Stroke resulting from severe illness has a high mortality rate and low survival rate. In treating phlegm, one must first regulate Qi; when Qi flows smoothly, phlegm dissipates spontaneously. In treating wind, one must first nourish Blood; when Blood circulates freely, wind extinguishes itself.
When the pathogen affects the Zang organs (Viscera), it is difficult to treat; when it affects the Fu organs (Hollow organs), it is easier to treat. If the pulse is slow and weak, accompanied by an open mouth, urinary incontinence, relaxed hands, stiff neck, upward rolling of eyes, snoring in the throat, emaciation, retroflexion of the head, and sweat droplets like pearls, these are all signs of difficult-to-treat conditions. Sudden loss of consciousness with mental clarity but inability to speak due to tongue stiffness: if the patient survives three to four months, blow powder made of Sichuan Bayberry Fruit (Glaucophyllum Fructus) and Raw Pinellia Rhizome into the nose. After regaining consciousness, administer Raw Pinellia Rhizome powder mixed with Ginger Juice to induce vomiting of phlegm and saliva, allowing the tongue to move again. For constipation, use honeyed dates to promote bowel movement, which will help restore Qi.
For stroke caused by deficiency of Yuan Qi (Original Qi), Baoyuan Decoction (Preserve Origin Decoction) with added Aconite Lateral Root (Prepared) is the main formula. Add blood-nourishing herbs for Blood deficiency, Qi-tonifying herbs for Qi deficiency, and phlegm-resolving herbs for excessive phlegm, modifying according to symptoms. Xiaoxuming Tang (Minor Continuation of Life Decoction) may also be used.
In stroke, patients who vomit profusely or suffer from urinary incontinence are considered untreatable. Those with wiry, slippery, forceful, or large pulses are also difficult to treat.
Restlessness after stroke is due to Qi deficiency failing to generate Blood, leaving the Heart without sufficient Blood nourishment.
The syndrome resembling stroke varies in type. Essentially, it is caused by the obstruction of Yang Qi and the disturbance of Ming (Spirit/Mind) by turbid fire. When Qi moves, vessels flow, and the five sense organs function correctly. Stagnation in the Liver causes eye disorders; stagnation in the Kidney causes ear disorders; stagnation in the Lung causes nasal disorders; stagnation in the Spleen causes oral disorders. Therefore, initially, Qi stagnation is regulated using formulas like Seven Qi Decoction (Seven-Air Decoction); subsequently... [Text cuts off]
Hemiplegia must be distinguished by left and right sides, generally treated with Shiquan Dabu Decoction (Ten Complete Tonification Decoction). At the onset, add Notopterygium Root (Qianghuo) and Saposhnikovia Root (Fangfeng) for three to five doses. For left-sided hemiplegia, use herbs that regulate Qi within Blood; for right-sided hemiplegia, use herbs that regulate Blood within Qi. All cases should be managed with this decoction, while pills and powders should be based on Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) with modifications.
Left-sided hemiplegia is referred to as "Qi within Blood" because the Left Guan position corresponds to Liver Wood, which represents ascending and generating Qi. Liver Wood generates Heart Fire, and the Heart governs Blood, hence "Qi within Blood." Right-sided hemiplegia is referred to as "Blood within Qi" because the Right Guan position corresponds to Spleen Earth, the source of Blood generation. Earth generates Lung Metal, and the Lung governs Qi, hence "Blood within Qi." The cause of immobility is primarily Liver Blood exhaustion leading to internal wind and stirring of fire; treatment should focus on nourishing... [Text cuts off]
