The throat presents with swelling and protrusion resembling a melon stalk; this condition must not be punctured with a needle, as it will injure the patient. Crush red salt into powder, dip a chicken feather into it, and apply to the affected area to eliminate the swelling.
Fleshy growths appear in layers within the throat, gradually swelling up with pores emitting foul odor. Frequently decocting and drinking leaves of *Aegiceras corniculatum* (Chouju Ye) will lead to recovery.
Throat carbuncle (Hou Yong) arising at the Adam's apple is also called "Meng Ju" due to its fierce and terrifying toxic nature. It belongs to the Ren Mai (Conception Vessel) and the Hand Taiyang and Hand Shaoyin meridians, caused by accumulated heat and resentment/anger. If left untreated for too long, leading to perforation into the pharynx, death ensues.
An abscess developing in the pharynx is named "Meng Ju". If untreated, it turns into pus; if the pus cannot drain and blocks the throat, death occurs within half a day. If pus has formed, draining it combined with consuming pig fat (Shi Gao) coldly for three days results in healing.
Throat carbuncle occurring between the Adam's apples is named "Hai Men Di Yi Guan" and is extremely malignant. It is caused by heart and liver fire flaring upward and spleen-lung toxic vapors attacking the throat. Knife and needle procedures are strictly contraindicated.
Redness, swelling, blood stasis, blockage, and pain in the throat belong to branch disease (Biao Bing). Spontaneous discharge of pus leads to self-healing. One should use Jin Suo Chi (Golden Lock Key) to induce vomiting of phlegm and saliva, pushing out accumulated heat. If there is distending pain from pus, incision is required; if throat carbuncle does not discharge pus, it is contrary to proper treatment principles.
Throat carbuncle located centrally on the outer throat causes swelling and pain that hinders eating, accompanied by redness, swelling, and fever. If rupture of pus is desired and the area feels soft with distending pain, acupuncture may be used. Internally administer tonifying formulas, and externally apply Yu Hong Gao (Jade Red Ointment). After muscle repair, if there is ulceration allowing internal fluids to leak out through the hole, I have treated several such cases without adverse effects.
Accumulated heat causing throat carbuncle presents with swelling like a yellow sugar plum with red veins on the surface. External symptoms include neck pain and toothache. This occurs because the Stomach Meridian is affected by heat, and stomach qi connects to the throat. Treatment involves first inducing expectoration of phlegm, then insufflating medicine with Yuan Ming Fen (Mirabilite); decoctions should include Dang Gui (Angelica Sinensis), Huang Qi (Astragalus), and doubled Pian Qin (Scutellaria Baicalensis).
This toxicity of throat carbuncle arises from emotional stagnation. If toxins develop in the throat and are not treated promptly, toxic vapors may attack internally, damaging the hyoid bone. Ulceration leads to oral pus discharge; although life-threatening, it becomes a chronic, cold, stubborn disease. Although swollen, the throat gate is half-blocked; though dangerous, the airway is both wide and tight. These are critical conditions but generally not fatal.
Throat carbuncle triggered by excessive consumption of spicy, roasted, rich foods, and strong alcohol involves heat pathogen affecting the Lung system. The throat appears formless, only red, swollen, and painful. Deficiency patterns may also present with fever, resolving in four to five days.
Treatment for throat carbuncle uses Bi Dan (Green Elixir) mixed with a small amount of Jin Dan (Golden Elixir) insufflated internally, along with Gao Zi Yao (Pill Paste Medicine) or decoctions.
Tongue-Throat Carbuncle: Obese individuals prone to dry heat often suffer this. Small tongue-like growths under the tongue are called "Double Tongue" (Chong She), connecting to throat swelling and pain, triggering carbuncle. Generally, concurrent presentation indicates severe prognosis.
For Double Tongue-Throat Carbuncle with constipation, add Yuan Ming Fen (Mirabilite) and Da Huang (Rhubarb) to decoctions; for difficult urination, add Liu Yi San (Six-to-One Powder).
Tongue-Throat Carbuncle is treated with Modified Xi Jiao Di Huang Tang (Rehmannia Decoction with Rhinoceros Horn). Insufflate Jin Dan (Golden Elixir) at the root and sides of the tongue continuously. For the throat, frequently insufflate Bi Dan (Green Elixir) and Shi Jin Dan (Ten Golden Pellets).
Throat fungus (Hou Jun) arises from melancholy, blood heat, and qi stagnation, commonly affecting women. It resembles floating duckweed, slightly elevated, purple-red, growing beside the throat. Mild cases resolve in half a day to twenty days; severe cases take over a month. Success depends on correct treatment and patient compliance with dietary restrictions.
Initial treatment for Throat Fungus: Insufflate 5 fen Bi Dan (Green Elixir) and 2 fen Jin Dan (Golden Elixir); later use 3 fen Jin Dan and 3 fen Bi Dan. Take pill paste medicines frequently without interruption.
The throat is the gateway of life and respiration and must not be neglected. Gangrene in the throat requires early intervention; within one or two days, life hangs in the balance. Mild cases progress slowly, severe ones rapidly. Use Liang Ge Jie Du (Cool the Chest and Resolve Toxicity) methods.
