi (obstruction in the chest) and Fan Wei (regurgitation/vomiting). The Yang of the Triple Burner is completely blocked. Prognosis is poor, but we propose first to unblock the clear Yang in the upper chest.
Prescription: Gua Lou (Trichosanthis Fructus) (2 qian), Xie Bai (Allii Macrostemonis Bulbus) (3 qian), Ban Xia (Pinelliae Rhizoma) (5 qian), Bai Mi (Apis Mellifera honey) (half wine cup), Gui Zhi Jian (Cinnamomi Ramulus tip) (5 qian), Xiao Zhi Shi (Aurantii Fructus Immaturus, small size) (8 fen), Chuan Po (Magnoliae Officinalis Cortex) (1 qian), Fu Ling (Poria) (2 qian), Jiang Zhi (Zingiberis Rhizoma juice) (3 small spoons).
Decoct 8 cups of water to obtain 3 cups. Take one-third three times a day.
On the 30th day. For the elderly patient with Yang deficiency and turbid Yin accumulation causing Fan Wei and Chest Bi, the method of opening and clearing Yang has already shown efficacy. However, vomiting of phlegm remains heavy. According to the principle that "vomiting after eating indicates Lack of Fire," treat with Zhu Yu Tang combined with Da Ban Xia Tang.
Prescription: Dan Wu Zhu Yu (Evodiae Fructus, lightly processed, soaked separately) (8 qian), Yang Shen (Panax quinquefolius Radix, stir-fried with ginger juice) (3 qian), Sheng Bai Mi (Raw Apis Mellifera honey) (one wine cup), Ban Xia (Pinelliae Rhizoma) (1 liang 2 qian), Sheng Jiang (Zingiberis Rhizoma, fresh) (2 liang).
Decoct 8 cups of water to obtain 3 cups. Take one-third three times a day. Decoct the dregs again with water to obtain half a bowl, and consume it.
On the 3rd day of the next month. Add to the previous prescription:
Fu Ling Kuai (Poria chunks) (5 qian).
On the 10th day of the next month. Remove Wu Zhu Yu from the previous prescription and add:
Xie Bai (Allii Macrostemonis Bulbus) (3 qian).
