Qiangshi Ya Hou (Choking and Hoarseness Throat)
This syndrome is caused by latent pathogenic factors lodged in the Lung, resulting in hoarseness and choking during swallowing. The six pulses are slow and fine, indicating a very critical condition. I once treated a patient who was nearly twenty years old and had suffered from this for three years. His food intake was minimal, consuming only congee daily, with his condition approaching a critical state when he sought my treatment. Upon palpating his pulse, although the disease had persisted for a long time, the pulse still had a root, suggesting it might be treatable. I prescribed Liuwei Tang (Six-Ingredient Decoction) with additions: Ephedrae Herba (Mahuang, 麻黄) (2 qian), Cinnamomi Ramulus (Guizhi, 桂枝) (1 qian), Perillae Folium (Suye, 苏叶) (2 qian), Akebiae Caulis (Mutong, 木通) (1 qian), Asari Radix et Rhizoma (Xixin, 细辛) (1 qian), Angelicae Dahuricae Radix (Baizhi, 白芷) (1 qian), Terminalia Chebulae Fructus (Hezi, 诃子) (2 qian), Gleditsiae Sinensis Spina (Zaohé, 皂核) (2 qian), Pinelliae Rhizoma Praeparatum cum Zingibere et Melle (Jiangzhi Chao Banxia, 姜汁炒半夏) (2 qian). After taking this continuously for five to six days, his food intake increased to three bowls, but the hoarseness remained. The prescription was then modified: Platycodonis Radix (Jiegeng, 桔梗) (1 liang 4 qian 5 fen, stir-fried with child's urine), Terminalia Chebulae Fructus (Hezi, 诃子) (7 qian 5 fen, stir-fried with child's urine), Glycyrrhizae Radix et Rhizoma (Gancao, 甘草) (7 qian 5 fen, stir-fried with child's urine), Menthae Haplocalycis Herba (Bohe, 薄荷) (1 qian), Ephedrae Herba (Mahuang, 麻黄) (1 qian). The decoction was used for gargling and oral administration; after ten doses, he recovered. Subsequently, tonifying herbs were taken to strengthen the Spleen and conclude the treatment.
Neiwai Zhong Hou (Internal and External Swollen Throat)
This lesion occurs in the lower part inside the throat gate. It results from the knotting of Yin and Yang Qi, causing swelling both internally and externally, possibly accompanied by ulcerative spots and symptoms of Fire stagnation. Use Liuwei Tang (Six-Ingredient Decoction) with additions: Scutellariae Radix stir-fried with wine (Huangqin, 黄芩) (3 qian), Prepared Rheum Palmatum Radix (Shu Dahuang, 熟大黄) (5 qian), Pumex (Haifushi, 海浮石) (2 qian). Apply external powder "Zixue Jinbuhuan" (Purple Snow Gold Exchange). The next day, switch to a modified formula: Moutan Cortex (Danpi, 丹皮) (1 qian 5 fen), Rehmanniae Radix Preparata (Shengdi, 生地) (2 qian), Scutellariae Radix stir-fried with wine (Huangqin, 黄芩) (2 qian), Gypsum Fibrosum (Sheng Shigao, 生石膏) (3 qian), Gardeniae Fructus (Shanzhi, 山栀) (1 qian), Akebiae Caulis (Mutong, 木通) (1 qian). Simultaneously, needle the Shaoshang (LU11) and Shangyang (LI1) acupoints on both hands, totaling four points. If accompanied by coldness in the back, add Notopterygii Rhizoma et Radix (Qianghuo, 羌活); if there is stomach reflux, add Puerariae Radix (Gegen, 葛根); juice from Platycladi Cacumen (Baizhi, 柏枝) can also be used for gargling.
Fengre Hou (Wind-Heat Throat)
This syndrome arises from contracting Wind-Heat. The entire throat presents with fine red spots, with roots showing pale white coloration. There are three or four patches on either side under the tongue. The six pulses are surging and tight. Use Liuwei Tang (Six-Ingredient Decoction) with additions: Scrophulariae Radix stir-fried with salt water (Xuanshen, 玄参) (2 qian), Scutellariae Radix stir-fried with wine (Huangqin, 黄芩) (2 qian), Gardeniae Fructus (Shanzhi, 山栀) (1 qian), Polygonati Rhizoma (Huafen, 花粉) (1 qian). Recovery occurs after one dose. Apply external powder "Jinbuhuan" (Gold Exchange) and simultaneously administer Baxian San (Eight-Immortals Powder).