Persistent pain and ulceration in the throat that do not heal are likely due to Yang Mei Chuang (Syphilis) toxin. Treat primarily with Bi Xie Tang (Dioscorea Hypoglauca Decoction), adding other herbs based on symptoms.
Guo Qiao Gan (Bridge Gangrene) forms below the throat above the windpipe, invisible to the eye and unreachable by insufflation, hindering eating. This is Yang Mei Jie Du (Syphilitic Toxin Knot) in the Lung and Stomach.
Throat tumor (Hou Liu) grows on either side of the throat, single or double, resembling large blood vessels without eyes, fruit-like tumors. Caused by Lung meridian heat, excessive speech damaging qi, anger/shouting, rapid reading, excessive wine, or roasted foods. Calm the spirit and rest; treat with medicine to cure completely. Do not puncture with needles.
Throat tumors on either side, single or double, sometimes broad-based and narrow-necked. Pain occurs only when provoked by anger or shouting; otherwise, no pain.
Throat Node (Hou Jie): Originates in the Jiwei region, initially feeling like a plum pit stuck between the throat and diaphragm, unable to swallow or spit out. After three days, it rises to the throat node area. Must be lanced, then insuffilate Dan Xiao Dan (Gall Nitre Elixir) into the throat, followed by taking seven pills of Xiong Huang Hua Du Dan (Realgar Antidote Pill).
Mei He Qi (Plum Pit Qi) is phlegm-qi knotting in the throat, unable to swallow or spit out, feeling like grass scratching and itching. Initially causes acid regurgitation and obstruction; later leads to complete blockage.
Phlegm-qi nodules in the throat resembling a stick are Mei He Qi, suitable for Qin Hua Wan (Holding Melt Pill).
*Jin Gui Yao Lue* states: Women feeling a lump like roasted meat in the throat are treated with Ban Xia Hou Po Tang (Pinellia and Magnolia Bark Decoction). "Zhi Luan" means dried meat. The throat feels sticky with roasted meat sensation, neither spittable nor swallowable. This is not due to intestines/stomach, so digestion/urination are unaffected; not due to exterior pathogens, so no body ache/chills. It is caused by accumulated cold injuring harmony with blood, causing qi overflow floating to the throat, condensing with dampness. Men occasionally suffer this too.
Suo Hou Chuang (Lock-Throat Sore) arises from Heart meridian toxic vapors and Small Intestinal wind at Ting Hui, spreading to Xuan Ying. Starts like scrofula, preventing eating, blocking passage, swelling and suppurating. Early treatment saves life; use Dang Gui (Angelica) and Lian Qiao (Forsythia). Apply Bing Pian San (Borneol Powder) with Ting You (Longhorn Beetle) and Bing Pian (Borneol)/She Xiang (Musk) crushed and applied.
Tian Bai Yi Chuang (White Ant Sore) involves internal throat ulcers and rotten nostrils. Not recorded in classic texts, often misdiagnosed as Hou Feng (Throat Wind). Caused by Lung-Spleen accumulated toxin; treat with Chuan Shan Jia San (Pangolin Scale Powder). Hou Xuan (Throat Lichen) is similar to Fei Hua Chuang (Lung Flower Sore).
Hou Xuan (Throat Lichen) results from deficiency fire flaming upward and excessive Lung Metal attacking the jaw joint, forming cracks like Celadon ware or red veins on autumn leaves. Hinders eating, causes sore throat. Though not immediately fatal, long-term loss of voice is incurable. Insufflate Bi Dan (Green Elixir), take pill paste, and decoctions with Bei Mu (Fritillaria) to descend qi. Observe taboos. Long-term Yin deficiency cough with throat pain indicates Fei Hua Chuang (Lung Flower Sore); treat with Kan Li Tang (Kan-Li Decoction) plus Xuan Shen (Scrophularia), Jie Geng (Platycodon), Gan Cao (Licorice). Avoid Borneol insufflation as acrid dispersal may worsen ulceration.
Phlegm-fire consumptive cough with throat pain is incurable.
Fei Hua Chuang (Lung Flower Sore) in the throat, colloquially "fish scale thorns," is difficult to save.
Hou Ci (Throat Thorn) often follows severe consumptive disease with rising deficiency fire and depleted nutritive blood. Red dots densely packed like mosquito bites on the throat indicate death; do not treat.
Some develop Hou Xuan inside the throat gate causing pain. Symptoms start with unbearable itching, red face, hot ears, followed by dryness upon swallowing saliva. Long-term formation causes pain, transforming into Yang Mei Si Luo (Syphilitic Thread Scrofula) or lichen. Lichens imply parasites in the throat due to Kidney Water depletion causing Kidney Fire rushing up while Lung Metal dries. Treat parasites and lichen simultaneously to strengthen righteousness and scatter evil. Use Hua Xuan Shen Dan (Transform Lichen Divine Elixir), followed by Run Hou Tang (Moisten Throat Decoction).
